Unusual manifestations of non-Hodgkin's lymphoma.

Two patients with biopsy proven lymphomatous infiltration of uncommon sites are presented. One had histiocytic lymphoma, involving the true vocal cord with a squamous cell carcinoma on the opposite vocal cord. The other had diffuse well-differentiated lymphocytic lymphoma, presenting with nodal and periorbital disease. After local radiation therapy and while clinically free of other lesions, he developed hemotochezia due to several polypoid lesions throughout the entire colon and prostatic infiltration with symptoms of lower urinary tract obstruction. Both problems were solved after specific t

The treatment of subglottic stenosis in children by prolonged dilatation.

Laryngeal stenosis can present with recurrent lower respiratory tract infections, hoarseness, stridor or decreased exercise tolerance. Asphyxia and death may follow. One accepted method of treatment is to perform a tracheostomy, dilate the stricture and insert an obturator for 6 to 9 mo. Ten children with a subglottic stenosis were treated by this technique and 7 completed the treatment. Results in 5 of these children were good, 1 was improved and 1 restenosed. These results compare favorably with those reported by others. This form of treatment is indicated for unyielding but dilatable strict

Voice spectrography in the evaluation of myasthenia gravis of the larynx.

Myasthenia gravis is a neuromuscular disorder that affects striated muscles especially those innervated by the cranial nerves. The standard diagnostic regimen is to find a reversal of symptoms by acetylcholine esterase administration. The permanent and objective recording of this effect is the key to an accurate assessment of this test. The voice spectrograph is a noninvasive means of evaluating the voice. It has been used successfully to make a clear and specific diagnosis of myasthenia gravis as it affects the larynx. The test can be used to distinguish myasthenia gravis from other functiona

Recurrent laryngeal nerve pathology in spasmodic dysphonia.

Since it was first described in 1871, spasmodic (spastic) dysphonia has been considered a disease of psychogenic origin. Unsupported theories of possible organic etiology have appeared sporadically in the literature. In 1976 sectioning of the recurrent laryngeal nerve for patients with this disease was reported with resultant improvement in voice production. This was attempted because the spasmodic dysphonic has, in effect, already compensated vocal cords bilaterally. It was reasoned, therefore, that if one of these was paralyzed the patient would immediately be converted to a state approximat

[Dysphonia following strumectomy. Laryngeal and vocal cord changes as sequelae of thyroid surgery].

In 142 patients function and morphology of the larynx were clinically assessed before and after strumectomy. The postoperative incidence of irreversible recurrent nerve pareses amounted to 2.1% whereas incidence of pareses of the n. laryngeus superior was 1.4%. The rather high number of laryngeal asymmetries (49.3%) is, in most cases, reversible after strumectomy. The number of intubation injuries does not significantly differ from the figures obtained after intubation anesthetics based on other indications. Hyperkinetic dysphonia is partly reversible after strumectomy, an unspecific recuperat

CO2 laser laryngoscopy in a variety of lesions.

We have had the opportunity to employ the CO2 surgical laser with suspension laryngoscopy and microscopic adaptations to treat a variety of laryngeal conditions over two years. Each type of laryngeal condition is discussed, and the benefits of laser management is evaluated. This type of laryngeal management is compared with other forms of treatment such as the standard surgical approaches, electrocautery, and cryosurgery.

Valve dynamics.

Flow between two contiguous elastic sheets (valve leaflets) generates one of two types of behavior. At low pressure differences, e.g. 1 cm H2O, the sheets part slightly and the fluid passes silently between them. The aperture under these silent conditions varies with the square of the length of the orifice, the transvalvular pressure difference and the kinetic energy, divided by the thickness of the leaflets. At higher transvalvular pressure differences the downstream end of the bicuspid valve alternately closes and reopens (flitter), and functions as an acoustic oscillator. The recurrence rat

Etiology of bilateral abductor vocal cord paralysis: a review of 389 cases.

The etiology of 389 cases of partial or complete bilateral abductor vocal cord paralysis has been determined and classified. One hundred and forty-nine were infants and children 12 years of age and under; 240 were adults, age 13 and older. In the infants and children the paralyses were congenital in 82 cases, of which 43 were associated with other congenital anomalies, and 39 were without associated anomalies. Fifty-nine cases were considered acquired, most being secondary to underlying congenital anomalies, particularly the associated findings of meningomyelocele, Arnold-Chiari malformation,

[Diagnosis and treatment of a case of anterior adhesion of the vocal cords].

A case of synechia laryngis of the vocal cords is presented. Etiology and treatment of this pathological condition are reviewed. For the present case the synechia seems to have been caused by an acute recurring laryngitis. A teflon oval sectionned tube is placed in the anterior commissure and anchored through the prelaryngeal skin, until epithelisation has occurred. This method is compared with other similar techniques.

Clinical course of patients with squamous cell carcinoma of the upper respiratory and digestive tracts with no evidence of disease 5 years after initial treatment.

Eleven hundred sixty-three patients with squamous cell carcinoma of the upper respiratory and digestive tracts, treated from January 1948 through December 1965, had no evidence of disease five years after the initial treatment and no other manifestation of squamous cell carcinoma within the first five years. After five years, the incidence of disease at the initial location, in the vicinity, or in remote sites was found to be identical in the surgical and irradiation groups. There is a correlation between severe late complications in the irradiated patients and the volume of tissue irradiated.

Ultrastructural features of human juvenile laryngeal papillomas.

The ultrastructural organization of 12 juvenile laryngeal papillomas was investigated. Four patients were more than 40 years old but had carried the disease since childhood. The other 8 patients were 2-10 years old and had carried the disease from 1-5 years. There is regularly observed a discontinuity of the basement membrane and the basal cells of ten exhibit abundant cytoplasmic processes. In the stratum spinosum there is often found glycogen-like inclusions as well as Odland bodies and keratohyalin granules. In the superficial layers there are nuclear remnants as in parakeratosis. Virus-lik

The evaluation of treatment of patients with extensive squamous cancer of the vocal cords.

Fifty-one patients with previously untreated squamous carcinoma originating on the true vocal cord, staged T4, were analyzed. Forty-eight of the 51 patients were selected for treatment. Total laryngectomy was the minimal surgical procedure, and the pathologist confirmed the presence of cancer outside the confines of the larynx in 47 of 48 patients. Seventy-three percent of the patients were thought clinically to have negative nodes, but examination by the pathologist showed a 20 percent false negative rate in this evaluation. Twenty-one percent of the patients had received an emergency tracheo

Leprosy of the larynx.

Five instances of lepromatous leprosy involving lesions of the larynx were encountered among a series of 280 laryngeal lesions. These are briefly described as involving the epiglottis in all cases, vocal cords in two, and extension into the pyriform fossa in one instance.
