[Virological surveillance of acute respiratory tract illnesses of children in Morioka, Japan. I. Epidemiological patterns of infection with respiratory viruses over a 10-year period].

A virological surveillance of acute respiratory tract illnesses (RTI) of children in Morioka, Japan, was maintained from September 1973 to December 1983. Nasal and throat swabs were collected from 4,334 children with RTI. These patients consisted of 3,500 children (80.8%) with upper RTI (URTI) and 834 children (16.2%) with lower RTI. When these patients with URTI were classified by maximum temperature recorded into 4 groups (less than or equal to 37.0 degrees C, 37.1-37.9 degrees C, 38.0-38.4 degrees C, and greater than or equal to 38.5 degrees C), the number of patients in each group was 1,90

[Respiratory tract infection and acute deterioration of obstructive lung disease].

Respiratory infection may aggravate chronic obstructive pulmonary disease. Viral respiratory infections may aggravate asthma, particularly in young individuals. Respiratory Syncytial virus and Rhinovirus dominate in children while, in adults, Influenza or Rhinovirus infections are most frequently concerned. Viral respiratory infections may also cause exacerbation of chronic bronchitis. Bacteria and their products scarcely play any part in asthmatic disease but may possibly aggravate chronic bronchitis and other forms of obstructive respiratory disease. In particular, Haemophilus influenzae and

[Virological surveillance of acute respiratory tract illnesses of children in Morioka, Japan. II. Rhinovirus infection].

Rhinoviruses (HRVs) were isolated from 307 children (7.1%) in the virological surveillance of 4334 children with acute respiratory tract illnesses in Morioka, Japan (September 1973-December 1983). Although HRVs were isolated throughout the year, frequency of HRV infection was significantly higher (p less than 0.001) during the April-November (233/2853; 8.2%) than during the December-March (47/1481; 5.0%). There were two peaks of incidence in May (9.5%) and September (9.1%). During the May-September, the rate of HRV infection was higher in patients under the age of 11 months than the next highe

Acute bronchitis in the community: clinical features, infective factors, changes in pulmonary function and bronchial reactivity to histamine.

A descriptive study of acute bronchitis in patients without pre-existing pulmonary disease was undertaken in the community during the winter months of 1986-87. Forty-two episodes were investigated in 40 individuals. The cardinal symptom was the acute onset of cough (100%), usually productive (90%). Wheezing was noted by 62% of patients, but heard on auscultation in only 31%. A potential pathogen was isolated in 29% of cases with a virus (eight cases) being identified more frequently than either Mycoplasma pneumoniae (three cases) or a bacterium (three cases). The acute illness was associated w

Routine viral culture for pediatric respiratory specimens submitted for direct immunofluorescence testing.

From 1986 to 1987, 69 (25%) of 274 specimens from children with lower respiratory tract syndromes were positive for respiratory syncytial virus antigen by direct immunofluorescence assay (DFA). Comprehensive viral culture was performed on all 205 DFA-negative specimens, and 72 specimens yielded viruses; 5 specimens yielded multiple agents. Thus, 52% of specimens yielded a specific virus, supporting the routine use of viral culture. Isolates from the DFA-negative specimens included respiratory syncytial virus (n = 7), rhinovirus (n = 34), hemadsorbing viruses (n = 13), cytomegalovirus (n = 11),

Occurrence and aetiology of acute respiratory diseases: results of a longterm surveillance programme.

Totals of 58,661,000 acute respiratory disease (ARD) cases, 1,376,651 bronchitis and pneumonia complications, and 93,042 deaths from influenza, bronchitis, pneumonia or chronic pulmonary affection were notified during 11 years of ARD surveillance from 1975 to 1986. All ARD seasons started with the first phase in September-December; this increase in morbidity was caused chiefly by adenoviruses, parainfluenza viruses, rhinoviruses and M. pneumoniae. Second wave of ARD morbidity occurring in January-April used to be explosive and was associated with an influenza epidemic in 9 of the 11 seasons; o

Acute bronchiolitis: predisposing factors and characterization of infants at risk.

Fifty-one infants hospitalized because of acute bronchiolitis and 24 control children were prospectively followed until 2 years of age. Respiratory syncytial virus was detected initially in 31 index children, parainfluenza virus in two, and rhinovirus in one. More index children were born during the months April through September, and six index children had neonatal respiratory complications. The index children were breast-fed for shorter periods, lived in more crowded homes, and had more siblings. No significant differences were found for atopic background and atopic illness at 2 years of age

[Causative relations between infections and allergies in obstructive respiratory tract diseases in childhood].

Obstructive bronchitis of young babies and infants is most often caused by respiratory-syncytial (RS)- and parainfluenza viruses, to a certain extent by adenoviruses. In the school age-group rhinoviruses and to a smaller extent mycoplasma pneumoniae gain further importance. All the viruses may cause or deteriorate asthmatic attacks, as well as during common cold or on the basis of documented allergies. Proposed mechanisms of viral-infection-induced obstructive airway disease are distinct lesions of bronchial-epithelium, followed by enhanced allergen-resorption and sensitization of submucosal m

Bronchopulmonary obstruction in children with respiratory virus infections.

