Occurrence and Determinants of Periprosthetic Fractures of the Femur During Intraoperative and Post-operative Period in Patients Undergoing Hip Arthroplasty: A Prospective Cohort Study.

BACKGROUND: Stachybotrys chartarum is a damp building mould and a potent toxin producer that has been related to serious cases of respiratory health problems. However, the direct link between exposure and health symptoms has not been established. OBJECTIVE: To examine the mechanism by which exposure to spores of satratoxin producing and non-producing S. chartarum strains induce inflammatory responses in murine lungs. METHODS: BALB/c mice were intranasally exposed for 3 weeks to spores of a satratoxin-producing and a non-producing S. chartarum strain. Inflammatory cell infiltration was charac

A Quality Improvement Initiative to Standardize Pneumocystis jirovecii Pneumonia Prophylaxis in Pediatric Patients With Solid Tumors.

BACKGROUND: Pediatric patients with extracranial solid tumors (ST) receiving chemotherapy are at an increased risk for Pneumocystis jirovecii pneumonia (PJP). However, evidence guiding prophylaxis practices in this population is limited. A PJP-related fatality at our institution highlighted inconsistent prescribing approaches and concerns about prophylaxis-related toxicity, prompting the development of a quality improvement (QI) initiative to standardize PJP prophylaxis for patients with ST. PROCEDURE: Our quality improvement (QI) initiative aimed to increase PJP prophylaxis initiation rates

Longitudinal mycological profiling of influenza-associated and COVID-19-associated pulmonary aspergillosis.

OBJECTIVES: Influenza- and COVID-19-associated pulmonary aspergillosis (IAPA and CAPA respectively) are associated with increased mortality in critically ill patients. We evaluated whether longitudinal bronchoalveolar lavage (BAL) galactomannan (GM) dynamics predict clinical outcomes in these patients. METHODS: In a retrospective cohort (2009-2024) at a tertiary care ICU in Belgium, 180 adult patients with probable/proven IAPA (n = 68) or CAPA (n = 112) requiring mechanical ventilation were included. A total of 698 BAL samples were analysed. GM optical density values were modelled using linea

Distribution of capsule and O types in Klebsiella pneumoniae causing neonatal sepsis in Africa and South Asia: A meta-analysis of genome-predicted serotype prevalence to inform potential vaccine coverage.

BACKGROUND: Klebsiella pneumoniae causes ~20% of sepsis in neonates, with ~40% crude mortality. A vaccine administered to pregnant women, protecting against ≥70% of K. pneumoniae infections, could avert ~400,000 cases and ~80,000 deaths annually, mostly in Africa and South Asia. Vaccine formulations targeting the capsular polysaccharide (K) or lipopolysaccharide (O) antigens are in development. Global K. pneumoniae populations display extensive K and O diversity, necessitating a polyvalent vaccine targeted to the serotypes associated with neonatal disease in relevant geographical regions. Trichoderma harzianum fungemia following COVID-19-related immune dysregulation in an immunocompetent patient: a case diagnosed by mNGS.

BACKGROUND: Trichoderma harzianum is a filamentous saprophytic fungus rarely implicated in human infections. Invasive Trichoderma infections are uncommon and are typically observed in immunocompromised hosts. CASE PRESENTATION: We report a 68-year-old immunocompetent male farmer who developed persistent fever and dizziness after COVID-19 infection. Initial empirical antibacterial and antiviral therapy failed to relieve symptoms. Repeated blood cultures and serological fungal tests were negative, whereas metagenomic next-generation sequencing (mNGS) of whole blood identified T. harzianum sequen

Risk factors for adverse events associated with trimethoprim-sulfamethoxazole treatment for Pneumocystis pneumonia in non-human immunodeficiency virus-infected patients: a multicenter, retrospective, observational cohort study.

BACKGROUND: Pneumocystis jirovecii pneumonia (PCP) is a severe opportunistic infection. Trimethoprim-sulfamethoxazole (SXT) is the first-choice treatment for PCP in human immunodeficiency virus (HIV)-infected and non-HIV-infected patients. However, the high incidence of adverse events makes treatment with SXT difficult. The risk factors for these adverse events in patients with non-HIV PCP remain unclear. METHODS: In this multicenter, retrospective, observational cohort study, we investigated these risk factors by analyzing data from patients with non-HIV PCP treated with SXT between June 2006

Therapeutic potential of a polyherbal extract against COVID-19 and associated inflammation and mucormycosis: In vitro characterization and in vivo validation in a human coronavirus 229E-infected mouse model.

