Unilateral versus bilateral abscess tonsillectomy. Implications for current surgical practice.

OBJECTIVES: To compare unilateral vs bilateral hot tonsillectomy in managing peri-tonsillar abscesses. METHODS: Literature review in Medline and other database sources, and critical analysis of data. RESULTS: Management options for peri-tonsillar abscesses range from intravenous antibiotics alone or needle aspiration for small collections to incision and drainage or abscess tonsillectomy for larger ones. Regarding the latter, there is no clear consensus in the literature on favouring unilateral over bilateral excision. CONCLUSION: Conventional wisdom would suggest that operating on acutely

Peritonsillar abscess as a clinical sign of oropharyngeal cancer - a Swedish national cohort study.

PURPOSE: Patients older than 40 years with peritonsillar abscess may routinely undergo follow-up to detect oropharyngeal cancer, but this approach lacks scientific evidence. This study aimed to assess the hypothesis that oropharyngeal cancer is not overrepresented in patients with peritonsillar abscess. METHODS: This population-based, nationwide Swedish cohort study compared all patients diagnosed with peritonsillar abscess during 2005-2023 with an age-, date-, and sex-matched non-exposed group from the same cohort. Conditional logistic regression provided odds ratios (OR) with 95% confidence

Peritonsillar abscess, deep neck infection, and proton pump inhibitor: A nested case-control study.

OBJECTIVE: To investigate the association between proton pump inhibitor (PPI) use and the risk of peritonsillar abscess (PTA) and deep neck infection (DNI). METHODS: This nested case-control study utilized data from the Health Screening Cohort of the Korean National Health Insurance Service (2002-2013). A total of 1953 patients with PTA and 5797 patients with DNI were identified and matched with controls at a 1:4 ratio based on age, sex, income, and residential area. PPI exposure was classified according to prescription timing and cumulative duration. Adjusted odds ratios (ORs) and 95% confid

Spontaneous Rupture of a Peritonsillar Abscess Mimicking Successful Incision and Drainage: A Case Report.

Peritonsillar abscess (PTA) is a common deep infection of the head and neck that can present with significant clinical symptoms. Patients typically present with fever, malaise, sore throat, otalgia, dysphagia, and the characteristic muffled "hot-potato" voice. Although PTA is generally managed effectively with antibiotics, analgesics, and abscess drainage, spontaneous rupture is uncommon and may falsely suggest clinical resolution, posing a diagnostic and management challenge. We present the case of a 25-year-old woman who presented with a five-day history of sore throat, fever, odynophagia, d

Transoral Ultrasound in the Diagnostic Workup of Peritonsillar Abscess-A Randomized Clinical Trial.

BACKGROUND: Peritonsillar abscess (PTA) is a common deep head and neck infection in younger adults. Because the condition can be difficult to distinguish from peritonsillar cellulitis on clinical examination, many patients undergo unnecessary diagnostic blind needle aspiration attempts. For this reason, transoral ultrasound has been suggested as a point-of-care imaging modality that may improve the diagnostic workup of PTA. This randomized clinical trial explores whether transoral ultrasound improves the management of patients with peritonsillar infection. METHODS: A multicenter randomized cl

Preoperative Assessment of the Oral Environment in Patients Undergoing Tonsillectomy.

OBJECTIVE: Although maintaining oral hygiene may help prevent tonsillitis, the association between the two remains unclear. Tonsillectomy is a commonly performed procedure in otorhinolaryngology and has been studied from various perspectives; however, no studies have evaluated its relationship with the oral environment. We evaluated the influence of the oral environment on the pathogenesis of tonsillitis by comparing the preoperative oral environment in patients undergoing tonsillectomy according to surgical indication in a retrospective study. METHODS: We included 123 patients (64 male patie

A hidden diagnosis in the tonsil: recognising and differentiating intratonsillar abscess.

OBJECTIVE: Peritonsillar abscess (PTA) and intratonsillar abscess (ITA) are distinct entities, but the latter is often overlooked due to its rarity and diagnostic difficulty. This study compares their clinical, laboratory, and microbiological profiles to identify distinguishing features. METHODS: This single-centre retrospective study included 367 patients diagnosed with PTA (n = 311) or ITA (n = 56) between 2019 and 2025. Demographics, symptoms, laboratory markers (e.g., white blood cells [WBC], neutrophils, lymphocytes, large unstained cells [LUC], C-reactive protein, sodium, neutrophil-to-

Anatomical risk factors for descending necrotizing mediastinitis in deep neck infections.

