A case of adenoid cystic carcinoma of the larynx: Diagnostic challenges, surgical management, and long-term surveillance.

Adenoid cystic carcinoma of the larynx is an uncommon malignant neoplasm, accounting for less than 1% of all laryngeal cancers. It arises from minor salivary glands, predominantly located in the subglottic region. ACC is characterized by slow but aggressive growth, with a notable propensity for perineural invasion and distant hematogenous dissemination, particularly to the lungs, which occurs in approximately 35%-50% of cases. Clinical presentation depends on tumor localization and may include dyspnea, dysphonia, dysphagia, and otalgia. Diagnosis is based on clinical and endoscopic examination

The impact of surgery type on the recovery of iatrogenic vocal fold injury.

PURPOSE: To study the impact of surgery type causing iatrogenic Vocal Fold Injury (VFI) on recovery and symptoms. METHODS: Retrospective case series of patients with iatrogenic VFI, divided by surgery type: (1) Thyroidectomy; (2) carotid endarterectomy (CEA), and (3) others. The main outcome was vocal fold recovery. Data collected included demographics, medical background, and clinical presentation. Univariate and multivariate logistic regressions were employed to identify recovery predictors. RESULTS: A total of 107 patients were included (69 thyroidectomies, 18 CEA, and 21 other surgeries).

Multi-timescale finite element modeling of vocal fold edema progression due to phonotrauma.

Edema of the vocal folds (VFs) is the result of an inflammatory response to injury (phonotrauma), or other deleterious stimuli, causing fluid build-up in the VF tissue. Although a mild level of edema has been postulated to be prophylactic, excessive edema has been identified as a pathway in the development of vocal hyperfunction. Herein we present a multi-time scale finite element model to examine the progression of VF edema in response to phonotrauma. Phonotraumatic damage was assessed by two measures: viscous dissipation and positive strain energy rate. Time-averaged phonotrauma over short t

Refractory hypocalcaemia-induced laryngeal spasm requiring emergency intubation in a malnourished young woman: overcoming diagnostic and therapeutic barriers in resource-limited settings.

BACKGROUND: Hypocalcaemia-induced laryngospasm is a rare but life-threatening cause of upper airway obstruction. Management is particularly challenging in resource-limited settings due to delayed laboratory results, limited monitoring, and unavailability of preferred medications. CASE PRESENTATION: A 20-year-old previously healthy woman presented with severe stridor, respiratory distress, and carpopedal spasm. Laboratory findings (obtained after initial treatment) revealed ionized calcium 0.9 mmol/L (reference range: 1.12–1.32 mmol/L), magnesium 1.6 mg/dL (reference range: 1.7–2.3 mg/dL), and

A Case Report of General Anesthesia for Abdominal Surgery in Severe Cavitary Tuberculosis: Strategies to Minimize Barotrauma.

A 37-year-old man with suspected tuberculosis and severe cavitary lung disease presented with an acute abdomen with pneumoperitoneum requiring emergent exploratory laparotomy. The patient presented with dysphagia, recurrent regurgitation, pancytopenia, treatment refractory elevated International Normalized Ratio (INR). His compromised respiratory status, likely to worsen under positive pressure ventilation, along with a high risk of aspiration of gastric content and coagulopathy during emergent abdominal surgery, required general anesthesia. In patients with extensive pulmonary tuberculosis an

Disseminated histoplasmosis involving the vocal cords and bilateral adrenal glands: a case report.

BACKGROUND: Histoplasmosis is a systemic fungal infection primarily involving the lungs. Dissemination to the vocal cords and bilateral adrenal glands is exceedingly rare. This case report illustrates the clinical and radiological manifestations of the disease at these atypical sites, providing crucial insights for clinical differential diagnosis. CASE PRESENTATION: We present a 50-year-old male patient admitted with recurrent fever and weight loss, with a history of diabetes mellitus and bat exposure. Imaging studies revealed a nodule in the left thyroid lobe (ACR TI-RADS 5), bilateral adren

False-Positive Prostate-Specific Membrane Antigen (PSMA) PET Uptake in the Vocal Cords Mimicking Metastatic Disease in Prostate Cancer: A Case Report Highlighting Diagnostic Pitfalls.

Prostate-specific membrane antigen positron emission tomography/computed tomography (PSMA PET/CT) is increasingly used in prostate cancer imaging because of its high sensitivity for detecting metastatic disease, even at low prostate-specific antigen (PSA) levels. However, false-positive uptake may occur in benign or inflammatory conditions, creating important diagnostic pitfalls. We report the case of a patient with oligometastatic low-volume prostate adenocarcinoma who presented with biochemical progression and a PSA level of 10.85 ng/mL. Histopathology confirmed a high-grade adenocarcinoma (

Immediate Effects of Pre-bronchoscopy Pulmonary Physiotherapy on Post-procedure Pulmonary Function in Individuals Undergoing Bronchoscopy.

Introduction Pulmonary function plays an essential role in procedural safety and respiratory stability. Flexible bronchoscopy is an important diagnostic and therapeutic procedure; however, procedure-related complications, including hypoxemia, oxygen desaturation, bronchospasm, and laryngeal spasm, may impair pulmonary function and post-procedure recovery. Breathing exercises may improve respiratory performance by enhancing breathing mechanics and strengthening respiratory muscle function. However, evidence regarding the role of pre-bronchoscopy pulmonary physiotherapy remains limited. Methods

Ventilation-assisted functional visualization: a stepwise workflow for planned management of a preoperatively identified tracheal diverticulum during thyroidectomy.

Tracheal diverticulum is frequently detected incidentally on cross-sectional imaging but may pose a significant intraoperative challenge when anticipated within or adjacent to the thyroid surgical field, particularly in close proximity to the recurrent laryngeal nerve (RLN) and during central compartment (level VI) dissection. Conventionally, surgeons use preoperative computed tomography (CT) to approximate diverticular anatomy and then rely on intraoperative experience and anatomic landmarks for dissection. Recognition is challenging because intraoperative localization relies on CT correlatio

Gene Expression Differences Between Vocal Fold Polyps and Reinke's Edema.

BACKGROUND: Vocal fold polyps and Reinke's edema are among the benign vocal cord lesions. Although they have different pathogeneses, their histological features are similar, making their diagnosis challenging. This study aims to investigate gene expression differences between Reinke's edema and vocal fold polyps to enhance diagnosis and provide insights into their pathophysiological mechanisms. METHODS: This cross-sectional study analyzed intraoperative tissue samples from the true vocal folds of patients diagnosed with either Reinke's edema or vocal fold polyps. Using RT-qPCR, we compared th

A Wolf in Sheep's Clothing: Laryngeal Myxoma Masquerading as a Vocal Cord Polyp.

Myxoma is a rare benign tumor of mesenchymal origin and is very rarely seen in the larynx. Because of its appearance, it is often mistaken for a vocal cord polyp. We report the case of a 19-year-old female who presented with hoarseness of voice for 1 month. Laryngoscopic examination showed a small polypoidal lesion on the vocal cord. The lesion was surgically removed. Histopathological examination showed sparse spindle-shaped cells in abundant myxoid stroma without atypia or mitotic activity, confirming the diagnosis of myxoma. Complete excision was curative. This case highlights the importanc