The rise of viral infections has been accompanied by an increased incidence of secondary opportunistic infections, such as mucormycosis, highlighting the urgent need for broad-spectrum therapeutics capable of targeting multiple pathological pathways. This study evaluates the antiviral, antioxidant, anti-inflammatory, and antifungal potential of a polyherbal extract (PHE) and its optimized self-nanoemulsifying intravenous formulation (PH-SNEDDS). PHE displayed strong free-radical-scavenging activity (DPPH, IC50: 21.99μg/mL; ABTS, IC50: 58.97μg/mL) and significantly downregulated pro-inflammator

Health Care-Associated Infections Among Neonates During the COVID-19 Pandemic.

IMPORTANCE: Neonatal intensive care units (NICUs) implemented enhanced infection prevention measures during the COVID-19 pandemic to interrupt viral transmission. It was hypothesized that these measures would also reduce nonviral health care-associated infections (HAIs). OBJECTIVE: To compare rates of viral and bacterial or fungal HAIs in infants admitted to the NICU before and during the pandemic. DESIGN, SETTING, AND PARTICIPANTS: This cohort study was conducted at 12 level 3 or level 4 NICUs in the US and Canada. Participants were inborn and outborn infants admitted from March 1, 2018, to

Evaluation of an in vivo pulmonary aspergillosis model for triazole susceptibility breakpoint development.

Use of mouse infection models for antimicrobial pharmacokinetic/pharmacodynamic (PK/PD) analysis can assist in dosing regimen design and susceptibility breakpoint development. A major hurdle for clinical translation of in vivo study output is defining the model endpoint linked to clinical success. Validation of the in vivo endpoint requires a clinical data set composed of success or failure linked to minimum inhibitory concentration (MIC), dosing regimen, and if possible human pharmacokinetic measures. The present studies utilized a clinical library of eight Aspergillus fumigatus strains in a

Intravenous trimethoprim-sulfamethoxazole desensitization as a therapeutic strategy for late-onset Pneumocystis jirovecii pneumonia in a kidney transplant recipient: a case report.

BACKGROUND: Pneumocystis jirovecii pneumonia is a potentially fatal opportunistic infection in kidney transplant recipients under intensified immunosuppression. Although most cases occur within the first post-transplant year, late-onset infections may develop after rituximab therapy or discontinuation of prophylaxis. Trimethoprim-sulfamethoxazole is the preferred agent for prevention and treatment, but allergy or intolerance limits its use. Desensitization protocols may allow reintroduction of this drug in selected patients. CASE PRESENTATION: A 37-year-old white female kidney transplant reci

Antifungal prophylaxis among critically ill COVID-19 patients: a meta-analysis and systematic review.

BACKGROUND: COVID-19-associated pulmonary aspergillosis (CAPA) affects a significant proportion of patients admitted to the ICU and is associated with increased mortality, underscoring the need for effective prophylaxis. While observational studies suggest a benefit from antifungal prophylaxis, its efficacy remains unconfirmed by randomized trials, and its impact on clinical outcomes is unclear. METHODS: We conducted a systematic review and meta-analysis (PROSPERO: CRD42023462988) of nine ICU-based studies, comparing antifungal prophylaxis (primarily inhaled amphotericin B or systemic posacona

Retrospective analysis of possible associations between Pneumocystis spp. and five immunosuppressive viral pathogens in three wild carnivore species.

Pneumocystis spp. proliferate under immunosuppressive conditions in mammalian lungs, and several pathogens have been discussed as potential contributors to fungal proliferation. This study aimed to investigate the possible associations between Pneumocystis spp. and immunosuppressive viruses in Serbian wild mammals. A total of 108 wild carnivores - including golden jackals (Canis aureus), Eurasian badgers (Meles meles), and red foxes (Vulpes vulpes) - were collected from Veliko Gradište, Stara Pazova, and Ugrinovci during the 2022/2023 hunting season. [Reappraisal of risk factors for invasive pulmonary fungal disease].

Invasive fungal disease (IFD) has historically been observed predominantly in neutropenic patients, particularly those with hematological malignancies or undergoing allogeneic hematopoietic stem cell transplantation. However, clinical practice has found that an increasing proportion of patients with IFD have underlying diseases such as chronic obstructive pulmonary disease, severe viral pneumonia, and diabetes mellitus, rather than neutropenia. These patients have significantly different risk factors and degrees of immunosuppression to neutropenic patients, which makes assessment more difficul
