Generalized Megaesophagus Concurrent With Toxocariasis in a Cat.

Feline megaesophagus is a rare disorder characterized by esophageal hypomotility and dilation, resulting in regurgitation, weight loss, and occasionally death. This report describes the clinical and paraclinical findings of generalized megaesophagus with a possible contribution of toxocariasis in a kitten. A 2-month-old female cat was referred with chief complaints of lethargy, hyporexia, dyspnea, and vomiting/regurgitation. Physical examination of the kitten revealed bradycardia, bradypnea, hypothermia, dehydration, and lung crackles. The most notable laboratory results were leukopenia with s

Identification of a spatially organized TREM2-associated permissive macrophage niche in human pulmonary tuberculosis alveolar pneumonia.

Pulmonary tuberculosis (TB), caused by lung infection with Mycobacterium tuberculosis (Mtb), often manifests both alveolar pneumonia and aggregation of immune cells containing Mtb, termed granulomas, within the same lung. Here, we integrate spatial transcriptomics, single-cell RNA sequencing, high-resolution imaging, and functional assays of lung biopsies from four men with pulmonary TB to compare alveolar pneumonia with adjacent granulomas. We find that alveolar TB pneumonia is enriched for TREM2⁺ lipid-laden macrophages characterized by increased lipid metabolism gene expression, reduced T c

Bacterial infections and antibiotic susceptibility in children with sickle-cell anemia on malaria chemoprevention using Dihydroartemisinin-piperaquine (DP) or Sulfadoxine-pyrimethamine (SP) in Uganda.

INTRODUCTION: Over 300,000 babies are born annually with sickle cell anemia (SCA) in sub-Saharan Africa. Despite malaria chemoprevention and early penicillin prophylaxis, these children develop febrile illnesses. The effect of malaria chemoprevention regimens on the incidence of bacterial infections and antimicrobial resistance is unclear. We assessed incidence of bacterial infections and resistance patterns in children with SCA receiving weekly DP or monthly SP. METHODS: This study was nested in a randomized, double-blind clinical trial comparing weekly DP with monthly SP. Participants were

Microfilaremic loiasis is associated with T cell hyporesponsiveness against SARS-CoV-2.

BACKGROUND: Loiasis is a chronic filarial infection endemic to Central and West Africa. Although long considered benign, increasing evidence links loiasis to substantial morbidity and mortality. The infection is associated with immune modulation, including Th2-skewed responses and elevated regulatory cytokines. Clinically, loiasis is classified as microfilaremic (presence of circulating microfilariae) or amicrofilaremic ("occult") disease, the latter defined by a history of eyeworm migration without detectable microfilaremia. This study investigated how chronic L. loa infection influences anti

Fatal coinfection by Mycobacterium fortuitum and Epicoccum sp. with concurrent severe coccidial enteritis in an olive ridley sea turtle (Lepidochelys olivacea).

Sea turtles are susceptible to opportunistic infections, particularly when debilitated, injured, or exposed to environmental stressors. Although nontuberculous mycobacteria are increasingly recognized as emerging pathogens in marine wildlife, documented cases in sea turtles remain limited, and coinfections involving mycobacteria and uncommon environmental fungi are rarely reported. Here, we describe a fatal multisystemic disease involving Mycobacterium fortuitum infection, Epicoccum sp.-associated coelomitis, and severe coccidial enteritis in an adult olive ridley sea turtle (Lepidochelys oliv

Infectious triggers in IgA vasculitis in children: current evidence, causal attribution and immunopathogenic mechanisms.

IgA vasculitis (IgAV) is the most common systemic vasculitis in childhood. Its seasonal distribution and frequent respiratory or gastrointestinal prodromes implicate environmental exposures. However, microbiological detection temporally proximate to disease onset is insufficient to establish causality in an individual patient. To critically evaluate infectious exposures associated with pediatric IgAV, we distinguished epidemiological associations from incidental detection and infectious mimics, and separate direct IgAV evidence from mechanisms extrapolated from IgA nephropathy or general mucos

FPGA-Based Medical Image Processing Using Hardware-Software Co-Design Approach.

This paper presents a field-programmable gate array (FPGA) based medical image processing framework using a hardware-software co-design approach for biomedical tasks such as Malaria and Pneumonia detection. The design is implemented on the AMD-Xilinx UltraScale+ MPSoC (ZCU104) FPGA, focusing on optimizing data movement between the Processing System (PS) and Programmable Logic (PL) through a customized high-level synthesis (HLS) process. Depth-wise convolution is employed to reduce computational complexity, while layer fusion is applied to optimize layer-wise execution, and custom cache is inte

Progress towards malaria elimination: insights from Cambodia's mobile malaria workers during the COVID-19 pandemic.

