Endoscopic sinus surgery is commonly used to treat chronic sinusitis. Subjects were 79 chronic sinusitis patients--50 men and 29 women aged 17 to 79 years (average: 50.6 years) undergoing endoscopic sinus surgery in our department from January 1993 to December 1997. In the pathology of recidive infection of upper and lower respiratory tract the chronic diseases of the paranasal sinuses play an important role. The authors ordered for their FESS patients a control examination, 1 year after operation. They present examination data of 30 patients. The authors describe a case of a man with a large choanal polyp descending into the hypopharynx. The man presented additional symptoms of full aspirin intolerance and pollinosis caused with grass pollen antigens. The Caldwell-Luc procedure was performed and the choanal polyp was successfully removed through the mouth. Postoperatively treatment with leukotriene antagonists and with an intranasal steroids with positive clinical effect was begun. However the patient discontinued the therapeutic process causing polyp recurrence within a short time.

Cystic fibrosis is a genetic disorder of the exocrine glandular function leading to the formation of thick mucus in the nasal and paranasal cavities. Nearly all the patients develop sinus disease but not all have complaints. Two different sinus diseases can be found: chronic sinusitis and nasal polyposis. Only the symptomatic cases have to be treated. Medical treatment consists of antibiotherapy for chronic sinusitis and nasal steroid sprays for nasal polyps. Sinus surgery is frequently needed in case there is a persistence of symptoms (and before lung transplantation).