BACKGROUND: Descending necrotizing mediastinitis (DNM) is among the most severe complications of deep neck infections (DNI), and the anatomical pathways that predispose to mediastinal extension remain incompletely characterized in large cohorts. We aimed to identify the anatomical risk factors associated with DNM in patients with DNI. METHODS: We retrospectively reviewed 496 consecutive patients hospitalized with DNI at a tertiary referral center between January 2015 and December 2025. Demographic data, the anatomical site of infection, the radiological dissemination pattern, surgical managem

Incidence and Clinical Significance of Retropharyngeal Edema in Node-first Presentation of Kawasaki Disease.

BACKGROUND: Diagnosing node-first Kawasaki disease (NFKD), which initially presents with fever and cervical lymphadenopathy, is challenging, particularly during the early stages of the disease. Although case reports have described retropharyngeal edema (RPE) on contrast-enhanced neck computed tomography (CT) in NFKD, its prevalence and diagnostic value remain unclear. We sought to clarify the clinical characteristics of NFKD and evaluate the significance of RPE. METHODS: We retrospectively reviewed children diagnosed with Kawasaki disease (KD) at Nagoya Memorial Hospital between January 2023

Comparison Between Bipolar Diathermy and Coblation Assisted Tonsillectomy in Pediatric Population: A Randomized Experimental Study.

BACKGROUND AND AIM: Tonsillectomy is one of the most commonly performed surgical procedures on children. Indications for tonsillectomy include recurrent tonsillitis, obstructive sleep apnea, and peritonsillar abscess, among others. Bipolar diathermy and coblation surgeries are the most common used surgeries. Bipolar Diathermy efficiently manages postoperative hemorrhage but often results in increased postoperative pain due to higher thermal tissue destruction. In contrast, coblation is a procedure that offers a lower output of heat and results in lesser pain and shorter healing times. The pres

Lambl's excrescences in a 9-year-old Japanese boy: differentiation from infective endocarditis.

Lambl's excrescences (LEs) are filiform extensions arising on the closing surface of cardiac valves, typically occurring at coaptation lines of left-sided valves. LEs are more commonly reported in adults and pediatric cases remain rare, with no previously documented cases in Japan. A 9-year-old Japanese boy initially presented with a fever and sore throat but developed right-sided neck pain several days later. Blood tests showed a white blood cell count of 33,900/mL and C-reactive protein concentration of 9.95 mg/dL. Contrast-enhanced computed tomography showed a right peritonsillar abscess, a

Design and Validation of a Molded Silicone Task Trainer for Simulating Peritonsillar Abscess Drainage.

Peritonsillar abscess (PTA) is a common and essential otolaryngologic presentation, and bedside incision and drainage is a core procedural skill for otolaryngology residents. We report the design and preliminary validation of a low-cost, anatomically realistic simulation model for PTA drainage. The model integrates a molded silicone oropharynx with a drainable abscess cavity into a commercial dental task trainer. Thirty-eight otolaryngology trainees and faculty performed simulated drainage and completed a postsimulation survey evaluating realism and educational value. Face validity was support

Incidentally discovered asymptomatic extrasinusal mucocele of the sphenoid greater wing: A case report and review of differential diagnoses.

Extrasinusal mucoceles are exceptionally rare cystic lesions that arise outside the paranasal sinuses and may pose diagnostic challenges, particularly when incidentally detected in atypical locations. Their imaging appearance can overlap with other cystic skull-base and intracranial lesions. We report the case of a 45-year-old woman with an asymptomatic extrasinusal mucocele incidentally discovered on CT performed during follow-up of a peritonsillar abscess. Imaging revealed a well-defined expansile cystic lesion in the spheno-temporo-orbital region, causing cortical thinning without surroundi

[German S2k guideline on antibiotic therapy of ENT infections : The most important information in brief].

Antibiotics save lives, but their long-term side effects are often underestimated. To prevent side effects and maintain the effectiveness of antibiotics, uncomplicated infections should be treated as narrowly as possible, while life-threatening infections should be treated as broadly as necessary. Acute upper respiratory tract infections should only be treated with antibiotics if there is a clear bacterial cause and the infection is severe. Penicillin V is the first-line treatment for tonsillopharyngitis, amoxicillin for acute otitis media (AOM) and acute rhinosinusitis (ARS), and a group 1 ce
