Orbital Plasmacytoma as Initial Presentation of Multiple Myeloma.

Multiple myeloma (MM) is a hematologic malignancy characterized by clonal proliferation of plasma cells in the bone marrow. It represents approximately 10% of hematologic malignancies and is the most common primary bone malignancy. This disease usually occurs between the ages of 40 and 70, although it is more frequently seen in patients over 65 years of age. On the other hand, extramedullary plasmacytoma-related diseases have a global annual incidence of three per 100,000 people. Orbital involvement in MM is considered rare. Orbital plasmacytomas represent less than 1% of all orbital lesions,

Consensus Review: Predicting Recurrence in Chronic Rhinosinusitis With Nasal Polyps: Multidisciplinary Insights From the RELAPSE Project.

Chronic rhinosinusitis with nasal polyps (CRSwNP) is a disease with a high incidence of postoperative recurrence despite medical and surgical treatments. However, a universal definition of recurrence and its determinants is lacking. Our objective was to establish a clear definition of recurrence and reach consensus on the contributing factors. A consensus study was conducted using the Delphi method. The literature on recurrence of CRSwNP was reviewed and assessed by a multidisciplinary scientific committee. The committee designed a 2-round online Delphi questionnaire addressing recurrence in C

A Case of Nasal Myiasis Without Any Predisposing Factors: An Uncommon Presentation.

Nasal myiasis is an uncommon human parasitic disease affecting the nasal cavity. In otolaryngology, myiasis is mostly encountered as aural myiasis and oral myiasis, but occurs rarely as nasal myiasis. It typically affects individuals with poor hygiene, chronic illness, or immunosuppression. Cases in immunocompetent individuals are exceptional and can present diagnostic challenges. We report the case of a 32-year-old immunocompetent female who presented with the spontaneous expulsion of small larvae through the nostrils. Live larvae were observed in both nasal cavities during endoscopic examina

Phenotypic comorbidity network analysis of nasopharyngeal carcinoma: a multicenter study based on hospital discharge records.

BACKGROUND: Nasopharyngeal carcinoma (NPC) patients frequently have multiple comorbidities that affect treatment efficacy and survival. Prior studies focused only on individual common comorbidities, lacking a comprehensive analysis of the full spectrum of chronic diseases in NPC patients from endemic areas. This study adopted phenotypic comorbidity networks (PCNs) based on large-scale hospital discharge data to systematically characterize the overall comorbidity patterns of endemic NPC patients. METHODS: This retrospective cohort study was conducted using data from NPC patients identified in

A prognostic model based on seven cell adhesion molecule-related genes to predict the prognosis of colorectal cancer.

BACKGROUND: Magnetic resonance cholangiopancreatography (MRCP) is useful to assess exocrine pancreatic function by combining rapid imaging acquisition with the administration of secretin, a gastrointestinal hormone that stimulates the secretion of bile and pancreatic juice. However, extensive data on this method are lacking. This study aimed to determine whether MRCP with secretin administration is able to simultaneously detect alterations of both the pancreatic ducts and exocrine pancreatic function. METHODS: All subjects older than 18 years who underwent magnetic resonance imaging (MRI) and

RLAgent: retrieval-augmented LLM agents for code-free RNA region-ligand interaction modeling.

Background. Unlike full reading, 'skim-reading' involves the process of looking quickly over information in an attempt to cover more material whilst still being able to retain a superficial view of the underlying content. Within this work, we specifically emulate this natural human activity by providing a dynamic graph-based view of entities automatically extracted from text. For the extraction, we use shallow parsing, co-occurrence analysis and semantic similarity computation techniques. Our main motivation is to assist biomedical researchers and clinicians in coping with increasingly large a

Spatial profiling reveals complex cellular responses of anti-IL5 treatment with mepolizumab in eosinophilic chronic rhinosinusitis with nasal polyposis.

There is limited understanding of the impact of anti-IL5 treatment on nasal polyp tissue biology in chronic rhinosinusitis with nasal polyps (CRSwNP). This study examined nasal polyp tissue cellular proteome and transcriptome responses to anti-IL5 treatment in CRSwNP utilizing spatial profiling. GeoMx Digital Spatial Profiling of 80 proteins and 1,833 messenger RNA targets in the polyp stroma and the whole transcriptome (18,815 messenger RNA targets) in polyp epithelia was undertaken on sinonasal biopsies collected from 20 individuals with eosinophilic CRSwNP before and after 16 and 24 wk of m

Rationale of New Grading System: Central Compartment Atopic Disease.

BACKGROUND: Central compartment atopic disease (CCAD) has recently been recognized as a distinct phenotype within the spectrum of type 2-dominant chronic rhinosinusitis (CRS). Although international guidelines highlight polypoid changes in the central nasal cavity, standardized diagnostic and classification criteria are still lacking. This study aims to develop and internally validate a consensus-based classification system through expert agreement. METHODS: A two-round modified Delphi study was conducted with 10 international rhinology experts. Participants evaluated the need for a specific

Extent of Sinus Surgery Is Associated With Disease Control in Biologic Treated Type 2 Dominant CRS.

