Optimizing Pneumocystis jirovecii Prophylaxis in Immunocompromised Patients: Defining the Clinical Role of Atovaquone.

BACKGROUND: Pneumocystis jirovecii pneumonia (PJP) is associated with poor outcomes in immunocompromised patients. Cotrimoxazole is the first-line prophylactic agent, with atovaquone an alternative. At our center, atovaquone use considerably increased after 2020. The aims of this retrospective cohort study were to understand the usage of atovaquone over time and explore the reasons for this increase. METHODS: Patients receiving atovaquone for PJP prophylaxis from 2015 to 2022 were included. Clinical data were extracted to characterize atovaquone prescribing patterns and cotrimoxazole contrain

Risk Factors and Outcomes of Pneumocystis jirovecii Pneumonia after Allogeneic Hematopoietic Stem Cell Transplantation for Hematological Malignancy: A 10-Yr Cohort Study.

Pneumocystis jirovecii pneumonia (PJP) is a life-threatening opportunistic infection that occurs after allogeneic hematopoietic stem cell transplantation (allo-HSCT). The risk factors and prognostic factors for PJP under current transplantation and monitoring practices are not well understood. In this retrospective cohort study, we aimed to characterize the clinical features, identify risk factors for its occurrence, and determine prognostic factors of PJP for survival. Between 2012 and 2022, 8596 patients underwent allo-HSCT at the Peking University Institute of Hematology. From this cohort,

Disseminated and fatal infection by Schizophyllum commune in a harbor seal (Phoca vitulina) kept under human care in Brazil.

BACKGROUND: Schizophyllum commune is an environmental basidiomycete increasingly recognized as an opportunistic pathogen in humans and domestic animals, whereas infections in wildlife remain rarely reported. In marine mammals, fungal infections involving the central nervous system and respiratory tract are uncommon and may pose diagnostic challenges. CASE REPORT: A captive 5-year-old female harbor seal (Phoca vitulina) from Brazil developed progressive neurological abnormalities over approximately 11 months, initially characterized by cervical dystonia and head deviation, followed by respirat

Copper-Catalyzed Direct Deaminative Borylation of Anilines - A Retro-Chan-Evans-Lam Reaction.

BACKGROUND: Serum (1,3)-beta-D glucan (BDG) is increasingly used to guide the management of suspected Pneumocystis pneumonia (PCP). BDG lacks specificity for PCP, and its clinical performance in high-risk cancer patients has not been fully assessed. Polymerase chain reaction (PCR) for PCP detection is highly sensitive, but cannot differentiate between colonization and infection. We evaluated the diagnostic performance of serum BDG in conjunction with PCP PCR on respiratory samples in patients with cancer and unexplained lung infiltrates. METHODS: We performed a retrospective analysis of adult

Normothermic regional perfusion (NRP) use in controlled donation after circulatory determination of death (cDCDD): results of the European Society for Organ Transplantation Bucharest consensus conference.

Normothermic Regional Perfusion (NRP) is emerging as a game-changer in enhancing outcomes for Donation after Circulatory Determination of Death (DCDD). NRP maintains physiological conditions through perfusion with oxygenated blood, outperforming conventional super-rapid recovery techniques significantly improving outcomes and organ utilization. Despite its clinical benefits, widespread adoption of NRP is impeded by heterogeneous organizational, legal, and ethical frameworks. At the ESOT Bucharest Consensus Conference, leading experts in transplantation achieved consensus on 130 relevant NRP-re

Suspected allergic bronchopulmonary candidiasis in a patient with concurrent multidrug-resistant bacterial pneumonia: A case report.

RATIONALE: Allergic bronchopulmonary candidiasis (ABPC) is a rare pulmonary hypersensitivity disorder; diagnosis is difficult because Candida albicans is usually a respiratory commensal. PATIENT CONCERNS: A 70-year-old man with hypertension and prior cerebral infarction was admitted for sudden right limb weakness (6.5 hours) and diagnosed with hemorrhagic cerebral infarction in the intensive care unit. DIAGNOSES: Persistent fever and pulmonary infiltrates developed despite broad-spectrum antibiotics. Total immunoglobulin E (IgE) was 2181 IU/mL; sputum grew C albicans + carbapenem-resistant P

DETECTX: a novel deep learning approach for detecting pneumonia, tuberculosis, and aspergillosis from chest X-rays.

