Anaesthesia in children with viral respiratory tract infections.

The effects and consequences of anaesthesia in a child with a respiratory tract infection (RTI) are controversial. There is a high incidence of viral RTI in children presenting for surgery and anaesthesia. The social and economic impact of postponing the procedure is significant; for the child, family and institution. The clinical effects of the common cold are well known, affecting the respiratory tract from the nose down to the small airways and lung parenchyma. The systemic effects of the toxic viraemic phase are also well recognized but not so the potential risk of a viral myocarditis. The

The role of the fiberscope in the critically ill patient.

The role of the fiberscope in the management of difficult and failed intubations has been well established and the importance of learning this valuable skill has been emphasized. Nonetheless, the fiberscope is underutilized in anesthesia and critical care practices because of a high rate of intubation failure. The main cause of failure is lack of expertise in maneuvering the fiberscope. Other technical causes of failure include fogging or clouding of the fiberscope's lens, drifting off the midline, and inability to advance the endotracheal tube or withdraw the fiberscope after completing intub

[The laryngeal mask in ophthalmologic surgery. Criteria and limitations of use in adults (analysis of 57 cases)].

Clinical and physical data about laryngeal mask use in eye surgery were collected in 57 patients. We observed 4 failures which required endotracheal intubation, 3 leaks during operative field installation, 4 salvations but no laryngeal spasm and no cough. There were no significative differences between body weights in laryngeal mask groups no 3 and no 4. Insufflation pressures of patients without curarisation were more important. The duration of ventilation was not correlated with salivation, leaks or increase of insufflation pressure.

Bilateral vocal cord paralysis secondary to treatment of severe hypophosphatemia in a continuous ambulatory peritoneal dialysis patient.

Neuromuscular complications, including tetany and laryngeal spasm, are recognized complications of hypocalcemia and hypomagnesemia. We present a continuous ambulatory peritoneal dialysis patient with hypomagnesemia who developed hyperphosphatemia and profound hypocalcemia after oral phosphate replacement for severe hypophosphatemia. The combination of hypocalcemia and hypomagnesemia resulted in life-threatening bilateral vocal cord paralysis. Phosphate replacement should be determined and given cautiously, particularly in patients with renal failure and concomitant electrolyte disturbances.

[Propofol: ED50 and ED90 induction doses in pediatric anesthesia].

OBJECTIVE: To study ED50 and ED90 induction doses of propofol in children. PATIENTS AND METHODS: Seventy-two children from 1 to 110 months of age and premedicated with pentobarbital 4 mg/kg participated in the study. Seven different doses of propofol (1.5, 2, 2.5, 3, 3.5, 4 and 4.5 mg/kg) were injected into the dorsal hand or foot vein in 30 seconds. We measured the response to application of the mask, corneal reflex and response to trapezius muscle compression 30 and 60 s after administration. A log-probit statistical analysis was used and complications were noted. RESULTS: The doses that p

A very small dose of suxamethonium relieves laryngospasm.

Intravenous suxamethonium, in a dose as small as 0.1 mg.kg-1, has been found to be reliable in the treatment of laryngeal spasm. Three episodes of vocal cord spasm observed during direct laryngoscopy were relieved by this dose of suxamethonium.

Electronic control of laryngeal spasm. II. Selective blockage of glottic adduction by a closed-loop circuit in the canine.

This work is the continuation of an earlier pilot study in which specially designed electrodes were used to block their own induced action potentials to restrain spasticity. In the current series of experiments, strap muscle and thyroarytenoid contraction (glottic closure) elicited by supramaximal ansa hypoglossi (N = 6) and recurrent laryngeal nerve (N = 4) stimulation (30 to 50 Hz, 0.1 to 6 mA, 0.1 to 1 msec), respectively, allowed afferent information via intramuscular sonomicrometer crystals to trigger a circuit that, in turn, stimulated blocking electrodes passed around the nerves downstr

[Episodic laryngeal dyskinesia: a functional cause of stridor].

We present five cases of episodic laryngeal dyskinesia. This term describes an entity associating acute dyspnea with inspiratory and/or expiratory stridor, sometimes ending in acute respiratory distress. The origin of this trouble comes from paradoxal contraction of the vocal cords, which is reversible and recurrent. The clinical presentation presents as a differential diagnosis for serious disorders such as bronchial asthma, laryngeal oedema, or stenosing lesions of the upper airways. The definitive diagnosis is made by excluding organic pathology, by visualising laryngeal spasm and by the in

The arytenoidopexy operation has been performed in 15 cases of bilateral abductor paralysis of different causes. A tube on n. 6 for women and a tube of n. 6.5 for men has been used for the intratracheal general anesthesia without a tracheotomy. The microdissection of the arytenoid has been realised under the operation microscope. The fixation suture of the arytenoid has been passed with a special needle set prepared by the experimental studies on the laryngectomised pieces. The functional respiration result has been controlled by the pre and postoperative spirometry. The laryngeal spasm disapp

Postextubation laryngeal spasm in an unanesthetized patient with Parkinson's disease.

