We assessed the Pneumocystis jirovecii PCR accuracy on non-invasive oropharyngeal wash (OW) specimens for the diagnosis of Pneumocystis jirovecii pneumonia (PJP) in non-HIV immunocompromised patients. In this retrospective study, 134 patients with suspected PJP undergoing both OW and deep respiratory sampling were included. PJP was defined by composite criteria. OW P. jirovecii PCR showed a sensitivity of 61.5% (95% confidence intervals [CI] 42.5-77.6) and a specificity of 97.2% (95% CI 92.2-99.1). Positive and negative predictive values were 84.2% and 91.3%, respectively. These results sugges

BACKGROUND: Fungal infections significantly compromise the prognosis of patients with interstitial pneumonia with autoimmune features (IPAF). However, the specific immune-related risk factors and their impact on clinical stability remain poorly defined. This study aimed to identify independent predictors for fungal infection and early readmission to optimize risk stratification.
METHODS: We conducted a retrospective analysis of 98 patients meeting the 2015 European Respiratory Society/American Thoracic Society (ERS/ATS) IPAF classification criteria. Fungal infections were confirmed through cl

BACKGROUND Severe Pneumocystis jirovecii pneumonia (PJP) in renal transplant recipients (RTRs) can rapidly progress to acute respiratory distress syndrome (ARDS) and is associated with high mortality. Glucocorticoids (GCs) play a paradoxical role, constituting a risk factor for infection and a trigger for immune reconstitution inflammatory syndrome upon withdrawal; they may also serve as a therapeutic agent for lung injury. We evaluated the efficacy of a standardized triple-therapy regimen designed to address this paradox. CASE REPORT We analyzed 7 RTRs admitted to the intensive care unit (ICU

BACKGROUND: Invasive fungal infection (IFI) causes poor outcomes in haematological malignancies, but data in mature T-cell and NK-cell lymphomas (T/NKCL) are scarce. We aimed to define epidemiology and outcomes of IFI in this population.
MATERIALS AND METHODS: This retrospective study enrolled adult patients with T/NKCL between 2019 and 2024. Proven and probable IFI were defined according to the 2020 EORTC/MSGERC criteria. Multivariable logistic regression was used to identify factors associated with IFI and death.
RESULTS: Among 203 patients, 43 (21%) developed 52 IFI episodes, including in

OBJECTIVES: Pneumocystis jirovecii pneumonia (PCP) is a severe opportunistic infection increasingly affecting HIV-uninfected immunocompromised patients. Although bronchoalveolar lavage fluid (BALF) PCR is highly sensitive, distinguishing infection from colonisation remains challenging. We evaluated the diagnostic performance of BALF quantitative PCR (qPCR) cycle threshold (Ct) values (BioGX, BD MAX) and an integrated diagnostic strategy combining qPCR and serum β-D-glucan (BDG) (Toxinometer MT-6500 system).
METHODS: We conducted a retrospective diagnostic accuracy study including all BALF P.

Serum 1,3-β-D-glucan (BDG) showed high negative predictive value (~99%) for Pneumocystis jirovecii pneumonia (PJP) in 106 non-human immunodeficiency virus immunocompromised children, but modest positive predictive value at low prevalence. These exploratory findings suggest BDG may help exclude PJP, pending prospective validation.

BACKGROUND: X-linked hyper-immunoglobulin M (XHIGM) syndrome is a rare primary immunodeficiency caused by mutations in the CD40 ligand gene (CD40LG), characterized by defective T-cell-dependent B-cell class-switch recombination. Patients typically present with recurrent infections in early childhood, but diagnosis is often delayed due to heterogeneous clinical manifestations and the fact that serum IgM levels may remain within the normal range in a significant proportion of patients. Here we report a case of XHIGM in an infant whose diagnostic journey began with recurrent lymphadenitis and cul

Nearly all known cases of antimicrobial resistance have been linked to agents that directly target microbes. This paradigm overlooks other drivers of microbial evolution and resistance. Here, we report compelling evidence of resistance-associated mutations in Pneumocystis jirovecii, a high-priority fungal pathogen designated by WHO as having unknown resistance. These mutations emerged under selective pressure from mycophenolic acid (MPA), a widely used immunosuppressant not used for Pneumocystis pneumonia (PCP) treatment. A potent inhibitor of inosine monophosphate dehydrogenase (IMPDH), MPA h

BACKGROUND: Pneumocystis jirovecii pneumonia (PJP) is a life-threatening opportunistic infection in immunocompromised patients. A major clinical challenge is the inability of current microbiological diagnostics (e.g., quantitative polymerase chain reaction [qPCR]) to reliably distinguish PJP from Pneumocystis jirovecii colonization(P. jirovecii colonization).
OBJECTIVE: This study aimed to develop and validate a diagnostic model for differentiating PJP from P. jirovecii colonization by analyzing 17 core model input predictors that integrate pathogen burden and host immune response profiles.