BACKGROUND: Aspergillosis, pneumonia, and tuberculosis are distinct respiratory illnesses, but they share similar symptoms, including cough, fever, and shortness of breath. This overlap can make accurate diagnosis challenging. Delayed or incorrect identification, particularly of aspergillosis, can lead to worsening patient outcomes and significant lung damage. METHODS: To address this diagnostic challenge, we developed a deep learning based system designed to differentiate between aspergillosis, pneumonia, tuberculosis, and healthy lungs using chest X-ray images. We implemented three models:

Dual-subtype positivity of influenza A(H1N1) and A(H3N2) is associated with worse hypoxemia, fungal co-detection, and adverse short-term outcomes in adults with influenza-associated community-acquired pneumonia.

OBJECTIVES: Dual-subtype influenza A positivity is poorly characterized. We evaluated whether detection of A(H1N1) and A(H3N2) during the same admission identifies a high-risk phenotype in adults hospitalized with influenza-associated community-acquired pneumonia. METHODS: This retrospective single-center study linked clinical and bronchoalveolar lavage fluid pathogen-spectrum records. Influenza A was detected by RT-PCR and BALF pathogens by targeted next-generation sequencing. The primary endpoint was composite adverse hospital disposition (in-hospital death or discharge against medical advi

Diagnosing pathogenic DNA from Pneumocystis Jirovecii in exhaled breath condensate: a feasibility study.

BACKGROUND: Pathogen identification in pneumonia is achieved in less than 25% of cases. Diagnosis of pneumocystis pneumonia relies on a multimodal approach combining clinical, radiological, and immunological assessment with microbiological confirmation, most often by polymerase chain reaction. However, the method cannot reliably distinguish between active infection and colonization because of limited assay standardization and lack of universally accepted interpretive criteria. This pilot study explored whether Pneumocystis jirovecii DNA could be detected in patients with suspected infection (P

Optimized Graph Structures for Calibrating Graph Neural Networks with Out-of-Distribution Nodes.

BACKGROUND: Bone autograft options may be limited in revision spinal fusion cases. Reconstruction of the iliac bone graft (IBG) donor site with cancellous bone allograft allows for reharvest for patients who subsequently have another fusion. This study examined viability of the reconstructed IBG (RIBG) donor sites. Secondarily, we assessed the feasibility of whether the RIBG sites could be reharvested for obtaining a successful arthrodesis for a secondary fusion. METHODS: Prospectively collected data of 154 consecutive lumbar revision fusions were reviewed, of which 17 cases had their IBG don

Extracorporeal Membrane Oxygenation for Pneumocystis jirovecii Pneumonia in a Child With Multisystem Langerhans Cell Histiocytosis.

BACKGROUND: Pneumocystis jirovecii pneumonia (PJP) is a rapidly progressive and potentially fatal opportunistic infection in non-HIV immunocompromised children. Mortality is particularly high when PJP progresses to acute respiratory distress syndrome (ARDS). The role of extracorporeal membrane oxygenation (ECMO) in this setting remains limited to select reports. CASE PRESENTATION: We report a 3-year-old boy with multisystem Langerhans cell histiocytosis receiving vinblastine and corticosteroids who developed severe PJP during consolidation therapy. Despite early initiation of high-dose trimet

Blood mNGS: an effective non-invasive diagnostic tool for Pneumocystis jirovecii pneumonia.

BACKGROUND: Pneumocystis jirovecii pneumonia (PJP) is a life-threatening opportunistic infection. Colonization is prevalent but cannot be reliably distinguished from active infection by conventional methods. Metagenomic next-generation sequencing (mNGS) is a promising diagnostic tool, but the value of blood mNGS for diagnosis, microbial community comparison, and outcome-related associations in PJP remains unclear. METHODS: We analyzed 73 suspected PJP patients with paired BALF and blood mNGS. Using strict diagnostic criteria, patients were classified as: PJP (n = 50) and P. jirovecii coloniza
