Identifying predictive baseline characteristics for super response to dupilumab in patients with severe uncontrolled asthma.

Dupilumab is an IgG4 human monoclonal antibody which acts upon the type 2 inflammatory pathway. It has been shown to have a significant impact on patients with severe asthma through reducing exacerbations and improving symptom control. A subgroup of patients experiences excellent symptom control and elimination of exacerbations within 12 months of starting dupilumab and are termed super responders. Here we retrospectively identify real-life patients on dupilumab at a single centre to review the baseline characterises comparing those who achieve super response against those that do not. 48 pati

Treatment Persistence and Factors Associated With Discontinuation in Real-World Biologic Therapy for Chronic Rhinosinusitis With Nasal Polyps.

BACKGROUND: Biologic therapies are effective for severe, uncontrolled CRSwNP, yet real-world data on treatment persistence remain limited. METHODS: This retrospective single-center study included 95 adults with CRSwNP who initiated dupilumab (88.4%), mepolizumab, or omalizumab between 2021 and 2025. The primary outcome was persistence with the index biologic therapy. Discontinuation was a permanent cessation, switching, or a ≥ 90-day dosing gap. Switching ended index-biologic persistence but not biologic therapy overall. Response was graded with European Position Paper on Rhinosinusitis and N

Inhaled, nasal and topical corticosteroid burden and association with corticosteroid-associated adverse events.

BACKGROUND: Patients with type 2 inflammation-associated diseases are often treated with multiple formulations of corticosteroids. Long-term risks of using combinations of local corticosteroids are poorly characterised. OBJECTIVE: Using Danish nationwide databases, investigate associations between local corticosteroid exposure and 11 corticosteroid exposure-associated conditions compared to non-users, and eventual dose-response relationship for exposed individuals. METHODS: Systemic corticosteroid-naïve adults who redeemed inhaled, nasal and/or topical corticosteroids 2000-2012 were matched

Predictive Value of Preoperative Examinations for Early Prognosis After Functional Endoscopic Sinus Surgery in Chronic Rhinosinusitis With Nasal Polyps.

BACKGROUND: Functional endoscopic sinus surgery (FESS) remains a primary treatment modality for chronic rhinosinusitis with nasal polyps (CRSwNP) refractory to medical management. This study aimed to evaluate the predictive value of routine preoperative examinations for early surgical outcomes following FESS in patients with CRSwNP. METHODS: This prospective cohort study evaluated whether routine preoperative assessments predict early outcomes after FESS in 181 patients with bilateral CRSwNP. Preoperative evaluations included Lund-Mackay CT scores, Lund-Kennedy endoscopy scores, symptom Visua

Efficacy of Dupilumab for Patients with Chronic Rhinosinusitis without Nasal Polyps: a Randomized Double-Blind Placebo Controlled Phase II Study.

BACKGROUND: Chronic rhinosinusitis without nasal polyps (CRSsNP) is a heterogeneous inflammatory disease affecting up to 8% of the population. No biologic therapies are currently approved for CRSsNP, and emerging data suggest that T2-high endotypes may respond preferentially to targeted therapy. OBJECTIVE: To evaluate the efficacy and safety of dupilumab, a monoclonal antibody targeting the interleukin-4/interleukin-13 signaling pathway, in patients with moderate-severe CRSsNP and to explore differential responses by T2-inflammatory endotype. METHODS: In a 6-month, randomized, double-blind,

Assessing the Mucosal Inflammatory Endotype of Obesity in Chronic Rhinosinusitis.

BACKGROUND: Chronic rhinosinusitis (CRS) is a heterogeneous inflammatory syndrome of the paranasal sinuses that is defined phenotypically as either with nasal polyps (CRSwNP) or without nasal polyps (CRSsNP). More recently, inflammatory markers have been used to cluster patients into putative endotypes that are presumed to have a common underlying etiology. Obesity is known to create a proinflammatory state in similar chronic inflammatory conditions such as asthma. However, the potential impact of obesity in CRS has not been investigated. METHODS: Samples were collected from 512 patients with

Impact of dupilumab on Sino-Nasal Outcome Test-22 (SNOT-22) domains: Focus on the ear.

BACKGROUND: Chronic rhinosinusitis with nasal polyposis (CRSwNP) is frequently associated with otologic symptoms because of Eustachian tube dysfunction. However, the clinical relevance of otologic symptoms in patients with CRSwNP and their response to biologic therapy remains poorly characterized. OBJECTIVE: To investigate baseline determinants of ear-related symptoms and evaluate the impact of dupilumab treatment. METHODS: This post-hoc analysis of the Dupilumab In Severe Asthma (DISA) study included 23 adults with CRSwNP and severe uncontrolled asthma. Participants underwent a 12-week trea

Integrated-omics endotyping of patients with chronic rhinosinusitis with nasal polyps and their response to glucocorticoid therapy.