Among 873 children hospitalized with confirmed respiratory viral infections, 492 suffered from bronchopulmonary obstruction. Respiratory syncytial virus infections were dominant among the patients with bronchopulmonary obstruction (80%). The majority of patients with rhinovirus infections suffered from bronchopulmonary obstruction (57%). Two main groups of children at risk from developing bronchopulmonary obstruction during respiratory virus infections were found: 1) infants, suffering from bronchiolitis, mainly due to respiratory syncytial virus, and 2) children of all ages with bronchial ast

Respiratory viral infection and wheezy bronchitis in childhood.

The role of respiratory viral infection in wheezy bronchitis was studied in 163 children, aged 0-12 years, in a London general practice. Virological investigations were also performed when these same children had acute upper respiratory illness without wheeze. A virus was isolated in 146 (26.4%) of 554 episodes of wheezy bronchitis, rhinoviruses accounting for almost half of the isolations. The relative frequency with which individual viruses were isolated in wheezy bronchitis was similar to that in acute upper respiratory illness in 180 other children who had never had wheezy bronchitis. Role of viruses and bacteria in acute wheezy bronchitis in childhood: a study of sputum.

Sputum, nasal swabs, and throat swabs were obtained from 22 children aged between 5 and 15 years during 72 attacks of wheezy bronchitis. A virus, most commonly a rhinovirus, was isolated in 49% of all episodes and in 64% of 22 severe episodes requiring treatment with corticosteroids; the isolation rate was higher early in the illness than later. Virus was recovered more often from sputum than from the nose or throat, suggesting that viral replication occurs freely in the lower respiratory tract: the cytological findings in sputum were compatible with an inflammatory response to viral infection

Infections with viruses and Mycoplasma pneumoniae during exacerbations of chronic bronchitis.

The association of viral and Mycoplasma pneumoniae infections with acute exacerbations of chronic bronchitis was studied by serologic or isolation techniques in 46 adult men during the five years from 1964 through 1968. Serologic evidence of viral or M. pneumoniae infection was detected in 25% of 166 episodes of exacerbation and 14% of 138 remission periods (P = 0.02). Influenza A virus, parainfluenza virus type 3, and coronavirus OC43 predominated; infections with other viruses were infrequent. Infection with M. pneumoniae was detected serologically in four patients, but this organism was nev

[Pathogenesis of rhinovirus infection].

Rhinoviruses (RV) are the etiological factors of not only acute respiratory diseases of the common cold type but also of acute and chronic diseases of otorhinolaryngological organs. Virological and bacteriological examinations of patients with chronic pneumonia in the stages of exacerbation and incomplete remission revealed a high degree of the infection activity. Virus-bacterial associations were frequently found. RV were isolated from the lower respiratory tract (bronchopulmonary secrete, biopsy materials). The association of RV with the involvement of the lower respiratory tract in patients

Viral infection in wheezy bronchitis and asthma in children.

Virus isolation was attempted on 267 out of 360 patients with wheezy bronchitis or asthma admitted to hospital during a 3-year period. Viruses were isolated on 39 occasions, the most common being respiratory syncytial virus and rhinovirus. The peak months for virus isolation were February and August. Virus isolation was significantly more common in readmissions than in first admissions (P less than 0-01). Viruses were isolated in both sexes throughout childhood and though the admission rate fell with increasing age, the isolation rate was unaffected. The possible significance of viral infectio

Respiratory viral infection in childhood. A survey in general practice, Roehampton 1967-1972.

The role of viruses and M. pneumoniae in episodes of acute respiratory illness in childhood has been studied in a London general practice. The total isolation rate was 31-7 per cent, but the rate varied from 32-6 per cent in upper respiratory infections to 64-0 per cent in pneumonia. The clinical features associated with infection were influenced not only by the type of agent but also by age and other host factors in infected children. Rhinoviruses were more commonly isolated than any other agent and were frequently associated with wheezy bronchitis.

A review of routine tests for respiratory viruses in hospital inpatients.

During the period June 1967 to May 1972, viral tests were performed upon 1195 inpatients aged 12 and over in the Brompton Hospital. The overall diagnostic rate was 21-5%, comparing 9-3% by isolation and 14-9% by complement fixation (6-5% by four-fold rises in titre in paired sera, 8-4% by titres larger than or equal to 160 in single sera). Only 42% of all patients had an acute respiratory illness within one week prior to admission, which seriously curtailed the chances of isolating viruses and of obtaining early serological specimens. The value of complement fixation tests on single specimens

Viruses associated with acute respiratory infections 1961-71.

Influenza-like illness, cold and sore throat was the diagnosis given in over 80% of 5177 acute respiratory illnesses in patients swabbed over a 10-year-period. A pathogenic organism was isolated twice as frequently from patients with a sore throat or an influenza-like illness as from those diagnosed as suffering from croup or laryngitis and bronchitis. A laboratory diagnosis was commoner in school children than in older or younger persons. Most of the organisms isolated were found in association with all types of acute respiratory illness but, with increasing age of the patient, one particular
