Vocal fold sulcus.

Vocal fold sulcus is a cause of dysphonia which has not been recognized until recently. Awareness of its existence combined with use of laryngostroboscopy would enhance the management of this group of patients. Five such cases were treated initially by voice therapy and subsequently combined with microlaryngeal Teflon injections of the vocal cord. Representative photomicrographs and the end results of treatment are presented. A good voice, subjectively and objectively, was obtained in three patients, with satisfactory improvement in the other two.

Functional airway obstruction presenting as stridor: a case report and literature review.

We report the case of a young man who presented to 3 emergency departments with apparent upper airway obstruction and was intubated each time before being diagnosed with paradoxical vocal cord motion. His previous discharge diagnoses were laryngeal edema secondary to anaphylaxis, even though he had no other objective findings of IgE-mediated disease. Flexible fiberoptic laryngoscopy demonstrated tight apposition of the vocal cords during inspiration while symptomatic, but normal movement when asymptomatic. Psychiatric evaluation revealed severe posttraumatic stress disorder. Of the approximate

Familial vocal cord dysfunction associated with digital anomalies.

Familial vocal cord dysfunction is a rare condition that has been reported in only a few instances. This is a report of identical male twins, both of whom had congenital bilateral abductor vocal cord paresis associated with finger deformities. The vocal cord paresis progressed to paralysis that required tracheotomy, then returned to a slowly resolving paresis during which the vocal cords had uncoordinated motion generally known as synkinesis. Another male sibling and the mother had a history of stridor during infancy and finger deformities. Several other relatives had digital abnormalities, an

T2 glottic cancer. Recurrence, salvage, and survival after definitive radiotherapy.

The records of all patients with T2, NO squamous cell carcinoma of the true vocal cords treated with definitive radiotherapy at The University of Texas M. D. Anderson Cancer Center between 1970 and 1985 were analyzed to investigate treatment outcome, prognostic factors associated with tumor recurrence, and the potential impact of improved initial treatment on patient survival. There were 114 patients (male to female ratio, 13:1) with a median age of 62 years at presentation. All were treated with external beam irradiation to a modal dose of 70 Gy in 35 fractions over 7 weeks. The median field

[Comparative research on vocal polyps and nodules].

One hundred and eighty-four cases of vocal polyps and 64 cases of vocal nodules are reported. Of these cases, 6 polyps and 10 nodules had been studied with electron microscope: 40 cases of polyps and 15 cases of nodules were investigated histo-chemically with PAS, AB-PAS, HID-AB, Congo Red and other methods. It seems that polyps are distinctly different from nodules. They are different diseases. The main difference between them is that polyp of the vocal cord is caused primarily by circulation impediment, thrombosis, exudation and edema of the lamina propria of the mucous membrane due to infla

Laryngostroboscopy and glottic dysrhythmia's.

Computer simulation of stroboscopic pictures of vocal folds with normal, regular, oscillations on the one hand, and of well-defined situations of glottic dysrhythmia's on the other hand, indicates clearly the limitations of the method, due to the irregularities of both frequency and amplitude of vocal fold oscillations. Yet, interesting propaedeutical signs suggest some kinds or ranges of glottic dysrhythmia, and allow a rough estimation of jitter and shimmer, both to be considered as fundamental physiopathological parameters.

[Neoplastic development of laryngeal amyloidosis. Apropos of a case].

The authors report the case of a 62 years old patient who presented a squamous laryngeal carcinoma 8 years after the discovery of amyloidosis localized to one vocal cord. In this patient, a relationship would seem to exist between the two pathologies, with degeneration of amyloidosis being responsible, though a random coexistence cannot, of course, be completely excluded. After a review of laryngeal amyloidosis and its management, this particular case is discussed in relation to other cases described in the literature.

The role of computed tomography and coronal plane tomography in radiotherapy for laryngeal cancers.

The results of a study undertaken prospectively in 1983 and 1984 to assess the value of computed tomography (CT) and conventional tomography in the planning of radiotherapy for laryngeal cancer are presented. Of 32 cases treated in 1983, 23 had both CT scans and coronal plane tomography prior to radiotherapy. At the end of 1983 these radiographs were reviewed and an assessment made of their value in treatment planning. As a result of this review it was decided that cancers of the larynx confined to the vocal cords should routinely have conventional coronal plane tomograms rather than CT scans.

Vibratory patterns of normal and microscopically operated larynges. Observation of lower surface of vocal fold in dogs.

The authors investigated the effect of microsurgical intervention involving the canine larynx on vocal fold vibration. In normal cases both lower lips and free edges vibrated symmetrically. The lower lip coincided with the free edge and collided with that of the other side at the midline. Immediately after the collision, the lower lip rapidly retracted laterally at the beginning of a closed phase. It stayed in the most lateral position for the rest of the closed phase until the free edge started to move away laterally. The lower lip then began to move medially to occlude again with the other s

[Significance and prognosis of positive surgical section margins in surgery of laryngeal cancer].

During the surgical treatment of head and neck cancers the surgeon may have to face the problem of deciding what measures to adopt when the tissue at the margin of the surgical sections proves histologically to be cancerous. Many factors may influence the decision for supplementary therapy; among these, of primary importance are stage and site of the tumor, the effectiveness of non surgical therapy, and the psychophysical status of the patient. In order to better consider this problem a retrospective study, limited to laryngeal surgery, and a review of the literature have been carried out. Rev

[Pathogenesis and clinical aspects of Reinke edema. Long-term studies].

