Endoscopic Marsupialization and Drainage of a Septated Sphenoid Sinus Mucocele in a Revision Skull Base Setting.

Sphenoid sinus mucoceles are rare lesions that account for less than 2% of all sinonasal mucoceles. In revision cases, surgery is more challenging because prior operations alter the normal anatomy and remove important surgical landmarks. A 79-year-old male presented with a headache 10 years after prior sphenoid surgery for fungal disease. Preoperative MRI showed a multiloculated, expansile sphenoid mucocele. Bony erosion involved the sellar floor and the right internal carotid artery (ICA) canal, with additional erosion of the mid-clivus. Endoscopic transnasal marsupialization was performed us

A case of a paranasal sinus mucocele of the frontal sinus invading the anterior cranial fossa and orbital wall.

This report details a rare case of a frontal-ethmoid sinus mucocele with concurrent invasion of both anterior cranial fossa and orbital wall, manifesting primarily with ophthalmologic symptoms. A 64-year-old woman presented with right periorbital swelling accompanied with mild pain, photophobia, and epiphora. Endoscopic sinus surgery revealed extensive destruction of the skull base bone. Intraoperative findings included an intact dura mater without cerebrospinal fluid leakage. Histopathology identified a benign mucocele lined by respiratory epithelium. This case represents an exceptional prese

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Nucleic Acid-Functionalized Gold Nanorods Modulate Inflammation and Dysregulated Intestinal Barriers for Treatment of Ulcerative Colitis.

Background: Nonsuicidal self-injuries (NSSIs) are an important contributing factor to adolescent suicide, and various shared factors influence the risk of both NSSIs and suicide attempts (SAs). Both are important predictors of suicide and are part of a continuum of suicidal behaviors. Further exploration of the relationship between adolescent NSSI and SA may facilitate suicide prevention efforts. Methods: An online survey was conducted among 9,140 participants. Network analysis methods were used to explore expected influence (EI), bridge expected influence (BEI), edge weights, and differences

Trauma-Informed Care for Intimate Partner Violence and Sexual Assault: Simulated Participant Cases for Emergency Medicine Learners.

INTRODUCTION: Emergency medicine (EM) providers often care for patients who present with concerns related to sexual assault (SA) or intimate partner violence (IPV). However, many providers feel uncomfortable discussing SA and IPV with patients. We aimed to design a curriculum using trauma-informed care principles to improve self-assessed competency in caring for this patient population. METHODS: EM learners, including residents, EM physician assistant fellows, and medical students, attended a 25-minute didactic session introducing the concept of trauma-informed care and important questions to

Detailed evaluation of the risk of infraorbital nerve injury in postoperative maxillary cyst surgery.

OBJECTIVE: Postoperative maxillary cyst (POMC) develops as a delayed complication after radical surgery. The infraorbital nerve (ION) is a terminal branch of the trigeminal nerve and is one of the low-frequency collateral injuries that may occur in maxillary sinus surgery. The risk of ION injury may increase during POMC surgery due to anatomical variations: however, this has not yet been examined in detail. Therefore, we herein investigated variations in the ION in POMC cases with a focus on its relationship with the aperture site of the cyst, to clarify the risk of injury. METHODS: A multifa

Surgical treatment of frontal sinus fractures: a retrospective evaluation of 116 cases.

There are various treatment options for frontal sinus fractures. They mainly depend on whether the anterior or posterior table and the frontal sinus outflow tract (FSOT) are affected. Our study retrospectively assessed the surgical management of 116 frontal sinus fractures in terms of the different surgical treatment options as well as the resulting complications. In our cohort, all cases of displaced anterior table fractures (n = 73) were treated by open reduction and fixation (ORIF). In cases of posterior table fractures (n = 43), cranialization, obliteration, sinus preserving measures, or n

Imposter behind the eye: spheno-ethmoidal mucocele mimicking meningocele with compressive optic neuropathy.

