The Tecumseh study of respiratory illness. X. Relation of acute infections to smoking, lung function and chronic symptoms.

The relationship of acute respiratory illness and infection to chronic bronchitis was investigated in the community of Tecumseh, Michigan. Families were randomly selected for study from among all those in the community and they were followed for periods of one year. Occurrence of acute respiratory illnesses was identified on a weekly basis. Frequency of chronic symptoms of cough and sputum production was separately ascertained. Lung function studies were performed three times during the course of the surveillance year and blood specimens were collected at those times. The blood specimens were

[Some respiratory viruses in the etiology of acute upper respiratory infections].

The viral etiology of acute respiratory diseases, determined in 107 children admitted to hospital during the 1966-1969 period, was confirmed in 36% of the cases by isolation of the viral agent and in 59% by serologic reactions. Parainfluenza viruses, especially of type 1 and 3, were encountered more often in the etiology of acute or obstructive laryngitis, and the syncytial respiratory virus in tracheobronchitis.

[Results of serologic surveys of children with acute stenosing laryngotracheobronchitis].

The materials of serologic surveys of 269 children with acute stenosing laryngotracheobronchitis hospitalized in Children's Infectious Hospital No. 3, Leningrad, are presented. Acute stenosing laryngotracheobronchitis developed most frequently against the background of low titres of antibody to viruses of the respiratory group. Severe cases of the disease were caused by influenza A2 virus, milder cases by influenza B, parainfluenza, respiratory syncytial and adenoviruses.

Viral infection in wheezy bronchitis and asthma in children.

Virus isolation was attempted on 267 out of 360 patients with wheezy bronchitis or asthma admitted to hospital during a 3-year period. Viruses were isolated on 39 occasions, the most common being respiratory syncytial virus and rhinovirus. The peak months for virus isolation were February and August. Virus isolation was significantly more common in readmissions than in first admissions (P less than 0-01). Viruses were isolated in both sexes throughout childhood and though the admission rate fell with increasing age, the isolation rate was unaffected. The possible significance of viral infectio

Quantitative shedding patterns of respiratory syncytial virus in infants.

Quantitative shedding patterns of respiratory syncytial virus in 40 infants hospitalized with acute disease of the lower respiratory tract were determined for elucidation of the pathophysiology of infection with the virus. Nasal wash specimens were collected on admission and daily thereafter and were tested for the presence and quantities of respiratory syncytial virus. The following pattern of shedding was observed. (1) The virus was shed for prolonged periods. For the first seven days of hospitalization, 92%-100% of the infants tested continued to shed virus. At discharge 87% were still shed

The Tecumseh study of respiratory illness. VIII. Acute infection in chronic respiratory disease and comparison groups.

Individuals with chronic lung disease and their families were selected from the Tecumsch community along with similarly selected families as comparison groups and studied for 1-year periods. Occurence of acute respiratory illness was ascertained weekly by telephone and calculated as an annual rate. Persons with chronic bronchitis not only experienced more acute lower respiratory illness than healthy comparison subjects, but total illness rates were somewhat higher as well. Infection rates were determined from blood samples taken 3 times from each participant during the surveillance year. Antib

The local antibody response to R.S. virus infection in the respiratory tract.

Nasopharyngeal secretions were taken during the acute phase of illness from 66 infants and children admitted to hospital with lower respiratory tract infections. Second secretions were taken, after an interval of 7 days, from 33 of these patients. A significant increase in neutralizing activity to R.S. virus was demonstrated in the nasopharyngeal secretions of patients in response to severe R.S. virus infection. Seventeen out of 25 patients (68%) with R.S. virus infections developed a rise in secretory neutralizing titre, compared with only 1 out of 8 patients (13%) with respiratory infections

Acute respiratory failure in bronchiolitis and pneumonia in infancy. Modes of presentation and treatment.

The modes of presentation and the management of acute respiratory failure in 11 infants with severe lower respiratory tract infections (due to respiratory syncytial virus in eight) are described. Progressive respiratory difficulties leading to exhaustion, peripheral circulatory collapse, recurrent apnoeic attacks, or generalized convulsions were the main clinical presentations resulting in severe ventilatory failure. In nine infants preventilation carbon dioxide tensions exceeded 65 mm Hg. It seems likely that the use of intermittent positive-pressure ventilation in these patients contributed

Coronavirus infection in acute lower respiratory tract disease of infants.

A serologic surveillance of lower respiratory tract disease in 417 hospitalized children under 18 months of age revealed infection with coronaviruses (strains OC43 and/or 229E) in 34 (8.2%). During the same interval, one of 13 control infants was infected. There were two distinct periods lasting six and 14 weeks, respectively, during which the incidence rose to as high as 18.9% of patients with lower respiratory tract disease. The incidence of coronavirus infection in patients with pneumonia and bronchiolitis was higher than the incidences of adenoviruses, influenza, parainfluenza viruses type

Respiratory syncytial virus infections in children in Montreal: a retrospective study.

Seventy-seven hospitalized children from whom respiratory syncytial virus was subsequently isolated were studied in retrospect. This demonstrated the association of host invasion by this agent with an acute respiratory disease requiring hospitalization for approximately one week.Boys considerably outnumbered girls, the average age was 7.5 months, and in 56% the admission diagnosis was "bronchiolitis". Cough and difficulty in breathing were the most common presenting complaints and "pneumonia" the most common radiological diagnosis. Nearly half the patients also harboured accepted disease-causi

Epidemiology of respiratory syncytial virus infection among infants and children in Chicago.

From January 1, 1967 to December 1971, the temporal pattern of respiratory syncytial virus infection was investigated in infants and children younger than 18 months hospitalized for acute lower respiratory tract disease. Of 4696 infants and children with acute lower respiratory tract disease admitted to the Cook County Hospital, 2530 were tested for virus infection by virus isolation or serologic procedures or both. Overall, respiratory syncytial virus infections were detected in 12% and parainfluenza 3 virus in 10.8% of individuals tested. Other respiratory viruses were less commonly identifi

Pathogenesis of respiratory syncytial virus diseases in infancy.

In a series of 54 children under 1 year old with respiratory syncytial virus infection, the highest proportion of severe illnesses (bronchiolitis and pneumonia), and the highest proportion of `slow' antibody responses, were found in children under 6 months. Severe illness may therefore be related to immunological immaturity and to lack of protection by maternal antibody, rather than to an allergic response as has been suggested. A high male to female ratio (approximately 2: 1) was found both in the present series, and in two larger series of respiratory syncytial virus infection in infants.
