Clinical phenotype profile and risk factors for sinusitis and obstructive sleep apnea in children with adenoid hypertrophy.

BACKGROUND: Adenoid hypertrophy (AH) is a common childhood condition frequently associated with multiple comorbidities. However, its specific clinical phenotype and the independent risk factors for key complications, particularly in children requiring surgery, remain inadequately elucidated. METHODS: This retrospective cohort study included 161 children undergoing surgery for AH. Demographic data, disease severity indicators, comorbidities, and laboratory results were collected. Descriptive statistics, chi-square tests, and multivariate logistic regression models were used for data analysis.

Olfactory training with multisensory stimulation: neural insights into cross-modal integration.

BACKGROUND: Olfactory training (OT) has emerged as a first-line non-pharmacological treatment for olfactory dysfunction. This study aimed to determine whether multisensory stimulation during OT improves olfactory recovery and to explore the underlying neurobiological mechanisms. METHODOLOGY: Individuals aged 18-80 years with post-viral or idiopathic olfactory dysfunction lasting for at least one month were recruited. Participants were allocated to either multisensory OT (MOT, odor-congruent video clips during OT) or conventional OT (COT) for three months. A subset of participants underwent re

Local allergy in the atopic march: new insights into adenotonsillar hypertrophy.

Adenoid hypertrophy (AH) and combined adenotonsillar hypertrophy (ATH) are primary causes of childhood sleep-disordered breathing (SDB), and they are strongly correlated with atopic diseases affecting the respiratory mucosa. Allergen sensitization, class-switching of B cells, and IgE production in the adenotonsillar tissue, namely local atopy, are crucial steps in the pathogenesis of allergic rhinitis (AR) and asthma. The adenotonsillar tissue is also responsible for a considerable part in circulating specific IgE, potentially contributing to the pathogenesis of atopy in other organs. Atopic c

Development of an explainable prediction model for the risk of moderate-to-severe obstructive sleep apnea in children.

UNLABELLED: Early identification of children at high risk for moderate-to-severe obstructive sleep apnea (OSA) is crucial for timely intervention, yet is often hindered by limited access to polysomnography (PSG). We aimed to develop an interpretable clinical prediction model using easily obtainable clinical and inflammatory biomarkers to distinguish moderate-to-severe from mild pediatric OSA. We conducted a retrospective study of 164 children diagnosed with OSA by PSG. From multiple biomarkers and clinical variables, least absolute shrinkage and selection operator (LASSO) regression was employ

Rhinovirus Infects B and CD4 T Lymphocytes in Hypertrophic Tonsils in Children.

Prolonged detection of rhinovirus (RV) in secretions after a typical cold and asymptomatic shedding are frequently reported. Although RV has been detected in human hypertrophic tonsils, its replicative status and host cell range remain unclear. In this study, we analyzed RV replication, infected cell types, and recovery of infectious virus in adenoids, palatine tonsils, and respiratory secretions from 293 children with tonsillar hypertrophy undergoing tonsillectomy. Samples were screened by real-time RT-PCR, and RV-positive samples were analyzed using immunohistochemistry (IHC), chromogenic in

Independent association of serum IgA levels with moderate-to-severe obstructive sleep apnea in children with adenotonsillar hypertrophy.

BACKGROUND: Adenotonsillar hypertrophy (ATH) is the leading cause of obstructive sleep apnea (OSA) in children. While the role of allergic inflammation (IgE-mediated) has been widely studied, the contribution of mucosal immunity-specifically Immunoglobulin A (IgA)-to disease severity remains unclear. This study aimed to investigate the association between serum immune markers and OSA severity and to construct a multivariate diagnostic model for moderate-to-severe disease. METHODS: A total of 110 children diagnosed with ATH were enrolled in this cross-sectional study. Based on polysomnography

Association between adenoid-to-tonsil size ratio and craniofacial characteristics in children with adenotonsillar hypertrophy: a cross-sectional study.

OBJECTIVES: Adenotonsillar hypertrophy (ATH) is a major cause of upper airway obstruction and is associated with specific craniofacial characteristics. However, the craniofacial morphology of children with ATH is controversial. This study aimed to clarify these discrepancies by investigating the association between the adenoid-to-tonsil (A/T) size ratio and craniofacial characteristics in children with ATH, and examining whether age modifies these associations. METHODS: Children aged 6-10 years were recruited and classified into ATH or no hypertrophy (NH) groups based on lateral cephalograms.

Associations between Immune Cell Traits and Tonsil and Adenoid Hypertrophy in Children: a Bidirectional Two-Sample Mendelian Randomization Study.

BACKGROUND: Mounting evidence indicates associations between immune cells and chronic diseases of the tonsils and adenoids (CDTA). Whether these observational links reflect underlying causality has yet to be determined. To elucidate causal links between immune cell traits and CDTA we used two-sample Mendelian randomization (MR). METHODS: Summary data of 731 immune cell traits were acquired from the OPEN GWAS repository, encompassing absolute cell counts (n = 118), morphological parameters (MP; n = 32), relative cell counts (n = 192), and median fluorescence intensity of surface antigens (n =