A male patient in his mid-30s presented with progressive right eye proptosis and vision loss, accompanied by retro-orbital and occipital pain. He had a history of skull base trauma and endoscopic repair for cerebrospinal fluid rhinorrhoea 7 years earlier. Imaging revealed a lesion adjacent to a cribriform plate defect, initially suggesting a non-expanding meningocele. However, intraoperatively, the lesion was identified as a spheno-ethmoidal mucocele with orbital extension. Endoscopic drainage and marsupialisation were performed, resulting in immediate resolution of proptosis and marked improv

Beyond endoscopy: the ongoing role of frontal osteoplastic flap surgery-a case series and evidence-based analysis.

INTRODUCTION: Frontal sinus surgery is among the most technically demanding areas in rhinology due to the complex anatomy of the frontal recess and proximity to critical structures such as the orbit and anterior cranial fossa. Although endoscopic sinus surgery (ESS) is the standard for most sinonasal pathologies, the frontal osteoplastic flap (OPF) remains important in select clinical scenarios. OBJECTIVES: To evaluate the role of OPF in current rhinologic practice by presenting a retrospective case series and reviewing contemporary literature. METHODS: A retrospective analysis was conducted

Maxillary Sinus Augmentation and Implant Installation in Patients With Pseudocysts and Mucous Retention Cysts.

This case study systematically assessed diverse approaches to sinus augmentation in the presence of sinus pathology. Three patients were carefully selected and categorized as pseudocyst (PsC) (type 1), mucous retention cyst (MRC) smaller than 20 mm (type 2), and MRC larger than 20 mm in size (type 3). All patients underwent sinus augmentation procedures, with each case utilizing a unique surgical approach. Spontaneous drainage was performed for the patient with PsC (type 1), followed by uncomplicated sinus augmentation. For the patient with an MRC smaller than 20 mm (type 2), aspiration of the

Mucosal cyst aspiration in conjunction with maxillary sinus elevation: A clinical cohort study.

INTRODUCTION: Patients with mucosal cysts in the maxillary sinus require special consideration in patients who require implant therapy for the restoration when undergoing implant therapy for the restoration of the posterior maxillary dentition. Treatment strategies for these clinical situations remain controversial in the literature. Thus, this study seeks to describe a safe and effective therapeutic strategy for sinus augmentation in patients with pre-existing maxillary antral cysts. METHODS: A total of 15 patients and 18 sinuses were consecutively enrolled in this cohort study and underwent

[The evolution of ideas about cyst-like stretching of the paranasal sinuses. Part I].

Treatment of patients with severe chronic and recurrent forms of sinusitis, complicated by pathological stretching of the paranasal sinuses, is not a trivial task. This is especially true for those clinical cases where a pathological increase in the size of the sinus leads to widespread destruction of its walls and may be accompanied by serious complications from adjacent structures. The paper presents an analytical review of publications on the topic of pathological stretching of the paranasal sinuses. Potential factors influencing the development of this pathology, mechanisms of pathogenesis

Severe facial pain and removal of maxillary sinus mucous retention cyst.

Maxillary sinus retention cysts (MRCs) are typically asymptomatic and require no treatment. An early 30s man presented with a decade-long history of severe left-sided chronic facial pain (CFP). Multiple prior treatments resulted in an edentulous patient with persistent pain. Imaging revealed a dome-shaped radiopaque change in the left maxillary sinus. History and clinical examination suggested persistent idiopathic facial pain, and doubts about the outcome of a surgical intervention were explained to the patient. Surgical removal of the MRC via lateral antrotomy led to complete symptom resolut

Frontal Sinus Mucopyocele Presenting as a Painful Subcutaneous Forehead Mass in a Patient with Chronic Sinusitis.

Mucoceles of the paranasal sinuses are benign, expansile lesions that develop secondary to sinus ostia obstruction. Presenting signs and symptoms vary widely but frequently include frontal headache and swelling, as well as visual changes and globe displacement depending on orbital involvement in the case of frontal sinus mucoceles. Given the potential for orbital or intracranial involvement, urgent imaging with computed tomography (CT) is important for patients with symptoms concerning for a frontal sinus mucocele. Definitive treatment is surgical. In this article, we report a case of a 50-yea
