Abnormal "hair-like" filaments in chronic maxillary sinusitis . A scanning electron microscopic investigation.

Scanning electron microscopic investigations were performed on the maxillary sinus mucosa of five healthy persons and seven patients with chronic maxillary sinusitis. The occurrence of previously undescribed hair-like filaments in one case of chronic maxillary sinusitis is presented. These abnormal filaments are much longer (about 50--60 micron) and thinner (about 0.08 micron) than normal kinocilia (about 0.2 micron thick and 5--7 micron long), thus exhibiting a hair-like appearance. As the microvilli (cytofila) in maxillary sinus mucosa have the same thickness (about 0.08 micron), these hair-

[How good is the inferior meatal antrostomy in chronic maxillary sinusitis? (author's transl)].

44 inferior meatal antrostomies in 32 patients with chronic maxillary sinusitis were followed at two month intervals over a period of two years. Closure of the antrostomy occurred in 29.4% of the cases reported. Allergy was unimportant for the operative result, nor was an increased frequency of infections demonstrated following surgery. Subsequently, improvement of sinusitis was noticable, even after closure of the fenestration. In contrast to other reports in the literature, mucociliary drainage from the maxillary sinus to the nose occurs through the operatively placed fenestra.

[How effective is the inferior meatal antrostomy in chronic maxillary sinusitis? (author's transl)].

44 inferior meatal antrostomies in 32 patients have been followed over a period of 2 years. Control of the results by means of sinuscopy, ultrasonography, x-ray's, biopsies and subjective as well as objective findings revealed good results in 89% of the patients. Closure of the fenestration occurred in 29,5% but had no bearing on the recurrency rate of sinusitis. Improvement was noticeable even after closure. Allergy did not play an important role. Mucociliary transport was evident through the window, permitting adequate ventilation and drainage.

[Bony changes in chronic odontogenic maxillary sinusitis].

The authors undertook histological studies on the bone of the facial wall of the maxillary sinus from 64 out of 84 patients who had been operated on for chronic maxillary sinusitis of dental origin in the course of 16 months. Apart from mucosal changes, the specimens from 16 patients showed also osseous changes in the form of periosteal reactions of varying extent. With regard to therapy, it is concluded that patients suffering from odontogenic sinusitis should for preference be treated by surgical intervention, because the conservative treatment will not necessarily result in complete healing

Gallium-67 scanning in the differentiation of maxillary sinus carcinoma from chronic maxillary sinusitis.

Gallium-67 scans of 25 patients in whom the clinical symptoms and radiographic findings were suggestive of either maxillary sinus carcinoma or chronic sinusitis proved to be valuable in the differentiation between the two disease processes. Those patients with carcinoma had positive scans, while those with sinusitis had either negative or only weakly positive scans.

[Endonasal antrostomy with endoscopical control for chronic maxillary sinusitis (author's transl)].

A transnasal approach to the maxillary sinus as a standard operation in chronic maxillary sinusitis is described. A mucosal flap in the lower nasal meatus is electrically circumcised, and a window of appropriate size is formed by osteotomy. The removal of pedicled polyps and cysts from the cavity is controlled by endoscopy, while most parts of the thickened mucosa remain in place and may recover and also line the uncovered areas. If necessary, an opening in the facial wall of 5 mm diameter is established for endoscopical control or manipulations. The preliminary results of 56 endonasal sinutom

Penetration of erythromycin stearate into maxillary sinus mucosa and secretion in chronic maxillary sinusitis.

The penetration of oral erythromycin stearate (Abboticin), administered in a dosage of 500 mg three times a day, into the maxillary sinus mucosa and secretion was studied in 15 patients (22 sinuses) operated on for chronic maxillary sinusitis. The average concentration in serum was 2.3 microgram/ml, 1.2 microgram/ml in secretion, and 1.8 microgram/ml in mucosa. These concentrations are highly effective against diplococci and most aerobic and anaerobic streptococci (MIC value 0.06 microgram/ml) but not against Haemophilus influenzae (MIC value for 80% of 2 microgram/ml).

Acidic glycosaminoglycans in the ground substance in paranasal sinus diseases.

Acidic glycosaminoglycans (AGAG) in the normal maxillary sinus mucosae were analysed by electrophoretic separation and densitometric quantitation, and then compared with those in chronic maxillary sinusitis, nasal polyps and maxillary sinus cancer. The total amount of AGAG in the edematous maxillary sinusitis and nasal polyps increased significantly. Both increases were almost twice as large as the normal content. When the changes of each AGAG in the total amount were compared with the normal content, chondroitin sulfate A and C showed the most conspicuous increase, chondroitin sulfate B was r

[DDAVP: alternative to replacement treatment in mild haemophilia A and von Willebrand-Jürgens syndrome (author's transl)].

1-Deamino-8-D-arginine vasopressin (DDAVP), a vasopressin analogue, increases factor VIII levels in plasma. A female carrier of haemophilia A with known bleeding diathesis and markedly reduced factor VIII activity was successfully treated with DDAVP during bilateral Caldwell-Luc's operation for chronic maxillary sinusitis. An estimated 5000 U factor VIII concentrate was calculated to have been saved. DDAVP is the first alternative to replacement therapy in the treatment of moderate and mold forms of haemophilia A and von Willebrand's disease.

[Sturcture and function of the mononuclear phagocytic system (MPS) in chronic rhinosinusitis. A light and electron microscopic investigation (author's transl)].

Neither the concept of the Reticulo-Endothelial-System (RES) Aschoff's (1924) nor that of the Reticulo-Histiocyte-System (RHS) provides a satisfactory framework into which the present knowledge of the phagocytic mononuclear cells can be fitted. Current knowledge concerning morphology, histochemistry (peroxydase and esterase activity), immunology (specific surface antigens, receptors on the cell membranes), function (immune phagocytosis, pinocytosis), kinetics (3H-thymidine labelling) and culture makes it possible to place all highly phagocytic mononuclear cells and their precursors in one syst

Endoscopic and histopathological observations of chronic maxillary sinusitis.

111 patients are studied in order to find possible relations between the endoscopic images and histopathological findings in the chronic maxillary sinusitis. The clasification of the different types of sinusitis is based on the aspect of the mucosa (normoplastic, hypoplastic, polypous, polypoido-polypous) and on the histological features (lymphoplasmocytic, eosinophilic, eosino-lymphoplasmocytic infiltration, low cellularity). The confrontation of the sinusoscopic and histopathological observations give interesting information, such as differenciation between stabilized and evolutive sinusitis

[Subdural empyema as a complication of maxillary sinusitis (description of 3 cases)].

The authors report 3 cases of subdural empyema with different course and outcome. In all of them subdural empyema developed as a complication of chronic maxillary sinusitis. In the first case a coexistent brain abscess was the cause of death. In the second case operation carried out when the patient was in extremely severe condition failed to prevent death since the diagnosis had been made too late. Complete recovery was obtained in the third case owing to early clinical diagnosis confirmed with angiography, followed by early surgical treatment of the primary focus and early craniotomy. The im

Penetration of azidocillin into the secretion and tissues in chronic maxillary sinusitis and tonsillitis.

The penetration of azidocillin into the sinus mucosa and secretion as well as into tonsils and adenoids was studied. Altogehter 73 patients were included and they were arranged in four groups according to age, diagnosis and drug dose. The average azidocillin concentration was satisfactory and predictable in all tissues and well over the MIC for most upper respiratory tract pathogens studied. In the maxillary antrum secretion the values were much lower and in many cases under the MIC for hemophilus. This is thought to depend upon the drug's being bound to proteins in the secretions. The clinica
