Otolaryngology--head and neck surgery : official journal of American Academy of Otolaryngology-Head and Neck Surgery2026eng
OBJECTIVE: Peritonsillar abscess (PTA) after surgical tonsil removal is an infrequent complication. Residual lymphoid tissue after tonsillotomy may increase this risk compared to tonsillectomy. This study aimed to identify cases of PTA following tonsillotomy or tonsillectomy in a pediatric population and compare the incidence and case details.
STUDY DESIGN: Retrospective comparative case series.
SETTING: Tertiary pediatric hospital.
METHODS: A review was conducted of patients ≤18 years of age who underwent tonsillotomy or tonsillectomy from January 2010 to January 2025 and later presented w

INTRODUCTION: Peritonsillar abscess (PTA) is usually caused by common oropharyngeal microorganisms. Gardnerella vaginalis, which is primarily associated with bacterial vaginosis, is rarely implicated in oropharyngeal or deep neck infections.
CASE PRESENTATION: An otherwise healthy 18-year-old man developed a left PTA that resolved after incision and drainage, 5 days of intravenous ampicillin/sulbactam, and subsequent oral antimicrobial therapy. Initial abscess culture yielded only normal oral flora. Eight days after discharge, he engaged in oral sex with a female partner. Four days later, PTA

Anastomotic leaks (AL) after esophagectomy contribute to significant morbidity (up to 30%) and mortality (5-10%). Indocyanine green (ICG) fluorescence angiography offers real-time perfusion assessment, potentially reducing AL compared to conventional methods. This meta-analysis evaluates ICG's efficacy in reducing AL after esophagectomy with gastric pull-up, focusing on cervical versus intrathoracic anastomoses. This systematic review and meta-analysis followed PRISMA guidelines. A comprehensive search of PubMed and Google Scholar from 2005 to the present was performed using MeSH terms to iden

BACKGROUND/AIM: Metabolic comorbidities, such as diabetes mellitus (DM) and hypertension (HTN), can influence the severity and progression of oropharyngeal infections. This study investigated the impact of these conditions on inflammatory and metabolic markers in patients with severe infections.
PATIENTS AND METHODS: This retrospective study included patients diagnosed with peritonsillar phlegmon, laterocervical/submandibular abscess, and peritonsillar abscess. Metabolic [glycemia, body mass index (BMI)] and inflammatory [erythrocyte sedimentation rate (ESR), C-reactive protein (CRP)] paramet

INTRODUCTION: Peritonsillar abscess is the most frequent complication of acute tonsillitis. Although adult epidemiology and microbiology are well characterized, age-specific pediatric data remain limited. We investigated the incidence, microbiology, and suitability of penicillin monotherapy for pediatric peritonsillar abscess.
METHODS: We conducted a retrospective cohort study of patients aged 0-17 years admitted with peritonsillar abscess to Aarhus University Hospital, Denmark (2012-2024). Age-stratified incidence rates were calculated using national population data. Bacterial findings and t

Peritonsillar abscess (PTA) is a common deep neck infection in which bedside drainage is performed close to major cervical vessels. Although catastrophic vascular complications are rare, clinicians have limited quantitative information on how far the internal carotid artery (ICA), external carotid artery (ECA), and internal jugular vein (IJV) lie from the abscess in individual patients. This protocol describes a contrast-enhanced computed tomography (CT)-based workflow to confirm unilateral PTA, acquire neck CT images under standard clinical settings, measure linear distances from the anterior

Postoperative bleeding is a well-known complication of tonsillectomy. Although inadequate hemostasis and vascular injury are common causes, occult coagulation disorders may also contribute. Factor XIII deficiency is an extremely rare condition that is particularly difficult to detect preoperatively because PT and APTT typically remain normal. A 27-year-old man underwent bilateral tonsillectomy for a recurrent peritonsillar abscess. Despite normal coagulation screening, the patient developed repeated postoperative hemorrhages requiring surgical management. Further evaluation revealed reduced Fa

Tonsil stones or tonsilloliths are mineralised concretions in the crypts of palatine tonsils. They are usually small and asymptomatic. Sometimes, they can be recurrent, large in size and produce symptoms like a sense of foreign body in throat, mild pain and heaviness in throat and halitosis. In such cases, a surgical intervention may be required for permanent cure. We came across one such patient with massive tonsillolith which mimicked a peritonsillar abscess initially. Eventually, the large tonsillolith was surgically removed in toto. It measured 5.2 × 2.5 × 2.5 cm which makes it the largest

OBJECTIVE: Research suggests that intracapsular tonsillectomy (ICT) is less morbid than extracapsular tonsillectomy. It is underutilized, in part, because of concerns regarding its effectiveness and the possible need for repeat surgery in the future. We conducted long-term follow-up patient interviews to evaluate symptom control and incidence of revision tonsillectomy in children who had undergone ICT years earlier.
METHODS: A computerized collection of office notes and operative reports was queried to identify children undergoing ICT in the years 2010-2023 at an academic pediatric otolaryngo

