[Lipid complex in subpalatine tonsils in normal state and pathology].

A fat-acid compound of lipids in the subpalatine tonsils was studied by gas chromatography in normal state, chronic tonsillitis and hypertrophy of tonsils. We have determined that in saturation of the lipid complex raised in the subpalatine tonsils due to an increase in the level of arachidonic acid. It gives evidence for an activation of the lipid peroxidation leading to a pathological process.

Upper airway obstruction-related sleep apnea in a child with thalassemia intermedia.

Obstructive sleep apnea can be caused by hypertrophy of tonsils and adenoids or neuromuscular diseases. The authors describe a child with thalassemia intermedia in whom severe obstructive sleep apnea syndrome developed. Computed tomography scanning revealed an obstruction of the nasopharynx resulting from extramedullary hematopoiesis. The child was treated with hydroxyurea and blood transfusions. Relief of symptoms was noted 1.5 months after initial treatment. Extramedullary hematopoiesis causes sleep apnea syndrome in thalassemic patients, and the treatment of hydroxyurea and blood transfusio

[The surgical risk in sleep apnea: the implications for tonsillectomies].

Hypertrophy of tonsils or adenoids is the commonest cause of obstructive sleep apnea (OSA) in children. Adenotonsillectomy (AT) is frequently curative in children with OSA but riskier than the same procedure without OSA. It is crucial to identify OSA among the patients programmed for AT because they require a detailed evaluation, frequently including total or limited polysomnogram. Patients with OSA need a continuous surveillance before, during, and after surgery, ideally in a referral hospital.

A case of non-Hodgkin's lymphoma with macroglobulinemia.

A case of non-Hodgkin's lymphoma with macroglobulinemia is reported. A 48-year-old man consulted our hospital with complaints of dysphagia and sleep apnea. On the first examination, degree III hypertrophy of tonsils, Bence-Jones protein of K-type on urinalysis, and an M-peak in the gamma-glb. Beta-glb areas on fractionation of protein were found. Of serum immunoglobulins, IgM was increased markedly with 3,946 mg/dl and about 30% atypical lymphocytes were noted in the bone marrow. Bilateral tonsillectomy was carried out. The diagnosis of malignant lymphoma, non-Hodgkin, follicular, medium-sized
