Dupilumab Beyond the Airway: Decreased Morbidity in a Real-World Analysis.

BACKGROUND: Post hoc analyses of clinical trials have characterized dupilumab's adverse effects, yet the real-world impact in chronic rhinosinusitis with nasal polyps (CRSwNP) and asthma is not well described. This study aims to characterize the risks of lymphoma, cardiovascular events, eosinophilia, joint pain, inflammatory arthritis, and sleep apnea in dupilumab-treated CRSwNP and/or asthma patients compared to those not taking dupilumab, and to other biologics. METHODS: This retrospective cohort study used TriNetX, a de-identified database containing over 100 million patient records. Demog

The Elusive Diagnosis: A Case of Extranodal Natural Killer (NK)/T-Cell Lymphoma.

Extranodal natural killer (NK)/T-cell lymphoma is a rare malignant tumor, more prevalent in Asia and Latin America, and associated with Epstein-Barr virus infection. Its diagnosis is challenging due to its clinical presentation, which may mimic chronic sinusitis, dacryocystitis, or granulomatous diseases. This lymphoma follows a rapid and aggressive course, with a poor prognosis. We report the case of a 71-year-old man presenting with nasal obstruction, clear rhinorrhea, and facial heaviness. Initially, acute dacryocystitis and periorbital cellulitis were suspected and treated with antibiotics

Association Between Biologic Therapy and Reduced Acute Rhinosinusitis Episodes: A Retrospective Database Study.

OBJECTIVE: To assess whether biologics (dupilumab, mepolizumab, and omalizumab) used for conditions other than chronic rhinosinusitis with nasal polyps (CRSwNP) reduce the frequency of acute bacterial rhinosinusitis (ABRS) episodes. METHODS: This retrospective cohort study utilized the All of Us Research Program database to identify patients treated with dupilumab, mepolizumab, or omalizumab, excluding those with CRSwNP. A control group matched by race, gender, and comorbidities (asthma, allergic rhinitis, atopic dermatitis, smoking status) was created using propensity score matching (1:1 rat

Chronic Granulomatous Fungal Rhinosinusitis Mimicking a Giant Cell Tumor of the Maxilla: A Diagnostic Pitfall.

Chronic granulomatous fungal rhinosinusitis is an uncommon form of invasive fungal sinusitis that typically affects immunocompetent individuals and often presents as a slowly progressive mass lesion, closely mimicking malignancy clinically and radiologically. We report the case of a 56-year-old woman who presented with a painless swelling over the left maxillary region for four months. She had a history of a similar lesion at the same site nine years earlier, which had been surgically excised elsewhere and diagnosed as a giant cell tumor of the maxilla. Magnetic resonance imaging revealed an e

Bilateral Thalamic Infarction Secondary to Deep Cerebral Venous Thrombosis: A Diagnostic Challenge Associated With Hormonal Therapy.

Deep cerebral venous thrombosis (DCVT) represents an uncommon and potentially devastating variant of venous cerebrovascular disease, characterized by the occlusion of the Galenic system and the straight sinus. This pathology disrupts the drainage of critical diencephalic structures, resulting in venous congestion that radiologically manifests as symmetrical bilateral thalamic edema or infarction. We report the case of a 56-year-old female with a history of chronic oral estradiol valerate and levonorgestrel use for hormone replacement therapy, who presented with acute neurological impairment, i

Conduction Aphasia in a Case of Left Cortical Veins and Left Lateral Sinus Thrombosis Due to Multiple Risk Factors: A Case Report and Review of the Literature.

Aphasia is a complex neurological syndrome that includes a multitude of signs and symptoms that describe a patient's inability to use language (understanding and producing spoken and/or written language) after it has already been acquired, which is caused by cerebral lesions situated in the dominant (left) cerebral hemisphere in right-handed people. Aphasia has a prevalence of 25-30% in acute ischemic stroke (especially in arterial infarcts). In patients who suffered cerebral venous and dural sinuses thrombosis (CVST), aphasia has been noticed in almost 20% of cases, its presence being conside

Unveiling natural anti-inflammatory compounds for sinusitis treatments by modulating FOXP3, C5aR1, and LIF.

Chronic rhinosinusitis (CRS) is a condition with a significant healthcare and economic burden, with a prevalence of 12% in the West. The existing therapies mainly attenuate downstream inflammatory mediators but do not address the underlying immune-regulatory abnormalities that drive disease recurrence and lifelong treatment. Evidence has shown that a regulatory axis involving forkhead box P3 (FOXP3) + regulatory T cells (Tregs), the complement component 5a receptor 1 (C5aR1), and leukemia inhibitory factor (LIF) is crucial for regulating immune tolerance in sinonasal tissues. This review discu

Nasal irrigation increases upper-airway microbial diversity: a prospective pilot study.