Croup is a common childhood respiratory illness with peak incidence in October and November. Inflammation of the subglottic structures results in obstruction of the upper airway, causing the characteristic barking cough, hoarseness, and inspiratory stridor. Fever may also be present. Patients with suspected croup should be assessed for more severe causes of upper airway obstruction, such as bacterial tracheitis, retropharyngeal abscess, peritonsillar abscess, foreign body obstruction, and epiglottitis. Radiography or laboratory testing is typically unnecessary for diagnosis but should be consi

PURPOSE: The purpose of this study was to compare cold and hot haemostasis techniques during tonsillectomy in terms of the posttonsillectomy haemorrhage (PTH) risk and patient-reported postoperative pain. The primary objective was to compare the incidence of PTH from the cold- versus hot-haemostasis-treated tonsillar bed within 30 days. The secondary objective was to compare pain perception between the two sides. METHODS: This was a randomized, interventional, single-centre, self-controlled clinical trial that included adolescents (aged ≥ 12 years) who underwent tonsillectomy at Nordsjællands

Background: Peritonsillar abscess (PTA) is a common deep neck infection associated with significant healthcare resource utilization. Needle aspiration (NA) and/or incision & drainage (ID) are the most frequently used treatment modalities; however, their comparative clinical outcomes remain controversial. Methods: This retrospective cohort study included 185 adult patients hospitalized with a diagnosis of PTA between January 2018 and January 2026 at a tertiary care center. Patients aged ≥18 years who underwent NA or ID were included. Demographic characteristics, clinical findings, laboratory pa

PURPOSE: To evaluate the feasibility of ED map derived from unenhanced DECT for detecting peritonsillar abscess.
METHODS: This retrospective study included patients with clinically suspected peritonsillar abscess who underwent DECT. Two independent radiologists evaluated the presence of peritonsillar abscess on unenhanced conventional 120-kVp images (CTconv), 70- and 40-keV virtual monochromatic images (VMIs), ED map, and contrast-enhanced CT (CECT), using a 4-point diagnostic confidence, scored as follows: 4, highly likely; 3, likely; 2, unlikely; and 1, highly unlikely. Scores of 3 and 4 we

INTRODUCTION: Extranodal natural killer/T-cell lymphoma, nasal type (ENKTL), is a rare EBV-associated malignancy characterized by destructive tumors in the nasal cavity, often causing delayed diagnosis due to tissue necrosis. Artificial intelligence (AI) has potential to assist in complex diagnostic challenges.
OBJECTIVES: This study aimed to evaluate the diagnostic performance and clinical relevance of publicly accessible AI-driven platforms in identifying ENKTL based on a real, challenging clinical case.
MATERIALS AND METHODS: Clinical data from a 26-year-old male patient with protracted a

Spontaneous tonsillar hemorrhage (STH) is a rare but potentially life-threatening complication most commonly associated with tonsillitis and, less frequently, peritonsillar abscess. Prompt recognition and management are critical due to the risk of airway compromise and vascular injury. We report the case of a 30-year-old female who developed massive oropharyngeal hemorrhage following spontaneous drainage of a peritonsillar abscess, requiring emergent airway protection and surgical intervention. The patient underwent Quinsy tonsillectomy, which revealed an extensive defect created by the hemato

The persistence of resistant pathogens in otorhinolaryngology is associated with substantial disease burden and limited effectiveness of the available antimicrobials. We aimed to provide an updated overview of nationwide antimicrobial susceptibility surveillance for major pathogens causing otorhinolaryngological infections in Japan. The 12th joint four-society survey was conducted between 2021 and 2023 in 37 institutions nationwide. Bacterial isolates from patients with acute otitis media, acute exacerbation of chronic otitis media, acute tonsillitis, and peritonsillar abscess were analyzed. A

The term standard of care is a legal concept in health care representing the duty a practitioner owes to each patient, and this duty is the same for telehealth encounters, whether video based or audio only, as it is for in-person encounters. This article presents an anonymized case analysis of a fatal peritonsillar abscess misdiagnosed during an audio-only telehealth encounter, highlighting the telehealth regulatory variations across states and emphasizing professional liability considerations for nurse practitioners (NPs) delivering remote care. Through the PEP and AUDIO telehealth frameworks

Shewanella species are motile, oxidase-positive, non-fermenting Gram-negative bacilli widely distributed in marine and aquatic environments. Although primarily environmental organisms, Shewanella spp. are rare human pathogens, most often associated with skin and soft tissue infections. Involvement of the head and neck region is exceptional. We report the case of a 28-year-old immunocompetent male with no history of seawater exposure who presented with a one-week history of odynophagia, fever, trismus, and dysphagia. Clinical examination revealed a unilateral peritonsillar abscess. Needle aspir
