Clinical phenotype profile and risk factors for sinusitis and obstructive sleep apnea in children with adenoid hypertrophy.

BACKGROUND: Adenoid hypertrophy (AH) is a common childhood condition frequently associated with multiple comorbidities. However, its specific clinical phenotype and the independent risk factors for key complications, particularly in children requiring surgery, remain inadequately elucidated. METHODS: This retrospective cohort study included 161 children undergoing surgery for AH. Demographic data, disease severity indicators, comorbidities, and laboratory results were collected. Descriptive statistics, chi-square tests, and multivariate logistic regression models were used for data analysis.

Eosinophil-Mast Cell Inflammation in Middle Ear Effusion Associated With Chronic Rhinosinusitis With Nasal Polyps.

Chronic rhinosinusitis with nasal polyps (CRSwNP) is predominantly associated with type 2 inflammation, although the inflammatory profile may involve both eosinophils and mast cells. Middle ear disease in CRSwNP is usually attributed to Eustachian tube dysfunction, but inflammatory involvement of the middle ear may also occur. We report the cytological evaluation of middle ear effusion from a patient with CRSwNP. May-Grünwald-Giemsa staining revealed eosinophils, mast cells, and extracellular granules consistent with degranulation. The simultaneous presence of these cellular components suggest

Predictive biomarkers of biologic therapy response in chronic rhinosinusitis with nasal polyps: a systematic review.

BACKGROUND: Biologic therapies have transformed management of severe chronic rhinosinusitis with nasal polyps (CRSwNP), but response variability persists. Baseline biomarkers may identify patients most likely to benefit, yet no marker is validated for routine clinical use. This systematic review evaluated baseline biomarkers predicting clinical and endoscopic response to biologic therapy in adults with CRSwNP. METHODS: A systematic search identified studies reporting baseline biomarkers and response to biologics (dupilumab, omalizumab, anti-IL-5 agents) in adult CRSwNP. Ten studies with 489 p

Real-world two-year outcomes of mepolizumab in patients with chronic rhinosinusitis with nasal polyps and coexisting severe asthma.

PURPOSE: Chronic rhinosinusitis with nasal polyps (CRSwNP) and severe asthma frequently coexist as manifestations of type 2 airway inflammation. Although mepolizumab has demonstrated efficacy in both conditions, long-term real-world data on integrated airway disease control remain limited. We evaluated the long-term effectiveness of mepolizumab in patients with CRSwNP and coexisting severe asthma over 2 years. METHODS: We conducted a longitudinal real-world observational cohort study at a tertiary referral center. Baseline data were collected retrospectively, and follow-up assessments were pr

Chronic Rhinosinusitis With Nasal Polyposis in EGPA: A Real-Life Multidimensional Analysis Through Mepolizumab Dosing and Role of Adjunctive Endoscopic Sinus Surgery (ESS).

BACKGROUND: Eosinophilic granulomatosis with polyangiitis (EGPA) is a rare necrotizing vasculitis involving upper- and lower-airways. Despite biologics, sinonasal disease often persists. Evidence on ENT trajectories under anti-IL5 treatment and role of endoscopic sinus surgery (ESS) remains limited and is often described without standardized assessment tools. This study provides a systematic, multidimensional evaluation of sinonasal outcomes under biologic therapy and analyzes the role of ESS in a real-life cohort. METHODS: We retrospectively analyzed 52 EGPA patients with a 24-month follow-u

TGF-β/Smad-Associated Epithelial-Fibroblast Remodeling Underlies Collagen Deposition in Chronic Rhinosinusitis With Nasal Polyps.

BACKGROUND: Chronic rhinosinusitis with nasal polyps (CRSwNP) is frequently accompanied by epithelial barrier disruption and tissue remodeling. This study aimed to investigate whether epithelial-mesenchymal transition (EMT), fibroblast activation, collagen deposition, and TGF-β/Smad signaling are coordinated during CRSwNP nasal mucosal remodeling. MATERIALS AND METHODS: Publicly available single-cell RNA sequencing datasets from two independent human cohorts were integrated to characterize epithelial, fibroblast, and myeloid heterogeneity, disease-associated transcriptional changes, pseudotim

Immune Checkpoint Inhibitors and Laryngological Outcomes in Patients with Non-Head and Neck Cancer.

OBJECTIVES: As immune checkpoint inhibitor (ICI) utilization increases in cancer treatment, immune-related adverse events continue to be recognized. Voice, swallowing, and oral dysfunction substantially impair quality of life and treatment tolerance in cancer patients, but the laryngological risks of ICI therapy remain unclear. We compared outcomes for these functions after ICI versus non-ICI antineoplastic therapies. METHODS: Using the TriNetX research network, we identified adults with non-head and neck malignancies treated with ICIs versus non-ICI systemic antineoplastics. To isolate off-t

First record of Phlebotomus sergenti (Diptera: Psychodidae) in China and its potential epidemiological implications.

BACKGROUND: Leishmaniasis is a severe parasitic disease transmitted by sand fly bites. Phlebotomus sergenti is a principal vector of Leishmania tropica, causing anthroponotic cutaneous leishmaniasis (ACL) across the Mediterranean, North Africa, South Asia, and West Asia. The Xinjiang Uygur Autonomous Region, located in northwestern China, is a well-recognized endemic focus of visceral leishmaniasis (VL), with sporadic cutaneous leishmaniasis cases also documented in the region. Nevertheless, Ph. sergenti has never been reported in China prior to this study. METHODOLOGY/PRINCIPAL FINDINGS: Fie

What Kind of Rhinitis Does Your Patient Have: Practical Considerations for Choosing the Best Treatment for Patients with Chronic Rhinitis.

Chronic rhinitis is defined by one or more symptoms of nasal congestion, anterior and/or posterior nasal drainage, and sneezing. Chronic rhinitis is a common reason for adolescent and adult patients to seek medical care as the symptoms can become pronounced and when uncontrolled, negatively impact the quality of life and productivity of those affected. A thorough clinical history of rhinitis symptoms will help distinguish between the 2 major phenotypes of rhinitis, nonallergic and allergic rhinitis. The clinical history and symptoms are also important to consider when deciding the best treatme

Chronic pain: an overview of its assessment and management.

Virus-encoded microRNAs (miRNAs) have been shown to regulate a variety of biological processes involved in viral infection and viral-associated pathogenesis. Epstein-Barr virus (EBV) is a herpesvirus implicated in nasopharyngeal carcinoma (NPC) and other human malignancies. EBV-encoded miRNAs were among the first group of viral miRNAs identified. To understand the roles of EBV miRNAs in the pathogenesis of NPC, we utilized deep sequencing technology to characterize the EBV miRNA transcriptome in clinical NPC tissues. We obtained more than 110,000 sequence reads in NPC samples and identified 44

Tissue-resident memory T cells in the nasal mucosa: gatekeepers of chronicity and relapse in allergic rhinitis.

Allergic rhinitis (AR) is a pervasive type-2 inflammatory disorder of the nasal mucosa whose clinical course is defined less by acute flares than by recurrence and apparent resistance to cure. Although AR has traditionally been framed as a systemic, IgE-driven disease, a growing body of evidence points to the nasal mucosa itself as a site of durable, antigen-specific immune memory. Tissue-resident memory T (TRM) cells - non-recirculating, longevity-endowed lymphocytes that lodge in barrier tissues - are ideally positioned to convert transient allergen exposure into persistent inflammation.