BACKGROUND: Nasal irrigation is widely recommended for chronic rhinosinusitis and allergic rhinitis, but its effects on the upper-airway microbiome remain unclear. OBJECTIVE: To determine whether 30 days of buffered isotonic saline nasal irrigation alters bacterial diversity in the nasal cavity, nasopharynx, and oral cavity. MATERIAL AND METHODS: Ten volunteers performed twice-daily irrigation for 30 days. Swabs from the nasal cavity, nasopharynx, and posterior tongue were collected at baseline and day 30. Bacterial communities were analysed by 16S rRNA gene sequencing. The primary endpoint

Acromegaly Discovered After Head Trauma: Multivessel Coronary Disease and Postoperative Meningitis Complicating a Pituitary Macroadenoma.

Acromegaly is a rare endocrine disorder caused by chronic growth hormone (GH) excess, most often from a pituitary somatotroph adenoma. Because the somatic and metabolic features develop slowly, the diagnosis is often delayed for years. We report the case of a 59-year-old man whose acromegaly was identified incidentally after a 1.6 cm pituitary macroadenoma was found on MRI obtained for trauma evaluation following a bicycle accident. History and examination revealed several long-standing features of acromegaly, including widely spaced teeth, mild prognathism, increased shoe and hand size, sever

Surgical management of persistent maxillary sinusitis with subtle residual symptoms following lateral sinus floor elevation: a report of three cases.

Postoperative graft infection and subsequent acute maxillary sinusitis are infrequent complications following sinus floor elevation (SFE). However, inadequate management may result in persistent infection with only subtle residual symptoms. This case study presents three patients who developed persistent subclinical graft-related infection following SFE. Patient 1 underwent staged implant placement (two implants) after lateral SFE, whereas the other two underwent simultaneous implant placement and SFE (four implants in Patient 2, two implants in Patient 3). Between 2 and 3 weeks postoperativel

Persistent Rhinosinusitis-Like Symptoms Revealing a Lactotroph Pituitary Neuroendocrine Tumour: A Case Report.

Pituitary neuroendocrine tumours (PitNETs) commonly present with symptoms related to mass effect or hormonal dysfunction, including headache, visual disturbances, and endocrine abnormalities. However, atypical presentations without neurological or hormonal "red flag" symptoms may delay diagnosis, particularly in primary care settings where benign upper respiratory conditions are more frequently encountered. We report the case of a 56-year-old postmenopausal woman who presented with a five-month history of persistent postnasal drip, chronic non-productive cough and a sensation of nasal congesti

Orbital Plasmacytoma as Initial Presentation of Multiple Myeloma.

Multiple myeloma (MM) is a hematologic malignancy characterized by clonal proliferation of plasma cells in the bone marrow. It represents approximately 10% of hematologic malignancies and is the most common primary bone malignancy. This disease usually occurs between the ages of 40 and 70, although it is more frequently seen in patients over 65 years of age. On the other hand, extramedullary plasmacytoma-related diseases have a global annual incidence of three per 100,000 people. Orbital involvement in MM is considered rare. Orbital plasmacytomas represent less than 1% of all orbital lesions,

Consensus Review: Predicting Recurrence in Chronic Rhinosinusitis With Nasal Polyps: Multidisciplinary Insights From the RELAPSE Project.

Chronic rhinosinusitis with nasal polyps (CRSwNP) is a disease with a high incidence of postoperative recurrence despite medical and surgical treatments. However, a universal definition of recurrence and its determinants is lacking. Our objective was to establish a clear definition of recurrence and reach consensus on the contributing factors. A consensus study was conducted using the Delphi method. The literature on recurrence of CRSwNP was reviewed and assessed by a multidisciplinary scientific committee. The committee designed a 2-round online Delphi questionnaire addressing recurrence in C

A Case of Nasal Myiasis Without Any Predisposing Factors: An Uncommon Presentation.

Nasal myiasis is an uncommon human parasitic disease affecting the nasal cavity. In otolaryngology, myiasis is mostly encountered as aural myiasis and oral myiasis, but occurs rarely as nasal myiasis. It typically affects individuals with poor hygiene, chronic illness, or immunosuppression. Cases in immunocompetent individuals are exceptional and can present diagnostic challenges. We report the case of a 32-year-old immunocompetent female who presented with the spontaneous expulsion of small larvae through the nostrils. Live larvae were observed in both nasal cavities during endoscopic examina

Phenotypic comorbidity network analysis of nasopharyngeal carcinoma: a multicenter study based on hospital discharge records.

BACKGROUND: Nasopharyngeal carcinoma (NPC) patients frequently have multiple comorbidities that affect treatment efficacy and survival. Prior studies focused only on individual common comorbidities, lacking a comprehensive analysis of the full spectrum of chronic diseases in NPC patients from endemic areas. This study adopted phenotypic comorbidity networks (PCNs) based on large-scale hospital discharge data to systematically characterize the overall comorbidity patterns of endemic NPC patients. METHODS: This retrospective cohort study was conducted using data from NPC patients identified in
