Deaths: leading causes for 1999.

OBJECTIVES: This report presents final 1999 data on the 10 leading causes of death in the United States by age, race, sex, and Hispanic origin. Leading causes of infant, neonatal, and postneonatal death are also presented. This report supplements the annual report of final mortality statistics and responds to an increasing volume of requests by data users for leading-cause tables with more age and race detail than previously published. METHODS: Data in this report are based on information from all death certificates filed in the 50 States and the District of Columbia in 1999. Causes of death

Clinical and microbiological epidemiology of Streptococcus pneumoniae bacteraemia.

A survey of Streptococcus pneumoniae bacteraemia in the Grampian region of Scotland was carried out over a 2-year period. One hundred and four bacteraemic episodes were identified in 103 patients, an incidence of 9.8/100,000 population/year, and the mortality was 24%. Clinical information was abstracted from 92 sets of patient notes and 98 isolates of S. pneumoniae were available for further study. The incidence of S. pneumoniae bacteraemia was highest at the extremes of age and peaked at 78 cases/100,000 population/year in those over 80 years old. Many patients had predisposing conditions, of

A six-year epidemiologic review of pneumonia in Department of Veterans Affairs facilities.

OBJECTIVE: Pneumonia and acute lower respiratory infections are a major problem in the United States and worldwide. As one of the largest health care organizations in the United States, the Department of Veterans Affairs is an ideal location for an epidemiologic review of pneumonia over an extended period of time. METHODS: Data for this study were retrieved from the Department of Veterans Affairs Austin Automation Center, the central repository for patient data in the Veterans Health Administration (VHA). In addition, specific data regarding penicillin-resistant Streptococcus pneumoniae in VH

Royal Society of Tropical Medicine and Hygiene Meeting at Manson House, London, 12 December 1996. HIV and pneumococcal infection in Africa. Microbiological aspects.

By the 1930s several studies had shown that Streptococcus pneumoniae was an important pathogen in Nairobi (Kenya) and various risk factors for infection were recognized, including seasonally cold conditions, overcrowding and recent arrival in the city. Research into pneumococcal disease declined with the arrival of penicillin but recently interest has been rekindled by recognition of the pneumococcus as a human immunodeficiency virus (HIV)-associated pathogen and by the emergence of antibiotic resistance. The pneumococcus and its association with HIV were studied during the course of the Wellc

[Pneumonia complicated by bacteremia with Stomatococcus mucilaginosus].

A rare strain of Stomatococcus mucilaginosus (formerly Micrococcus) was isolated from blood of a patient after pneumonia, which was causing an enlargement of lymph nodes and groin fistula. Chemical composition of cell wall of this microorganism (analysis of amino acids and diagnostically important sugars), elementary composition of lipids (fatty acids, phospholipids and glycolipids) and molar percentage of guanine and cytosine (%GC), were determined. Results completely agreeable with results published elsewhere were obtained which resulted in reclassification of this species (Kocur (1982, Int.

Clinical profile and pattern of infection in Ethiopian children with severe protein-energy malnutrition.

The results of a prospective study on the clinical profile and pattern of infection in 90 Ethiopian children with severe protein-energy malnutrition (PEM) is presented. The study group consisted of 44(49%) with marasmus, 29(32%) with marasmic-kwashiorkor and 17(19%) with kwashiorkor. Their age ranged from 4 to 60 months and the median age at admission was 11.5, 15 and 20 months, respectively. Over 80% of the patients were infected and the lungs were the commonest sites. Bacterial pathogens, predominantly Gram negative enteric organisms, were isolated from 36% of blood and 37% of urine specimen

An outbreak of serotype 1 Streptococcus pneumoniae infection in central Australia.

An outbreak of serotype 1 Streptococcus pneumoniae infection involving both adults and children occurred in central Australia during the winter months of 1991. Eighteen patients, mainly Aboriginal men, presented with culture-positive serotype 1 bacteraemic pneumonia. In this group, 11 of 12 adults for whom medical records were available were alcohol dependent. Thirteen children who were separately studied were hospitalised with acute lower respiratory tract infection: none had bacteraemia but all had upper airway colonisation by type 1 pneumococci. Antibiotics taken by 8 of the 13 children bef

Childhood acute bacterial meningitis in the Sudan: an epidemiological, clinical and laboratory study.

The aims of the present study were to document the epidemiology, clinical features and complications of childhood acute bacterial meningitis (ABM) in The Sudan during both an inter-epidemic (endemic) period (1985-1986), and the 1988 serogroup A epidemic; and to examine the phenotypic and genetic similarities and differences of Neisseria meningitidis strains isolated in The Sudan and Sweden. A new enzyme immunoassay test (Pharmacia Meningitis EIA-Test) was evaluated as a potential rapid diagnostic method for the detection of Haemophilus influenzae (HI) type b, Neisseria meningitidis (MC) and St

Laboratory evaluation of an outbreak of nocardiosis in immunocompromised hosts.

Seven patients in a renal unit were proved to have nocardiosis in an interval of nine months. Six of these patients had received renal transplants. Serologic investigation suggested that two additional cases of undiagnosed pulmonary disease were also nocardial, and that there were no subclinical cases in patients or staff. Clinical-serologic correlations indicate that serologic evaluation may be a useful adjunct in diagnosis of nocardiosis, if used early and repeatedly, and to follow response to therapy. Epidemiologic investigations yielded cultures of Nocardia asteroides from air and dust ins

High prevalence of penicillin-insensitive pneumococci in Port Moresby, Papua New Guinea.

One-third of 57 strains of pneumococci isolated from patients with severe pneumococcal infections in Port Moresby, Papua New Guinea, in 1978 were penicillin-insensitive (PR). The infections were community-acquired and comprised cases of bacteraemia, bacteraemic pneumonia, and meningitis. The prevalence of invasive PR strains in children was significantly greater than in adults. Twenty-one serotypes were identified, of which ten types (6, 9, 10, 12-14, 18, 19, 23, 24) showed diminished sensitivity to penicillin; with types 12 and 18 penicillin-insensitivity has not been met with previously in P