Significant progress has been made in Cambodia towards malaria control and the goal of achieving elimination of all species by 2025. These efforts require constant vigilance and agility in malaria programming to maintain forward momentum. During the COVID-19 pandemic, these achievements were threatened by restrictive pandemic control measures, necessitating swift action from the National Center for Parasitology, Entomology and Malaria Control and partners to safeguard critical malaria services, primarily case detection and treatment by mobile malaria workers (MMWs) among hard-to-reach populati

Elevated mortality and upregulated SARS-CoV-2-associated pathways in innate and adaptive immune cells from individuals with Down syndrome.

Trisomy 21 increases the risk of severe outcomes and mortality in hospitalized individuals with Down syndrome (DS) following SARS-CoV-2 infection. Using data from the Brazilian Epidemiological Surveillance Information System Influenza (SIVEP-Gripe), we analyzed 102,767 hospitalized COVID-19 patients (1,115 DS and 101,652 non-DS, NDS). DS patients had a higher prevalence of comorbidities and required ventilatory support, ICU admission, and intubation more frequently than NDS patients (p < 0.001). Mortality was 4.5 times higher in DS patients aged 0-30 years (26.3% vs. 5.9%, p < 0.001) and remai

Neutralizing nanobodies against SARS-CoV-2 recognizing highly conserved epitopes at the Spike's S2 subunit.

The formation of a six-helix bundle between the conserved heptad-repeat regions 1 and 2 (HR1 and HR2) in SARS-CoV-2 Spike's S2 subunit is essential for membrane fusion and represents a promising therapeutic target. Previously, we reported recombinant proteins named CoVS-HR1, which mimic the HR1 region and block its interaction with HR2, inhibiting viral fusion. Moreover, they are recognized by plasma antibodies from COVID-19 convalescent patients. In this work, we generated camelid heavy-chain-only antibody fragments (VHHs), also named nanobodies (NBs), against a CoVS-HR1 variant mimicking the

Exploring factors affecting malaria vaccination intention and COVID-19 vaccine uptake: evidence from a joint analysis in rural Senegal.

BACKGROUND: Current malaria vaccination efforts target infants from 5 months of age; however, adult malaria mortality remains a significant and under-reported issue in high-transmission settings. Concerns are emerging that COVID-19-related vaccine hesitancy may extend to upcoming vaccines, including hypothetical ones such as a malaria vaccine for adults. This study investigates both shared and vaccine-specific determinants of malaria vaccination intention, comparing them with those influencing COVID-19 vaccine uptake. METHODS: A cross-sectional telephone survey was conducted in 2023 among 795

Inflammasome Activation and Oxidative Stress in SARS-CoV-2 Infection and Symptomatic Rebound.

BACKGROUND: Despite the efficacy of therapeutics and vaccines in reducing risk of severe coronavirus disease 2019 (COVID-19), severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infections continue to occur and rebound symptoms after initial improvement have been well described. The mechanisms driving COVID-19 rebound remain unclear. METHODS: Critical inflammatory responses were assessed by Imagestream, flow cytometry, and single-cell RNA sequencing in peripheral blood mononuclear cells from vaccinated individuals presenting with early (3 days postinfection, n = 6) or late (8 days po

Interstitial lung disease in small ruminants.

Interstitial lung diseases of sheep and goats, which are caused by a range of infectious, parasitic, and toxic agents, have substantial negative health and welfare impacts globally. Within this category of pulmonary disease, entities such as peste des petits ruminants (PPR) can undermine the livelihood of farming communities in sub-Saharan Africa and Southeast Asia; enzootic pneumonia, maedi, and ovine pulmonary adenocarcinoma compromise the productivity of farm enterprises where sheep are housed for prolonged periods. I detail the pathogenesis and lesions caused by a range of viral, bacterial

Genomic analysis reveals dynamics of SARS-CoV-2 during the initial phases of the COVID-19 outbreak in the Dominican Republic.

Genomic sequencing of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has been widely instituted during the coronavirus disease 2019 (COVID-19) pandemic to track the emergence and spread of new lineages. While this powerful tool can influence public health decisions and therapeutic development, not all regions of the world have had equal access to sequencing capacity, affecting surveillance. One such underrepresented region is the Caribbean islands, including the Dominican Republic (DR). To determine retrospectively what lineages were circulating in the DR in the first year of the

Helminth Antigens Modulate Virus-Induced Activation of CD154 (CD40L) Expression on T Cells in Onchocerca volvulus-Infected Individuals.

BACKGROUND: The interaction between helminth and viral infections has important implications for understanding viral disease outcomes and vaccine efficacy in helminth-endemic regions. We previously demonstrated that helminth seropositivity is associated with reduced Th1/Th17 cytokine levels and reduced COVID-19 severity; however, the underlying immunological mechanisms remain unclear. This study further investigated these mechanisms by assessing how helminth antigens influence SARS-CoV-2-induced T-cell responses in individuals infected with filarial parasites in vitro. METHODS: Peripheral blo