We present a patient with Parkinson's disease who experienced laryngeal spasm after tracheal extubation without having been anesthetized. This patient's trachea was intubated because of respiratory arrest. We postulate that her postextubation laryngospasm was related to Parkinson's disease.

A study of risk evaluation and quality control in anaesthesia.

Because the community expects increasingly high standards of medical performance, there is every reason for meticulous recording of the problems which can arise during one's work. The aim must be to quantify risks, identify problem areas and create an instrument to facilitate quality control and the analysis of critical events. In the course of one calendar year, problems occurring in all 14,735 patients who underwent anaesthesia in a regional hospital were recorded systematically, classified as to their degree of severity, and entered alongside relevant patient data into individual anaesthesi

[Registration of problems during anesthesia. Analysis of risk factors and quality control].

Society continues to increase its demands on the medical profession in regard to quality. We believe this situation should be met by a more systematic approach to risk evaluation and quality assessment of our work. We report an attempt to establish a routine system for recording complications during anesthesia. We included all 14,735 patients who were anesthetized during one year. Data on preoperative disease, type of anesthesia and operation, and problems encountered during anesthesia were recorded on the routine anesthesia chart. An arbitrary scale from 1-3 indicated the severity of the prob

Electronic control of laryngeal spasm. I. Blockage of orthodromically induced action potentials in intact canine recurrent laryngeal nerves.

Spastic dysphonia is a central nervous system phenomenon of unknown etiology characterized by uncoordinated voice tremor with erratic patterns of laryngeal contraction. Standard treatments have not been entirely satisfactory. The authors propose to apply a concept of selective nerve activity blockage, which leaves normal contractions undisturbed, as the basis for suppression of laryngeal spasticity. Single pulses of constant duration and increasing amplitude were injected into specially designed blocking electrodes placed around six recurrent laryngeal nerves (three dogs). Vocal cord adduction

Malleus head fixation: association with pseudohypoparathyroidism.

Pseudohypoparathyroidism (PHP) is a heterogenous syndrome resulting from bone and renal resistance to parathyroid hormone (PTH). Hypocalcemia and hyperphosphatemia occur despite elevated PTH levels. Head and neck manifestations of PHP include tetany, laryngeal spasm, and ectopic calcification in the eye, dentition, subcutaneous tissue and brain. The first case of malleus head fixation caused by ectopic calcification in a PHP patient is reported. Normalization of serum calcium and phosphate levels does not appear to retard or correct this abnormality. Ossiculoplasty or amplification is currentl

Laryngospasm-induced pulmonary edema.

We report the case of a 6-month-old child who developed acute pulmonary edema because of laryngeal spasm during orthopedic manipulations for congenital hip dysplasia. Laryngospasm was probably secondary to an unsuspected light level of anesthesia, maintained via face mask. No other predisposing factors, such as enlarged adenoid tonsils, laryngitis, epiglottitis, mechanical stimulation of the larynx or aspiration of foreign material were identified. Serious oxygen desaturation and bradycardia ensued, during inefficient attempts at positive pressure ventilation. After emergency intubation withou

[Pulmonary edema following airway obstruction].

This case report describes an adult patient who developed post extubation laryngeal spasm and later pulmonary edema following relief of the laryngeal spasm. The mechanism that causes pulmonary edema following upper airway obstruction is very probably due to a modified Müller maneuver (inspiration against a closed glottis), which will increase negative intrapleural pressure, and thereby cause increased pulmonary capillary permeability.

Spinhaler specific bronchoprovocation challenges: short interval administration of micronized freeze-dried allergen in children with bronchial asthma. III.

Two different methods of antigen bronchial challenge, with micronized freeze-dried extract of D. pteronyssinus administered as a powder by a spinhaler, were evaluated in 15 asthmatic children, who were skin test positive and RAST positive for house dust mite D. pteronyssinus. In one method, daily increasing dose method (DIDM), a single antigen challenge was performed every day successively. In the other method, cumulative dose method (CDM), the antigen doses were administered successively with 7-minute intervals. The tests were considered positive when there was a fall of FEV1 greater than or