BACKGROUND: Integrated omics approaches provide deeper insights into disease heterogeneity, but combined host immune and nasal microbiota profiles in chronic rhinosinusitis with nasal polyps (CRSwNP) remain poorly characterized. OBJECTIVE: This study aimed to identify biologically distinct CRSwNP endotypes by integrating transcriptomic and microbiota data from nasal brushings and to assess responses to corticosteroid therapy across these endotypes. METHODS: Baseline transcriptomic and microbiota data from the multicenter COMPASS cohort (ClinicalTrials.gov registration: NCT05598424) were inte

The Upper Airway as a Treatable Trait: A United-Airway Framework for Biologic Selection in Asthma.

The treatable traits approach reframes asthma management around measurable and modifiable factors rather than diagnostic labels, and it has become a cornerstone of the precision medicine model in chronic airway disease. Among the traits that shape asthma control, corticosteroid burden and treatment response, upper airway comorbidity is both prevalent and frequently under-recognised. Allergic rhinitis, chronic rhinosinusitis with and without nasal polyps, and olfactory dysfunction share anatomical, immunological and clinical links with the lower airway, and upper-airway comorbidities are increa

CT-based deep learning for identifying sinonasal inverted papilloma: a single-center retrospective study.

BACKGROUND: This study aimed to develop a CT-based deep learning model as a decision-support system for the classification of sinonasal inverted papilloma (IP), nasal polyps, and normal paranasal anatomy. METHODS: A retrospective dataset of 871 patients who underwent paranasal CT imaging between 2020 and 2024 was analyzed. All diagnoses were confirmed histopathologically. Two representative CT slices per patient were selected at the patient level by an experienced radiologist blinded to final diagnosis. The dataset was split into 70% training and 30% testing sets, with 20% of the training dat

Eosinophil-Mast Cell Inflammation in Middle Ear Effusion Associated With Chronic Rhinosinusitis With Nasal Polyps.

Chronic rhinosinusitis with nasal polyps (CRSwNP) is predominantly associated with type 2 inflammation, although the inflammatory profile may involve both eosinophils and mast cells. Middle ear disease in CRSwNP is usually attributed to Eustachian tube dysfunction, but inflammatory involvement of the middle ear may also occur. We report the cytological evaluation of middle ear effusion from a patient with CRSwNP. May-Grünwald-Giemsa staining revealed eosinophils, mast cells, and extracellular granules consistent with degranulation. The simultaneous presence of these cellular components suggest

Predictive biomarkers of biologic therapy response in chronic rhinosinusitis with nasal polyps: a systematic review.

BACKGROUND: Biologic therapies have transformed management of severe chronic rhinosinusitis with nasal polyps (CRSwNP), but response variability persists. Baseline biomarkers may identify patients most likely to benefit, yet no marker is validated for routine clinical use. This systematic review evaluated baseline biomarkers predicting clinical and endoscopic response to biologic therapy in adults with CRSwNP. METHODS: A systematic search identified studies reporting baseline biomarkers and response to biologics (dupilumab, omalizumab, anti-IL-5 agents) in adult CRSwNP. Ten studies with 489 p

Real-world two-year outcomes of mepolizumab in patients with chronic rhinosinusitis with nasal polyps and coexisting severe asthma.

PURPOSE: Chronic rhinosinusitis with nasal polyps (CRSwNP) and severe asthma frequently coexist as manifestations of type 2 airway inflammation. Although mepolizumab has demonstrated efficacy in both conditions, long-term real-world data on integrated airway disease control remain limited. We evaluated the long-term effectiveness of mepolizumab in patients with CRSwNP and coexisting severe asthma over 2 years. METHODS: We conducted a longitudinal real-world observational cohort study at a tertiary referral center. Baseline data were collected retrospectively, and follow-up assessments were pr

Chronic Rhinosinusitis With Nasal Polyposis in EGPA: A Real-Life Multidimensional Analysis Through Mepolizumab Dosing and Role of Adjunctive Endoscopic Sinus Surgery (ESS).

BACKGROUND: Eosinophilic granulomatosis with polyangiitis (EGPA) is a rare necrotizing vasculitis involving upper- and lower-airways. Despite biologics, sinonasal disease often persists. Evidence on ENT trajectories under anti-IL5 treatment and role of endoscopic sinus surgery (ESS) remains limited and is often described without standardized assessment tools. This study provides a systematic, multidimensional evaluation of sinonasal outcomes under biologic therapy and analyzes the role of ESS in a real-life cohort. METHODS: We retrospectively analyzed 52 EGPA patients with a 24-month follow-u
