Fulminant multi-organ failure associated with cereulide-producing Bacillus cereus after consumption of improperly stored rice.

A male in his 20s with no medical history was found with generalised seizures and profound hypoglycaemia after consuming improperly stored, reheated rice.Initial laboratory tests showed acute liver failure with hyperammonaemia, acute kidney injury and lactic acidosis, which rapidly progressed to multi-organ failure requiring urgent liver transplantation evaluation. With maximal supportive therapy, including glucose, ammonia and glutamine scavengers, renal replacement therapy, haemodynamic and respiratory support, he gradually improved, avoiding transplantation and achieving full neurological re

Foodborne Intoxication Most Likely Linked to Bacillus cereus in a School Canteen Hot Ready-to-Eat Meal.

On Tuesday, September 9, 2025, the Cantonal Laboratory of Basel City received reports of cases of vomiting following the consumption of a macaroni dish served at a school canteen. The outbreak involved 27 children, and the emetic symptoms started 40-90 min after lunch. Microbiological analyses of food samples obtained from the canteen kitchen revealed the presence of B. cereus at 1100 colony-forming units (cfu)/g in leftovers of a béchamel sauce that was an ingredient of the macaroni dish. Nanopore long-read sequencing methodology and the BTyper3 tool were applied to identify B. mosaicus subsp

Functional analysis of SigH in Bacillus cereus ATCC14579 and regulon prediction in the B. cereus group.

Bacillus cereus is a common pathogen causing frequent foodborne outbreaks with diarrheal or emetic syndromes. Its endospores survive mild processing conditions, leading to food spoilage and intoxication. Here, we assess the role of B. cereus ATCC14579 SigH (Spo0H), an alternative transition-phase associated RNA polymerase sigma factor. Comparative phenotypic analysis of a B. cereus ATCC14579 sigH-deletion mutant (ΔsigH) revealed differences in a wide range of phenotypes. First and foremost, ΔsigH lost its sporulation ability. Phenotypic microarray and growth analysis showed a dramatic loss of

The major toxins of Bacillus cereus - Current knowledge on distribution, regulation and mechanism of action.

The ubiquitous soil bacterium Bacillus cereus is one of the major pathogens causing toxin-based foodborne diseases, manifesting as diarrhoeal or emetic syndrome. Main symptoms of the former are diarrhoea and abdominal pain, caused by the proteinaceous enterotoxins haemolysin BL (Hbl), non-haemolytic enterotoxin (Nhe) and cytotoxin K (CytK). Responsible for the latter is the cyclic dodecadepsipeptide cereulide, leading to nausea and vomiting, and, in severe and rare cases, also to organ failure and death. This review first addresses the complex taxonomy of the B. cereus group as well as the dis

Antimicrobial resistance profiling and phenotypic characterization of archived clinical Bacillus paranthracis strains.

Bacillus paranthracis was formally defined as a species in 2017, after decades of carrying the name "emetic Bacillus cereus" based on cereulide production and clustering within the B. cereus sensu lato phylogenetic group III. Commonly associated with foodborne intoxication, reports rarely link B. paranthracis to non-foodborne clinical illness. As such, the new taxonomy and close resemblance of the name to the biothreat pathogen Bacillus anthracis cause confusion in diagnostic and public health settings. To address this issue, B. paranthracis clinical strains (n = 20) from the CDC collection we

A review: Three-gate framework for strain-level safety assessment of Bacillus cereus in food systems.

Bacillus cereus occupies an unusual position in food systems because individual strains may cause emetic intoxication, diarrhoeal toxicoinfection and spoilage, whereas selected strains have been investigated for agricultural, feed and food-biotechnology functions. Species identity alone is therefore an inadequate basis for safety decisions. This review synthesises current evidence and proposes a three-gate framework that converts strain data into use-specific actions. Gate 1 provides an early stop for definitive genomic hazards and transferable antimicrobial-resistance (AMR) determinants. Gate

Cytotoxicity and Genome Characteristics of an Emetic Toxin-Producing Bacillus cereus Group sp. Isolated from Raw Milk.

The Bacillus cereus group frequently contaminates milk and dairy products. Some members of this group can produce the heat-stable pre-formed toxin cereulide, which causes emetic foodborne intoxication. This study characterised emetic B. cereus group isolates from raw cow's milk in the biochemical, genetic, and toxigenic aspects. Of the 158 B. cereus group isolates derived from 99 raw milk samples, 7 (4.43%) harboured cereulide synthetase A (cesA), which encodes a cereulide synthetase associated with the emetic phenotype. Heat-treated culture filtrates from the cesA-positive isolates demonstrat

A poisonous cocktail: interplay of cereulide toxin and its structural isomers in emetic Bacillus cereus.