RATIONALE: Coinfection with Pneumocystis jirovecii and Aspergillus fumigatus in immunocompromised patients carries high mortality. More importantly, paradoxical clinical and radiological responses during treatment remain poorly understood.
PATIENT CONCERNS: A 66-year-old male with mantle cell lymphoma who had received prolonged corticosteroid therapy after suspected rituximab-associated lung injury presented with progressive pulmonary symptoms.
DIAGNOSES: Concurrent pulmonary infection with P jirovecii and A fumigatus was confirmed by bronchoalveolar lavage combined with metagenomic next-gen

Male infertility (MI) and male sexual dysfunction (MSD) can often coexist together due to various interplay factors such as psychosexual, sociocultural and relationship dynamics. The presence of each form of MSD can adversely impact male reproduction and treatment strategies will need to be individualized based on patients' factors, local expertise, and geographical socioeconomic status. The Asia Pacific Society of Sexual Medicine (APSSM) and the Asian Society of Men's Health and Aging (ASMHA) aim to provide a consensus statement and practical set of clinical recommendations based on current e

The epidemiology of invasive fungal infections (IFIs) among patients receiving Bruton tyrosine kinase inhibitors (BTKIs) remains incompletely characterized. We conducted a systematic review and meta-analysis of 88 studies including 23 737 patients to evaluate the prevalence and risk factors for IFIs in this population. Among 16 studies that applied the European Organization for Research and Treatment of Cancer and the Mycoses Study Group Education and Research Consortium consensus definitions of IFIs, the pooled prevalence of proven/probable IFIs was 2.83% (95% confidence interval [CI], 1.97-4

BACKGROUND: Infectious pseudoaneurysms are rare, and those caused by Cryptococcus, an organism commonly found in pigeon feces, are exceptionally uncommon. This case highlights this severe vascular complication as a sequel of cryptococcal infection. CASE PRESENTATION: A 71-year-old male with a history of pigeon exposure presented with a persistent fever and was diagnosed with cryptococcal pneumonia and bacteremia. After successful initial antifungal treatment, a follow-up chest CT scan three months later revealed a pseudoaneurysm at the aortic arch. The patient underwent successful endovascular

Mucormycosis, triggered by fungi of the order Mucorales, represents a potentially fatal invasive mycosis, with death rates over 50% despite intensive therapy. The COVID-19 pandemic brought a sharp increase in cases, especially in individuals with diabetes mellitus and those undergoing immunosuppressive treatment, emphasizing significant gaps in our comprehension of disease pathogenesis. Emerging molecular studies have highlighted key virulence factors, such as the CotH family of invasins that facilitate endothelial invasion via interaction with glucose-regulated protein 78 (GRP78), complex iro

PURPOSE: Diabetes mellitus is recognised as a global health problem and has increasingly been associated with fungal infections, as an independent risk factor. Diabetic patients are predisposed to both superficial as well as invasive fungal disease, and account for substantial morbidity and mortality. Recent pandemic of COVID-19 underlined the global problem of mucormycosis, however, the burden of fungal infections among diabetics has a much broader horizon. This review aims to summarise the spectrum of fungal infections encountered among diabetic patients, along with elucidating immunopathoge

Laryngeal mucormycosis is an uncommon form of fungal laryngitis that typically affects immunocompromised individuals. We report a rare such case in a male in his 40s who had recently recovered from severe acute respiratory syndrome-coronavirus disease-19 (SARS CoV-2 infection-COVID 19) and presented with a 1-month history of worsening hoarseness, haemoptysis, and noisy breathing. A chewing gum-like subglottic necrotic mass was biopsied. The rarity of the diagnosis was aided by histopathological and microbial study of the mass, which confirmed mucormycosis. Management included surgical debulkin

We describe the case of a 48-year-old Belgian woman who developed fever, dry cough and dyspnoea shortly after returning from a two-week trip to Suriname. CT scan of the thorax revealed 2 nodular masses with surrounding ground glass alterations in the right lung as well as multiple hilar and mediastinal lymphadenopathies. Bronchoscopy with broncho-alveolar lavage and endobronchial ultrasound-guided transbronchial needle aspiration of thoracic lymph nodes were performed. Broncho-alveolar lavage fluid showed no abnormalities, including a negative PCR for Aspergillus spp., and negative cryptococca

BACKGROUND: Histoplasmosis is an endemic mycosis that primarily affects the lungs, commonly found in the central and eastern United States, particularly the Ohio and Mississippi River valleys. While disseminated histoplasmosis (DH) is typically associated with immunocompromised individuals, rare cases have been reported in immunocompetent hosts following high-risk environmental exposures, particularly in high-risk occupational settings.
CASE PRESENTATION: We report a case of a 22-year-old immunocompetent male employee of a municipal water department in Texas who presented with fever, pleuriti

Histoplasma capsulatum var. capsulatum is a thermally dimorphic fungus endemic in regions like the Midwestern USA, Africa, and Central/South America. It grows as a mold in the environment and as yeast in human tissues. The disease's severity and presentation depend mostly on the host's immunity. In immunocompromised individuals, it often causes progressive disseminated histoplasmosis (PDH), a severe systemic condition. PDH symptoms include pneumonia, enlarged spleen and liver, enteritis, pancytopenia, lymphadenopathy, skin and mucosal lesions, and joint pain, making the diagnosis a challenge.
