Clinical Aspects of Upper and Lower Respiratory Tract Infections.

Respiratory tract infections are among the most common illnesses leading to medical consultation, and are associated with significant mortality. Community-acquired pneumonia is a common illness and, while Streptococcus pneumoniae continues to be the most frequent causative agent, atypical pathogens such as Mycoplasma pneumoniae, Chlamydia pneumoniae and Legionella species are now identified as additional common aetiological agents. Since clinical and roentgenographic features poorly predict the aetiological agent in most cases of community-acquired pneumonia, empirical therapy is generally rec

Towards a vaccine for chronic obstructive pulmonary disease.

This review discusses chronic obstructive pulmonary disease as an outcome of two pathogenic pathways: the first resulting from inhalation of toxins and the second a consequence of bacterial colonisation of damaged airways. Earlier assessment of the role played by bacteria in acute exacerbations was compromised by a deficiency of quality data and the use of parameters more relevant to invasive infection. Data are reviewed to support a hypothesis that states intrabronchial inflammation reflects an excessive and inappropriate host response (largely mediated by Th17 cells derived from gut-associat

Epithelial myoepithelial carcinoma of head and neck region.

Eptifibatide, a molecule isolated from the venom of the southeastern pygmy rattlesnake, selectively inhibits the platelet receptor IIb/IIIa. The safety and clinical efficacy of eptifibatide in patients undergoing percutaneous coronary intervention (PCI) was first evaluated in the Integrilin to Minimize Platelet Aggregation and Coronary Thrombosis (IMPACT) trial. In this study, the primary combined endpoint (composite of death, myocardial infarction [MI] or target vessel revascularization [TVR] at 30 days) occurred in 9.6% of the patients assigned to eptifibatide bolus followed by the 4-hour in

Acute exacerbations in COPD and their control with oral immunization with non-typeable haemophilus influenzae.

Chronic obstructive pulmonary disease (COPD) a term based on the demonstration of irreversible airways obstruction, introduced to unify a range of chronic progressive diseases of the airways consequent upon inhalation of toxins. While disease is initiated and progressed by inhaled toxins, an additional pathway of damage has emerged, with particular relevance to acute exacerbations. Exacerbations of disease due to an increase in the level of intrabronchial inflammation have taken on a new significance as their role in determining both acute and chronic outcomes is better understood. Cefditoren in upper and lower community-acquired respiratory tract infections.

This article reviews and updates published data on cefditoren in the evolving scenario of resistance among the most prevalent isolates from respiratory tract infections in the community (Streptococcus pyogenes, Haemophilus influenzae, and Streptococcus pneumoniae). By relating the in vitro activity of cefditoren (in national and multinational surveillance and against isolates with emerging resistant genotypes/phenotypes) to its pharmacokinetics, the cefditoren pharmacodynamic activity predicting efficacy (in humans, animal models, and in vitro simulations) is analyzed prior to reviewing clinic

Haemophilus influenzae and smoking-related obstructive airways disease.

BACKGROUND: Intralumenal bacteria play a critical role in the pathogenesis of acute infective episodes and airway inflammation. Antigens from colonizing bacteria such as nontypeable Haemophilus influenzae (NTHi) may contribute to chronic lung disease through an immediate hypersensitivity response. The objective of this study was to determine the presence of specific NTHi-IgE antibodies in subjects with chronic bronchitis (CB) and COPD who had smoked. METHODS: Serum, sputum, and saliva samples were collected from subjects with CB and moderate-severe COPD and healthy aged-matched controls. Tota

[Macrolides in antibiotic therapy of infections of the bronchial tree].

Macrolide antibiotics have been used to treat acute and chronic inflammation in the respiratory tract based on their antibacterial activity. Now it is well known that macrolides down-regulate damaging prolonged inflammation as well as increase mucous clearance and decrease bacterial virulence. Their ability to inhibit bacterial biofilm is a very important point in control of infections of the bronchial tree, where colonization of Haemophilus influenzae, Streptococcus pneumoniae and Pseudomonas aeruginosa were documented in clinical and experimental studies.