Since 1973, 307 patients have been treated for Reinke's oedema in the ENT Department of the University of Marburg. A retrospective study from 1983 and observations up to 1989 investigated the aetiology and the prognosis of the disease. Two aetiological factors were found: smoking and voice abuse. Surgical treatment, stripping of the vocal cords as described by Kleinsasser was successful. There were no recurrences and no malignant degenerations. The prognosis was considered to be good. Two other long-term studies of Reinke's oedema are compared with our findings. Controversial opinions about th

Acoustic analysis of pathological voice. Some results of clinical application.

Tape-recorded voices of 30 patients were acoustically analysed: 10 had glottic Tla carcinoma, 10 unilateral vocal fold polyp and 10 unilateral recurrent laryngeal nerve paralysis. The carcinoma cases were treated with laser surgery with/without radiotherapy, the polyp cases with endolaryngeal microsurgery and the paralysis cases with intrafold silicone injection. The acoustic analysis was conducted before and after the treatment for each patient. Three acoustic parameters, viz. pitch perturbation quotient (PPQ), amplitude perturbation quotient (APQ) and normalized noise energy (NNE), were empl

Visualization techniques in the nasal airway: their role in the diagnosis of upper airway disease and measurement of therapeutic response.

Flexible fiberoptic rhinopharyngolaryngoscopy permits the examination of most areas of the upper airway that heretofore have not been easily visualized with other available techniques. Use of the flexible fiberscope is safe, convenient, quick, and cost efficient. Abnormalities can be easily seen relating to the septum, turbinates, and mucosa, as well as the nasopharynx, adenoids, eustachian orifice, tonsils, posterior tongue, epiglottis, glottis, and vocal cords. The origin of nasal polyps can also be identified. Furthermore, effects of treatment can be followed with the fiberscope as well. Fo

[Impression of the thyroid cartilage lamina--differential diagnosis in hyperplasia of the ventricular fold].

Impression of the thyroid lamina is presented by means of CT examinations of the larynx and in correlation with the clinical features and with serial sections of the larynx in the horizontal plane. This anomaly of the thyroid lamina produces the laryngoscopic aspect of a hyperplastic false vocal cord. Measurements proved that the horizontal extension of the convex thyroid lamina is always longer than the lamina of the other side. The pathogenetic mechanism in the development of this anomaly lies--in our opinion--in the irregular increase in size that probably begins at the onset of puberty. Th

Percutaneous transtracheal ventilation for laser endoscopic treatment of laryngeal and subglottic lesions.

Obstructive lesions of the larynx and subglottic space are always a challenging problem for the endoscopist and anaesthetist. At this level, the efficacy and innocuity of a carbon dioxide laser treatment are related to the degree of endoscopic exposure. Thanks to the transtracheal high frequency jet ventilator, it is now possible to assure a free laryngeal endoscopic operative field. The transtracheal catheter is introduced percutaneously through the cricothyroid membrane into the trachea under endoscopic control and connected to a high frequency jet ventilator. From November 1983 to April 198

Vocal cord dysfunction: the importance of psychologic factors and provocation challenge testing.

We present three case reports involving patients with vocal cord dysfunction. The onset of symptoms in one case was coincident with a generalized cutaneous reaction to penicillin with laryngeal involvement. The other cases had been misdiagnosed as food allergy and chemical sensitivity. We describe the psychologic factors in these cases in terms of the primary and secondary gain operative in the somatoform disorder of conversion reaction and emphasize the importance of belief and learned sensitivity in the induction of symptoms. The necessity of considering psychologic factors and the use of bl

Acute airway distress due to thyroid pathology.

Patients with multinodular goiter or related thyroid disorders rarely have acute airway distress due to tracheal deviation or compression. However, our institution cares for a large number of patients with untreated multinodular goiters, and in the progression of this disorder, tracheal deviation and airway problems are relatively common. During the past 4 years, we have cared for 24 patients who were admitted with acute, life-threatening airway distress that required emergency intervention. Nine patients had emergency intubation, the remaining 15 had stridor on admission and underwent emergen

Complications in the management of large glomus jugulare tumors.

This study analyzes the complications encountered in the surgical treatment of 17 patients with large glomus jugulare tumors. All 17 patients sustained either new cranial nerve palsies or exacerbation of preexisting palsies. These involved, in descending order of frequency, the facial nerve, the vagus and glossopharyngeal nerves, and the hypoglossal nerve. Postoperative palsies of the facial nerve involved 17 patients, as compared to 5 preoperatively. Fifteen patients had postoperative partial or complete paralysis of the vagus nerve as compared to eight preoperatively. Ten patients had postop

[Critical evaluation of jet ventilation from the viewpoint of the endoscopist].

Endoscopists' clinical experiences using jet ventilation (N2O/O2 mixture) for microlaryngoscopy, endolaryngeal microsurgery and tracheobronchoscopy of adults, children and (small) infants are advantageous. Especially authors accentuate: Improved clearness of laryngeal area; overcoming intubation obstacles; extraordinary variability of sounds' arrangement. Authors observed no complications caused by this method. A "symptom of absent sharpness" (microscope) refers to dignity of epithelial alterations at vocal cords. The hygienic molestation of endoscopist by gas backflow is disadvantageous; on t

Upper airway dysfunction detected by respiratory flow oscillations.

In 3 patients with chronic respiratory symptoms, we identified rapid flow oscillations on flow-time and flow-volume recordings, corresponding to fluttering of lax but otherwise normal-appearing upper airway structures at cinefluoroscopy and fiberoptic endoscopy. This entity of upper airway dysfunction was responsible for, or at least contributed to, the respiratory symptoms as evidenced by dramatic relief of dyspnea following tracheostomy in one patient, and significant improvement of flow rates following helium breathing in the other 2 patients. We suggest that identification of flow oscillat
