Nonlinear age-season dynamics and macrolide resistance of Mycoplasma pneumoniae: a large pediatric cohort study during the post-pandemic resurgence.

BACKGROUND: Mycoplasma pneumoniae (MP) is a major cause of community-acquired pneumonia in children and demonstrates distinct age- and season-dependent patterns. However, nonlinear risk profiles, the burden of severe complications, and macrolide resistance remain incompletely characterized in large pediatric cohorts. OBJECTIVE: To describe the epidemiological features of MP infection in a large pediatric pneumonia cohort, delineate nonlinear associations of age and season with MP-related risks, and evaluate macrolide resistance patterns and the burden of plastic bronchitis (PB). METHODS: We

Pathogenesis and Risk Factors of Post-Infectious Bronchiolitis Obliterans in Children: A Focus on Adenovirus and Mycoplasma Infections.

Post-infectious bronchiolitis obliterans (PIBO) is a severe chronic airway disease in children following lower respiratory tract infections. Human adenovirus (HAdV) and Mycoplasma pneumoniae (MP) are the major associated pathogens, with geographic variations in their relative importance. This review analytically compares the mechanistic divergence and convergence between HAdV and MP. Both pathogens converge on MyD88/NF-κB/MAPK signaling and neutrophil-driven inflammation, but diverge in initial host engagement (CAR/integrins vs. TLR2/6 and CARDS toxin) and inflammasome activation (TLR9-related

Walking Pneumonia - Not Always so Timid. A Case of Severe ARDS Due to Fulminant Mycoplasma Pneumonia.

Mycoplasma pneumoniae pneumonia (MPP) most commonly presents as bronchitis or as a self-limited 'walking' community acquired pneumonia. A fulminant form of MPP may occur in 0.5 %-2 % of patients and is characterized by rapidly progressive respiratory failure and Acute respiratory distress syndrome (ARDS). Our case highlights the importance of considering fulminant MPP as a cause of ARDS, especially in the young adult and middle-aged population. We review the current literature and management principles of this rare disease with an emphasis on antibiotic stewardship and corticosteroid therapy.

Case Report: Aspergillus fumigatus-associated plastic bronchitis in a pediatric patient with cystic fibrosis complicated by allergic bronchopulmonary aspergillosis.

While plastic bronchitis (PB) is most commonly associated with viral pathogens and Mycoplasma pneumoniae (MP) infections, fungal etiologies are exceptionally uncommon in children. We described a case of a 10-year-old girl with Aspergillus fumigatus-induced PB. She had a 10-month history of intermittent coughing as the predominant symptom, without acute progressive dyspnea. Her medical history included cystic fibrosis (CF) complicated by allergic bronchopulmonary aspergillosis (ABPA). The bronchoscopy revealed a large amount of thick, gelatinous mucus plugs in the right lower lobe bronchus. Aft

Case Report: Mycoplasma pneumoniae-induced rash and mucositis (MIRM) in an 8-year-old girl: concise review of a challenging case.

We report an 8-year-old girl presenting with an 8-day history of cough, 5-day fever, and recent-onset conjunctival hyperemia and oral mucosal erosions. Laboratory testing confirmed Mycoplasma pneumoniae infection (IgM antibody and DNA positive). Chest x-ray demonstrated bilateral increased and blurred lung markings, consistent with acute bronchitis. Initial treatment with azithromycin and methylprednisolone sodium succinate (MPSS) improved respiratory symptoms but triggered a progressive rash involving the trunk, perineum, and anus. Adjuvant therapy with intravenous immunoglobulin (IVIG, 22.5