Food intoxications evoked by emetic Bacillus cereus strains constitute a serious threat to public health, leading to emesis and severe organ failure. The emetic peptide toxin cereulide, assembled by the non-ribosomal peptide synthetase CesNRPS, cannot be eradicated from contaminated food by usual hygienic measures due to its molecular size and structural stability. Next to cereulide, diverse chemical variants have been described recently that are produced concurrently with cereulide by CesNRPS. However, the contribution of these isocereulides to the actual toxicity of emetic B. cereus, which p

Cereulide production capacities and genetic properties of 31 emetic Bacillus cereus group strains.

The highly potent toxin cereulide is a frequent cause of foodborne intoxications. This extremely resistant toxin is produced by Bacillus cereus group strains carrying the plasmid encoded cesHPTABCD gene cluster. It is known that the capacities to produce cereulide vary greatly between different strains but the genetic background of these variations is not clear. In this study, cereulide production capacities were associated with genetic characteristics. For this, cereulide levels in cultures of 31 strains were determined after incubation in tryptic soy broth for 24 h at 24 °C, 30 °C and 37 °C.

Liver failure after Bacillus cereus food poisoning, an under-recognized entity: A case report.

BACKGROUND: Bacillus cereus (B. cereus) is known to cause 2 types of foodborne diseases; the diarrheal and emetic syndromes. They are largely underreported due to their usually self-limiting course. Rare and sometimes fatal cases of liver failure, pulmonary hemorrhage and cerebral oedema have been reported mainly in children and young adults. We present here a case of liver failure associated with B. cereus food poisoning in a middle-aged patient. CASE SUMMARY: A 48-year-old female patient presented to the emergency department for emesis, diarrhea, chills without fever, asthenia and diffuse a

The Food Poisoning Toxins of Bacillus cereus.

Bacillus cereus is a ubiquitous soil bacterium responsible for two types of food-associated gastrointestinal diseases. While the emetic type, a food intoxication, manifests in nausea and vomiting, food infections with enteropathogenic strains cause diarrhea and abdominal pain. Causative toxins are the cyclic dodecadepsipeptide cereulide, and the proteinaceous enterotoxins hemolysin BL (Hbl), nonhemolytic enterotoxin (Nhe) and cytotoxin K (CytK), respectively. Risk of Bacillus cereus in Relation to Rice and Derivatives.

Rice is a very popular food throughout the world and the basis of the diet of the citizens of many countries. It is used as a raw material for the preparation of many complex dishes in which different ingredients are involved. Rice, as a consequence of their cultivation, harvesting, and handling, is often contaminated with spores of Bacillus cereus, a ubiquitous microorganism found mainly in the soil. B. cereus can multiply under temperature conditions as low as 4 °C in foods that contain rice and have been cooked or subjected to treatments that do not produce commercial sterility. B. cereus p

Bacillus cereus food intoxication and toxicoinfection.

Bacillus cereus is one of the leading etiological agents of toxin-induced foodborne diseases. Its omnipresence in different environments, spore formation, and its ability to adapt to varying conditions and produce harmful toxins make this pathogen a health hazard that should not be underestimated. Food poisoning by B. cereus can manifest itself as an emetic or diarrheal syndrome. The former is caused by the release of the potent peptide toxin cereulide, whereas the latter is the result of proteinaceous enterotoxins (e.g., hemolysin BL, nonhemolytic enterotoxin, and cytotoxin K). The final harm

Distribution of the Emetic Toxin Cereulide in Cow Milk.

Bacillus cereus is frequently associated with food-borne intoxications, and its emetic toxin cereulide causes emesis and nausea after consumption of contaminated foods. The major source for contamination is found within contaminated raw materials containing the highly chemically resistant cereulide, independent of vegetative bacteria cells. Up to date, non-existing removal strategies for cereulide evoke the question of how the toxin is distributed within a food sample, especially cow milk. Milk samples with different milk fat contents were incubated with purified cereulide, separated by centri

Acute Liver Failure after Ingestion of Fried Rice Balls: A Case Series of Bacillus cereus Food Poisonings.

Bacillus cereus foodborne intoxications and toxicoinfections are on a rise. Usually, symptoms are self-limiting but occasionally hospitalization is necessary. Severe intoxications with the emetic Bacillus cereus toxin cereulide, which is notably resistant heat and acid during cooking, can cause acute liver failure and encephalopathy. We here present a case series of food poisonings in five immunocompetent adults after ingestion of fried rice balls, which were massively contaminated with Bacillus cereus. The patients developed a broad clinical spectrum, ranging from emesis and diarrhoea to life
