Division of the recurrent laryngeal nerve for idiopathic laryngeal spasm.

A rare case of idiopathic laryngeal spasm presented itself as sleep apnea in a middle-aged man. A tracheostomy followed by the division of the recurrent laryngeal nerve relieved all of the symptoms.

Evaluation of non-specific bronchial hyperreactivity in different respiratory diseases.

Bronchial hyperreactivity (BHR) has been observed in respiratory disorders other than asthma but asthmatics usually show BHR to differing degrees. The presence and degree of BHR in asthma was evaluated and compared with that in other respiratory disorders. High BHR was observed in asthma (84%, with no difference between atopics and non-atopics), recurrent or persistent cough (36%), laryngeal spasm (50%), Kartagener's syndrome (66%) and cystic fibrosis. BHR was low in patients with allergic rhinitis but no asthma and was absent in patients with unexplained dyspnoea (100%) and in healthy control

Cine CT demonstration of nonfixed upper airway obstruction.

Cine CT, with its very rapid imaging capabilities, was used to evaluate eight adult patients suspected of nonfixed upper airway obstruction. A method was developed for such imaging, which provides 10 images at each of 12 contiguous levels, extending from the soft palate through the extrathoracic trachea. These give a dynamic view of the airway during at least one full respiratory cycle and take about 6 sec to obtain. Results were compared with similar studies in 10 normal volunteers. Seven of the patients showed intermittent obstruction on cine CT, while the eighth was judged normal. All resul

The impossible intubation--what next?

A patient is presented in whom endotracheal intubation was impossible secondary to an obstructing substernal goiter and laryngeal spasm. Cardiopulmonary bypass was utilized to provide safe induction of anesthesia and intubation so that the substernal goiter could be removed. Other alternative methods in anesthetic management are discussed.

Acute pulmonary oedema. A complication of upper airway obstruction.

A case of postoperative laryngeal spasm is presented in which severe pulmonary oedema developed after the laryngeal spasm was relieved. The management is discussed and some suggestions for prevention of pulmonary oedema in similar circumstances made. The pathophysiology of pulmonary oedema after acute upper airway obstruction is described.

Laryngeal spasm mimicking bronchial asthma.

The list of asthma masqueraders grows, emphasizing the need for a careful analysis of the patient suffering from cough, wheezing and/or dyspnea. The present case report describes a 23-year-old patient with spastic vocal cord adduction, initially treated as bronchial asthma. Severe disruption in her arterial blood gases was present. The vocal cord adduction persisted in spite of valium anesthesia. The patient's symptoms and signs were relieved with a tracheostomy. However, the vocal cord spasm persisted.

Laryngismus fugax: transient laryngeal spasm secondary to brain stem ischemia.

Bilateral vocal cord immobility in adduction is an uncommon, yet potentially life threatening, disorder. Endolaryngeal and neuromuscular causes are well known and for the most part readily diagnosed by the otolaryngologist. Central neurologic etiologies are more unusual. A case of transient spastic laryngeal paralysis secondary to cardiac emboli and brain stem ischemia is presented. A mechanistic analysis suggests that this entity occurs in the setting of pseudobulbar paralysis, and that the ultimate ischemic event may be loss of glycine mediated inhibition of laryngeal motoneurons.

Rigid bronchoscopy under intravenous general anaesthesia with oxygen Venturi ventilation.

In a study of 100 patients undergoing rigid bronchoscopy under intravenous general anaesthesia with oxygen Venturi ventilation no major complications were observed. Minor complications included one adverse reaction to alphaxalone-alphadolone acetate (Althesin), one prolonged episode of laryngeal spasm after removal of the bronchoscope, and subsequent muscle pain attributed to suxamethonium in 36 patients. The last complication occurred significantly less frequently (p less than 0 . 025) in those patients who were pretreated with a small dose of a non-depolarising neuromuscular blocking agent.

A fatality during haloperidol treatment: mechanism of sudden death.

The authors report a case of unexpected sudden death in a woman receiving relatively high dose of haloperidol. They postulate that drug-induced laryngeal spasm leading to cardiac arrest via vagal reflexes may represent the mechanism of neuroleptic sudden death in some cases.

Acute superior laryngeal nerve palsy: analysis of 78 cases.

Acute superior laryngeal palsy is often clinically unrecognized and frequently overlooked. Yet, this motor paralysis occurs more frequently than facial paralysis. Like acute facial paralysis, superior laryngeal palsy often occurs as part of a cranial polyneuritis that is probably related to herpes simplex virus reactivation. Rotation of the larynx and shortening of one vocal cord have been found in such diverse syndromes as vestibular neuronitis, migraine and tension headaches, unexplained cough, tinnitus aurium, globus hystericus, and carotidynia. Electromyographic studies suggest that laryng

Development of laryngeal function: etiologic significance in the sudden infant death syndrome.

Recent clinical evidence implicates transient upper airway obstruction as a cause of potentially fatal cardiorespiratory disturbances. This investigation identifies age-related neurologic mechanisms which may be pertinent to the production of abnormal laryngeal closure as a possible cause of unexpected infant death. A period of transient laryngeal hyper-excitability is identified in pups 50--75 days post-natally. The mechanism of the hyper-excitable state resulting in increased risk of laryngeal spasm, appears related to: 1. the completion of central synaptic maturation: 2. transient reduction

Resuscitation at the site of a road accident.

Seven per cent of deaths resulting from motor car accidents are due to asphyxia without any other major injury. This highlights the importance of maintaining an unobstructed airway in patients following motor vehicle accidents. These cases of asphyxia are due to laryngeal spasm, aspirated foreign material, or pharyngeal obstruction by the tongue in a semi-conscious or unconscious patient. It is reasonable to assume that many of these lives could be saved by simple techniques, such as removing foreign material from the pharynx, pulling the chin and head back firmly, and turning the patient into

Laryngeal spasm: a neurophysiologic redefinition.

Within the limits of standard neurophysiologic techniques, we have attempted to redefine laryngeal spasm as distinct from the glottic closure reflex. This distinction is based upon the observation that laryngeal spasm is solely mediated by the superior laryngeal nerve. Stimulation of other afferent nerves, capable of eliciting the glottic closure reflex, produces little adductor after-discharge activity that is characteristic of laryngeal spasm. In this regard, modification of output function from the adductor motoneuron aggregate by means of temporal and spatial summation of sensory input dat

Laryngeal spasm complicating pseudomyotonia.

Pseudomyotonia is a syndrome characterized by progressive muscle stiffness, weakness, myokymia, hyperhydrosis, and wasting of muscle. It especially affects the distal aspect of the extremities. The electromyogram is characteristic, and shows continuous muscle activity at rest. We discuss a case that is believed to be the first recorded instance of pseudomyotonia complicated by airway obstruction secondary to laryngeal spasm. Treatment with phenytoin sodium resulted in rapid improvement of the peripheral symptoms and simultaneous resolution of the laryngeal spasm.
