Monitoring mepolizumab treatment in chronic rhinosinusitis with nasal polyps (CRSwNP): Discontinue, change, continue therapy?

BACKGROUND: Chronic rhinosinusitis with nasal polyps (CRSwNP) is a multifactorial inflammatory disease of the mucous membranes of the nose and sinuses. Eosinophilic inflammation is described as a common endotype. The anti-IL-5 antibody mepolizumab was approved in November 2021 as an add-on therapy to intranasal glucocorticosteroids for the treatment of adults with severe chronic rhinosinusitis with nasal polyps when systemic glucocorticosteroids or surgery do not provide adequate disease control. While national and international recommendations exist for the use of mepolizumab in CRSwNP, it ha

A systematic review of the sinonasal microbiome in chronic rhinosinusitis.

BACKGROUND: The interaction between the host and microorganisms in chronic rhinosinusitis (CRS) is poorly understood and is a growing area of interest. More recently, methodologies have been developed to assess the microbiome without the use of culture by analyzing the bacterial 16S ribosomal RNA gene. We reviewed the microbiome literature to better understand the role of microbes in CRS. METHODS: Systematic review of studies that used the 16S ribosomal RNA gene deep sequencing. RESULTS: Nine publications met the search criteria. Eight studies evaluated the microbiome in controls (total, 83

Otorhinolaryngological manifestations in granulomatosis with polyangiitis (Wegener's).

Granulomatosis with polyangiitis (Wegener's, GPA) is an uncommon disease of unknown etiology classically involves the ELK triad of the ear, nose, throat (E), lungs (L) and kidneys (K) with necrotizing granulomatous inflammation and vasculitis. Most of the initial symptoms begin in the head and neck region with a wide spectrum of involvement of any site ranging from the nasal septum, paranasal sinuses, oral mucosa, larynx and even the external, middle and internal ear. Diagnosis may be delayed because the onset is heterogeneous and sometimes limited to one organ. The pathologic findings of a ch

Microscopic endonasal surgery of nasal and paranasal cavities.

OBJECTIVE: To present the experience of our department on microscopic surgery of the nose and sinuses and compare our results with those of other standard approaches. Furthermore we attempt to specify the main indications for the use of the method. METHODS: During the period between January 1990 and December 2002, the prementioned technique was performed in a total of 250 patients. The preoperative evaluation, surgical technique and postoperative management of all patients are described in detail. RESULTS: Most common diagnoses included nasal polyps and chronic rhinosinusitis. The average ag

[Examination of the nose and sinuses].

The nasosinusal diseases are dominated by the rhinosinusitis. Within this framework, the data drawn from the clinical history and the endoscopy are essential. The imaging is here of secondary importance. It is especially useful to assess the extension of the affection and the surgical difficulties which can be encountered in the treatment of the inflammatory or infectious chronic diseases. On the other hand in the event of tumour it can be helpful to determine the etiologic diagnosis and the extension of the disease. The importance of some subjective symptoms as the nasal obstruction will be s

Acute sinusitis in the rabbit model: histologic analysis.

OBJECTIVES: Characterize the histology of the rhinogenic model of sinusitis and compare this with existing models of sinusitis. STUDY DESIGN: Prospective controlled trial in animals. METHODS: New Zealand white rabbits were implanted with a synthetic sponge, which was then impregnated with Streptococcus pneumoniae bacteria. After a specified time the animals were sacrificed, and whole-mount sectioning of both the infected and noninfected sinuses was performed. The sinuses were carefully examined for evidence of inflammatory changes. RESULTS: This model produced a sinus infection that is char