BACKGROUND: A greater benefit of biologics is observed after surgery in Type 2 chronic rhinosinusitis with nasal polyps (CRSwNP). However, the extent of surgery remains undefined in many studies. This study evaluated the extent of surgery on disease control in patients receiving biologics for refractory Type 2 dominant CRSwNP. METHODS: A cross-sectional study was performed on patients with CRSwNP treated with biologics for ≥ 6 months at a tertiary center. All patients had undergone prior sinus surgery, which included a sphenoethmoidectomy and then extension via Draf I, IIa, IIb, IIc, or III.

Age May Not be a Barrier: Cementless Total Knee Arthroplasty Yields Equivalent or Superior PROMs in Older Adults at One Year.

Increased travel to exotic destinations around the world is escalating the risk that an emerging virus may be imported into the UK. Rabies should be considered in the differential diagnosis of any encephalitic illness presenting in an appropriate epidemiological context. Molecular diagnostic tests that can rapidly discriminate rabies from other suspected infections will influence the use of anti-rabies prophylaxis for potential contacts with the victim. In 2001, the UK had two confirmed human rabies cases, imported from the Philippines and Nigeria, respectively. In case one, hemi-nested revers

Curvularia spicifera in Non-Invasive Fungal Rhinosinusitis: Case Reports and Diagnostic Insights.

The clinical cases described in this text add to the limited literature on chronic and allergic rhinosinusitis caused by dematiaceous fungi, particularly Curvularia spicifera. These cases highlight the growing recognition of fungal infections as a significant factor in the etiology of rhinosinusitis, a condition traditionally attributed to bacterial causes It has become evident that a comprehensive clinical approach, involving diagnostic imaging and laboratory examinations, particularly culture-based analysis, has been crucial in identifying the specific fungal pathogen responsible for the inf

Reciprocal adaptation is critical in enhancing S. aureus and P. aeruginosa biofilm biomass.

Polymicrobial communities impose a great challenge for clinical management of chronic infections. It is a consensus now that microbes exist as aggregated colonies shielded within polymeric matrix. Within this matrix more than one bacterial species can exist either in symbiotic or rival relationships. Herein, we investigated the host-specific interspecies interactions between Staphylococcus aureus and Pseudomonas aeruginosa in chronic rhinosinusitis (CRS). The indirect interaction between the two species was assessed using Transwell co-culture chambers, where S. aureus and P. aeruginosa (n = 3

Facial Erysipelas Following Pressurized Nasal Irrigation in Chronic Sinusitis: A Case Report.

Facial erysipelas is an uncommon but potentially serious manifestation of rhinosinusitis, as involvement of the facial "danger triangle" carries the risk of orbital and intracranial complications. Saline nasal irrigation devices are widely recommended as nonprescription, over-the-counter adjunctive therapy for the symptomatic management of sinonasal disease and are generally considered safe. We report the case of a 60-year-old man who developed rapidly progressive centrofacial erysipelas after one week of using a commercially available pressurized seawater nasal irrigation device for an acute

Sinonasal Nocardia farcinica in a cat with comorbidities.

A 15-year-old spayed female domestic shorthair cat was referred because of a chronic history of swelling over the right eye that had progressed to an open, exudative wound. The lesion had not responded to prolonged antimicrobial treatments that were chosen based on superficial culture results. Before this development, the cat had been diagnosed with International Renal Interest Society (IRIS) stage 2 chronic kidney disease with no other significant health issues. Computed tomography revealed fluid to soft-tissue attenuating material in the sinonasal cavities and concurrent bony lysis. Cytology

Actinomycetoma of the wrist: diagnostic insights from a pathology perspective (case report).

Actinomycetoma is a chronic, progressive subcutaneous infection caused by filamentous bacteria of the Actinomycetales order, characterized by tumefaction, sinuses, and grains. We report a rare case of actinomycetoma affecting the wrist of a 45-year-old male, which presented as a chronic, slowly enlarging swelling with multiple discharging sinuses. Initial clinical suspicion was broad, encompassing various chronic infections or soft tissue tumors. However, definitive diagnosis was established through meticulous histopathological examination of a biopsy specimen, revealing characteristic "sulphu

Actinomycotic Cholecystitis and Pancreatitis: Report of an Unusual Case.

BACKGROUND Actinomycosis is an uncommon bacterial infection caused by the commensal organism Actinomyces spp., which can become pathogenic upon tissue injury or disruption of the mucosal barrier. While primarily recognized for its cervicofacial and abdominopelvic presentations, actinomycosis can also involve the gallbladder and potentially the pancreas, posing diagnostic and therapeutic challenges due to its ability to mimic other conditions. CASE REPORT We present the case of a 26-year-old woman with a history of recurrent acute pancreatitis and cholecystitis, followed by the development of t

Disparity in Coronal Deformity Measurements: Standing Radiographs Versus Navigation in Varus Total Knee Arthroplasty.

BACKGROUND: The third generation of bisphosphonates is clinically in use for patients of osteoporosis or malignancy-linked hypercalcemia. The agents can also produce anti-tumor effects on bone metastasis of several types of tumors. We recently found that one of the agents achieved cytotoxicity to mesothelioma in vitro and in an orthotopic animal model. Mesothelioma is resistant to a number of chemotherapeutic agents, and suppression of local tumor growth is beneficial to the patients since metastasis to extra-thoracic organs is relatively infrequent until a late stage. METHODS/DESIGN: We demo
