BACKGROUND: Chronic rhinosinusitis (CRS) is a widespread inflammatory disease of the paranasal sinuses and the standard treatment includes surgery and the administration of corticosteroids or antibiotics. Antibiotics are well known to influence the microbial composition of the gut, including a reduced bacterial diversity and an increased antibiotic resistance. This underlines the need for alternative treatment approaches, especially regarding vulnerable patients such as children. Therefore, we evaluated the intestinal microbiota of CRS patients regarding plant-based anti-inflammatory monoterpe

Chronic rhinosinusitis (CRS), with or without nasal polyps (CRSwNP/CRSsNP), is a prevalent inflammatory condition of the sinonasal mucosa, for which intranasal corticosteroids (INCS) are widely used as first-line therapy. This systematic review evaluated the efficacy and safety of INCS, used alone or with adjunctive treatments, in improving clinical outcomes in CRS patients. A comprehensive literature search was conducted across PubMed, Cochrane Library, Scopus, Web of Science, and the Virtual Health Library (VHL) through May 2025, including randomized controlled trials (RCTs) assessing INCS e

BACKGROUND: Central compartment atopic disease (CCAD) is a nasal inflammatory condition associated with allergy and categorized as type 2 dominant chronic rhinosinusitis with nasal polyps (CRSwNP). Sinus opacification in this condition is secondary to obstruction from central tissue remodeling. Although the literature supports a unique phenotype of CCAD, controversy persists due to variations in published studies. Standardized diagnostic criteria for CCAD are lacking. The current literature was systematically evaluated to propose a set of diagnostic criteria for CCAD.
METHODS: A comprehensive

Nasal obstruction syndrome (NOS) is inherently complex due to the combination of nasal anatomy and physiology and pathophysiologic processes that together affect airflow, filtering, smell, and the general health of the respiratory system. The nasopharynx consists of different structures together: the septum, turbinates, and nasal valves that together perform the jobs associated with the nasal polyp, which is to regulate air conditioning and mucociliary clearance. Changes that can be recognized in the septum, turbinates, and nasal valve will individually and/or collectively affect the potential

INTRODUCTION: Nasal congestion/obstruction (NC) contributes to the high disease burden in patients with severe chronic rhinosinusitis with nasal polyps (CRSwNP). Patient perception of NC may not accurately reflect nasal patency, while peak nasal inspiratory flow (PNIF) is an objective method with established thresholds for normal nasal airflow. This analysis evaluated the association between NC and PNIF and the impact of baseline PNIF on dupilumab efficacy in patients with severe CRSwNP.
METHODS: This was a post hoc analysis of patients treated with placebo or dupilumab 300 mg every 2 weeks i

BACKGROUND: Chronic rhinosinusitis with nasal polyps (CRSwNP) is predominantly driven by type 2 inflammation. The biologics dupilumab and omalizumab, which target drivers and mediators of type 2 inflammation (interleukin [IL]-4/IL-13 signaling and immunoglobulin E [IgE], respectively), are efficacious in treating CRSwNP but direct comparisons are few. In EVEREST (EValuating trEatment RESponses of dupilumab versus omalizumab), the first head-to-head trial in respiratory biologics, we aimed to compare the efficacy and safety of dupilumab and omalizumab in patients with severe CRSwNP who had mild

Chronic rhinosinusitis with nasal polyps (CRSwNP) is a debilitating inflammatory disease associated with high recurrence rates and limited response to current therapies. Tezepelumab, a human monoclonal antibody targeting thymic stromal lymphopoietin (TSLP), has emerged as a promising upstream biologic intervention. In the phase 3 WAYPOINT trial, tezepelumab significantly reduced nasal polyp score (-2.07), nasal congestion severity (-1.03), and SNOT-22 scores (-28.4), while decreasing the need for endoscopic sinus surgery by 98% versus placebo. Post hoc analyses of the NAVIGATOR trial showed SN

Chronic rhinosinusitis (CRS) is a frequent inflammatory disorder that can markedly impair patients' quality of life. Endoscopic sinus surgery (ESS) remains the treatment of choice for refractory cases, but postoperative inflammation and adhesion formation are still major concerns. Corticosteroids are often prescribed to address these complications, although systemic administration carries the risk of adverse effects. This has led to growing interest in localized, topical corticosteroid therapies. This review followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISM

INTRODUCTION: Chronic rhinosinusitis (CRS) is a heterogeneous inflammatory disease with variable treatment outcomes based on endotypes. Perioperative stratification of these patients remains a significant challenge. In the present study, we sought to use machine learning (ML) approaches in conjunction with clinical and inflammatory biomarker data to identify patients presenting for revision surgery as a proxy for recalcitrant disease and better understand which data types may be useful in classifying patients.
METHODS: Six hundred thirty-four CRS patients were included. All features were cate