Evaluation of coamoxiclav and other antibiotics against S pneumoniae and H influenae from paediatric cases of acute respiratory infections.

Streptococcus pneumoniae and Haemophilus influenzae are most important respiratory pathogens with increasing antimicrobial resistance among the key pathogens responsible for community-acquired respiratory tract infections and have the potential to limit the effectiveness of antibiotics available to treat these infections. In the present study, a total of 18 isolates of Streptococcus pneumoniae and 9 isolates of Haemophilus influenzae were characterised from specimens obtained from patients of acute respiratory tract infections including otitis media, tonsillitis, bronchitis, pneumonia and sinu

High frequency of β-lactamase-negative, ampicillin-resistant strains of Haemophilus influenzae in patients with chronic bronchitis in Japan.

In Japan, the increasing incidence of β-lactum-resistant Haemophilus influenzae infections is of growing concern. We retrospectively studied whether the prevalence of β-lactamase-negative ampicillin-resistant strains of H. influenzae was influenced by chronic lung diseases. H. influenzae isolates, obtained from patients who were diagnosed with acute or chronic bronchitis, or acute exacerbation of chronic bronchitis in 2005, were studied. In addition to susceptibility testing, polymerase chain reaction (PCR) was performed for the detection of TEM-1 β-lactamase, and Asn526-Lys and Ser385-Thr ami

The diagnosis and treatment of elderly patients with acute exacerbation of chronic obstructive pulmonary disease and chronic bronchitis.

The syndrome of chronic obstructive pulmonary disease (COPD) consists of chronic bronchitis (CB), bronchiectasis, emphysema, and reversible airway disease that combine uniquely in an individual patient. Older patients are at risk for COPD and its components--emphysema, CB, and bronchiectasis. Bacterial and viral infections play a role in acute exacerbations of COPD (AECOPD) and in acute exacerbations of CB (AECB) without features of COPD. Older patients are at risk for resistant bacterial organisms during their episodes of AECOPD and AECB. Organisms include the more-common bacteria implicated

Haemophilus influenzae and the complement system.

The respiratory tract pathogen Haemophilus influenzae is responsible for a variety of infections in humans including septicemia, bronchitis, pneumonia, and acute otitis media. The pathogenesis of H. influenzae relies on its capacity to resist human host defenses including the complement system, and thus H. influenzae has developed several efficient strategies to circumvent complement attack. In addition to attracting specific host complement regulators directly to the bacterial surface, the capsule, lipooligosaccharides, and several outer membrane proteins contribute to resistance against comp

Collaborative studies in mucosal immunology in Goroka.

A collaborative program between the Papua New Guinea (PNG) Institute of Medical Research and the Hunter Mucosal Group has completed studies relevant to protection of the airways against bacterial infection. Specifically, these studies addressed the mucosal capacity to produce local immunoglobulins and the capacity of the airways to respond to an oral vaccine containing inactivated nontypeable Haemophilus influenzae (NTHi). The mucosal IgA response to NTHi antigens was blunted in both children and adults in PNG compared with that found in Australian children and adults, whose airways are coloni

The bacteriology of lower respiratory infections in Papua New Guinean and Australian indigenous children.

Indigenous children in Australia and children in Papua New Guinea (PNG) share a high burden of respiratory disease. In PNG the focus has been on pneumonia as a major cause of mortality. While pneumonia incidence remains high in Australian Indigenous children, improved access to better health care has resulted in reduced mortality. However, severe and recurrent pneumonia are risk factors for chronic suppurative lung disease or bronchiectasis in Australian Indigenous children. Bronchiectasis is associated with significant morbidity, and early death in adulthood. This paper includes an outline of

INTRODUCTION: Few studies exist exploring the experiences of Lesbian, Gay, Bisexual, Trans, Queer, and other personal identifications within the queer spectrum (LGBTQ+) medical students, particularly utilising first-person narratives. The few studies that exist illustrate an international landscape of discrimination beyond that experienced by non-LGBTQ+ peers. This study aimed to enhance understanding of LGBTQ+ medical students' experiences in their own words to consider if and what change is needed.
