Evaluation of an F Protein-Based Recombinant Protein for Immunization Against Respiratory Syncytial Virus.

Respiratory syncytial virus (RSV) is one of the most important etiologies of acute respiratory infections that cause bronchiolitis in children under 5 years of age. Treatments are expensive, no vaccine is available, and this is an important cause of hospitalization. Costimulatory molecules have been reported to be good inducers of antiviral type 1 immune response. This study aimed to generate a recombinant vaccine with the RSV F protein fused to 4-1BBL to evaluate the activation of an antiviral response in vitro and the production of neutralizing antibodies in vivo. The codon-optimized F gene

Evaluating the accuracy of ICD-10 codes for syncytial respiratory virus diagnosis in hospitalized patients: A record-linkage study (2022-2023).

BACKGROUND: Respiratory Syncytial Virus (RSV) is a leading cause of severe respiratory infections in young children and older adults. Accurate RSV surveillance is essential to understanding its disease burden and evaluating vaccine impact. METHODS: We assessed the accuracy of ICD-10 coding for RSV hospitalizations in the Brazilian Hospital Information System (SIH) by linking it with the Severe Acute Respiratory Syndrome (SIVEP) notification system (2022-2023). Laboratory-confirmed RSV-positive and RSV-negative cases in SIVEP were used as the reference standard. Sensitivity and specificity wer

Nirsevimab prophylaxis on pediatric intensive care hospitalization for severe acute bronchiolitis: a clinical and economic analysis.

BACKGROUND: Severe acute viral bronchiolitis is a common cause of admissions to pediatric intensive care units (PICUs), resulting in a significant organizational burden each winter. The recent introduction of generalized neonatal prophylactic therapies using Nirsevimab, a monoclonal antibody targeting the respiratory syncytial virus (RSV), has significantly reduced consultations and hospitalizations. However, its impact on the medico-economic aspects of the PICU remains poorly defined. METHODS: We analyzed all infants admitted to our unit for severe acute bronchiolitis over six consecutive ep

Post-RSV Bronchial Web Formation in a Lung Transplant Recipient: A Case Report.

Bronchial webs (BW) are fibrinous membranes which obstruct the airway lumen. A 33-year-old female presented with acute lung allograft dysfunction following a recent respiratory syncytial virus (RSV) infection, having undergone bilateral lung transplant 2 years prior. Bronchoscopy identified subsegmental pale, translucent webs occluding six subsegments across both lung allografts. Each subsegment was dilated with an 8 mm balloon dilatation catheter. There was immediate symptomatic improvement without significant spirometric improvement. The described association between CLAD and BW may reflect

Identification of healthcare visits due to respiratory syncytial virus infection in electronic medical records.

Introduction. Acute lower respiratory infections (ALRI) due to respiratory syncytial virus (RSV) are a significant cause of disease. Most of them are treated outpatient without etiological investigation, making it difficult to estimate the disease burden. In 2020, an algorithm was developed to identify consultations for ALRI in electronic health records. We evaluated the algorithm's behavior in patients with RSV ALRI. Methods. The cross-sectional study included children under 5 years of age who consulted for ALRI with viral screening. The algorithm was applied to their health records, calculat

The impact of respiratory syncytial virus on asthma development and exacerbation.

Asthma is a chronic inflammatory disorder of the lower airways clinically characterized by recurrent wheezing, breathlessness, cough, and dyspnea and the most prevalent chronic disease among children and adolescents. Respiratory viral infections are implicated in asthma inception and exacerbation, with respiratory syncytial virus (RSV) emerging as a key contributor. RSV is a leading cause of acute lower respiratory tract infections, particularly infant bronchiolitis, and is associated with a type 2-biased immune response, diminished interferon activity, epithelial barrier dysfunction, and alte

Immunomodulation in Respiratory Syncytial Virus Infection: Mechanisms, Therapeutic Targets, and Clinical Implications.

Respiratory syncytial virus (RSV) remains a leading cause of acute lower respiratory tract infections globally, particularly affecting infants, older adults, and immunocompromised individuals. While recent advances in prophylaxis, such as long-acting monoclonal antibodies and maternal immunization, offer promise for prevention, therapeutic options for active infection remain limited. Severe RSV disease is often driven not solely by viral replication but by dysregulated host immune responses, including excessive cytokine production, T helper type 2 (Th2) and T helper type 17 (Th17) cell polariz

Predictive Factors and Clinical Markers of Recurrent Wheezing and Asthma After RSV Infection.

Respiratory syncytial virus (RSV) is a major cause of acute lower respiratory infections (ALRIs) in young children, especially bronchiolitis, with significant global health and economic impact. Increasing evidence links early-life RSV infection to long-term respiratory complications, notably recurrent wheezing and asthma. This narrative review examines these associations, emphasizing predictive factors and emerging biomarkers for risk stratification. Early RSV infection can trigger persistent airway inflammation and immune dysregulation, increasing the likelihood of chronic respiratory outcome

An Intranasal Challenge Model in African Green Monkeys (Chlorocebus aethiops) for Mild-to-Moderate COVID-19 Disease Caused by Subvariant XBB.1.5.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) primarily causes mild to moderate respiratory illness in humans, but infection can also lead to long-term complications, including chronic fatigue, respiratory and cardiac issues, or even death. In November 2021, the emergence of the highly transmissible Omicron variant marked a significant shift in the pandemic, with its subvariants rapidly spreading and continuing to evolve worldwide. The continuing introduction of Omicron subvariants underscores the need for the development of up-to-date vaccines, as well as for appropriate animal

Can plitidepsin be used as an antiviral against RSV?

UNLABELLED: Human respiratory syncytial virus (HRSV) is a main cause of acute lower respiratory tract infections in infants, the elderly, and immunocompromised patients. Although vaccines have recently been approved for the elderly and for pregnant women, there is no curative treatment for HRSV. HRSV replicates in the cytoplasm of infected cells, and transcription and replication of the viral genome depend on the viral RNA polymerase complex, which recruits cellular factors for RNA synthesis. Among them, the eukaryotic translation elongation factor 1A (eEF1A) was previously shown to be critica

Inpatient Management of Bronchiolitis Before and After the COVID-19 Pandemic.

Introduction Bronchiolitis is the most common cause of respiratory-related hospital admissions in children under two years. It is typically caused by viral infections, most commonly respiratory syncytial virus (RSV). Despite the availability of various international guidelines, Malta lacks local protocols, leading clinicians to rely on international recommendations for management. Objectives The primary aim of this audit was to compare epidemiological and management trends in bronchiolitis among children under one year admitted to Mater Dei Hospital between 2019 (pre-COVID) and 2024 (post-COVI
