Acute severe neck pain and dysphagia following cervical maneuver: diagnostic approach.

INTRODUCTION: Overlooking an etiologic hypothesis in acute neck pain with dysphagia may lead to misdiagnosis. CASE REPORT: A 51-year-old man who had received cervical manipulation came to the emergency unit with evolutive acute neck pain, cervical spine stiffness and odynophagia, without fever or other signs of identified pathology. Cervical X-ray and CT angiography of the supra-aortic vessels ruled out traumatic etiology (fracture or arterial dissection) and revealed an accessory bone, orienting diagnosis toward retropharyngeal abscess, which was, however, belied by endoscopy performed under

Comparative analysis of emm types, superantigen gene profiles and antibiotic resistance genes among Streptococcus pyogenes isolates from ocular infections, pharyngitis and asymptomatic children in south India.

Group A Streptococcus (Streptococcus pyogenes) is responsible for a wide array of infections and incidence is high in developing countries like India. Although distribution of emm types of S. pyogenes in India has been described, its association with the virulence genes and ocular isolates is less concentrated. In the present study emm type surveillance as well as its association with toxin gene profile was analyzed. Ocular infected cases such as lacrimal abscess, corneal ulcers, mucocoele showed the presence of 20 S. pyogenes isolates. For noninvasive isolates, we screened 370 pharyngitis cas

Indications of pediatric tonsillectomy.

Possible indications for tonsillectomy include sleep apnea and other obstructive sleep-related breathing disorders, recurrent tonsillitis, peritonsillar abscess, periodic fever, aphthous stomatitis, pharyngitis, adenitis (PFAPA), and other miscellaneous rare conditions. Over the last century indications have changed, with a decrease in infectious causes and an increase in sleep apnea disorders. Sleep apnea in children is difficult to diagnose short of polysomnography (PSG) which is expensive and disturbing, especially in young children. In sleep apnea confirmed by PSG, tonsillectomy relieves t

A cost-effectiveness analysis of identifying Fusobacterium necrophorum in throat swabs followed by antibiotic treatment to reduce the incidence of Lemierre's syndrome and peritonsillar abscesses.

The main purpose of this paper was to estimate the cost per quality-adjusted life year (QALY) saved by identifying Fusobacterium necrophorum in throat swabs followed by proper antibiotic treatment, to reduce the incidence of Lemierre's syndrome and peritonsillar abscesses (PTA) originating from a pharyngitis. The second purpose was to estimate the population size required to indicate that antibiotic treatment has an effect. Data from publications and our laboratory were collected. Monte Carlo simulation and one-way sensitivity analysis were used to analyse cost-effectiveness. The cost-effectiv

Similar recovery rates of Fusobacterium necrophorum from recurrently infected and non-infected tonsils.

INTRODUCTION: Recent studies implicate the presence of Fusobacterium necrophorum (FN) in recurrent tonsillitis (RT), acute tonsillitis and peritonsillar abscess. The objective of the present study was to determine whether FN plays a role in RT by comparing bacteriologic results from patients suffering from RT, tonsillar hypertrophy and halitosis or persistent sore throat syndrome (PSTS). We analyzed both tonsils to determine the degree of concordance. MATERIAL AND METHODS: A prospective study was conducted in 80 patients aged 8-30 years who were undergoing elective tonsillectomy. The patients

[Ultrasound of foreign bodies in the head and neck. Cases compared with an in vitro model].

BACKGROUND: High resolution ultrasound is an accepted method in identification and classification of various head and neck alterations. Foreign bodies are a seldom encountered entity and are supposed to have a characteristic echogenicity. In literature there are numerous publications describing diagnosing foreign bodies in other specialities like gynaecology or surgery with ultrasound. The aim of the study was to examine these findings and to reproduce them in the head and neck area. MATERIALS AND METHODS: The study includes the retrospective analysis of 6 cases. 3 cases have been reconstruct

[Pneumonia caused by Fusobacterium necrophorum: is Lemierre syndrome still current?].

Fusobacterium necrophorum is a non-spore-forming gram-negative anaerobic bacillus that may be the causative agent of localized or severe systemic infections. Systemic infections due to F.necrophorum are known as Lemierre's syndrome, postanginal sepsis or necrobacillosis. The most common clinical course of severe infections in humans is a progressive illness from tonsillitis to septicemia in previously healthy young adults. A septic thrombophlebitis arising from the tonsillar veins and extending into the internal jugular vein leads to septicemia and septic emboli contributing to the development

Double-blind, randomised, controlled study of post-operative pain in children undergoing radiofrequency tonsillotomy versus laser tonsillotomy.

INTRODUCTION: In the last decade tonsillotomy has come into vogue again, whereas the number of tonsillectomies is decreasing rapidly. Currently, most tonsils are reduced by utilise electrosurgery, radiofrequency or carbon dioxide laser. However, it is not clear whether radiofrequency tonsillotomy is as effective as laser or other surgical techniques in respect of post-operative pain and haemorrhage. MATERIAL AND METHODS: A prospective, randomised, double-blinded, controlled, clinical study was conducted in the otorhinolaryngology department of Ludwig Maximilians University, Munich, Germany. T

Atypical locations of retropharyngeal abscess: beware of the normal lateral soft tissue neck X-ray.

Retropharyngeal abscesses (RPA) are uncommon but potentially lethal deep neck space infections, over 95% of which occur in children under six years of age. Without a high index of suspicion, early recognition and prompt intervention, catastrophic consequences can ensue, and mortality can be as high as 60% if jugular vein thrombosis or mediastinitis occurs. While older children may have specific complaints referable to the pharynx, infants and young children may present with vague symptoms. To date, a lot of emphasis continues to be placed on the importance of lateral soft tissue neck X-ray in

Aggressive fibromatosis of the oropharynx: a multidisciplinary approach to a benign disease.

We present the case of a 23-year-old woman with aggressive fibromatosis of the oropharynx that was initially treated elsewhere as a peritonsillar abscess. We discuss the characteristics of this rare tumor and review the literature, stressing the importance of postoperative follow-up for peritonsillar abscesses to avoid missing other important diagnoses, such as the one described here.

Fatal retropharyngeal abscess: a possible marker of inflicted injury in infancy and early childhood.

UNLABELLED: Retropharyngeal abscesses are serious infections of the deep tissues of the neck associated with significant morbidity and mortality due to their proximity to vital structures. The most common age range is 2-4 years, with neonates accounting for approximately 10% of cases. The following report demonstrates possible forensic issues that may arise in such cases. CASE REPORT: A 10-week-old infant was found dead in her father's arms an hour after feeding. At autopsy death was due to a large retropharyngeal abscess with disseminated Streptococcus pneumoniae sepsis. Other findings at au

Recurrent acute thyroid swellings because of pyriform sinus fistula.

Anterior neck abscesses are not rare; but their origin from within the usually infection-resistant thyroid gland is not thought of in the first instance. We encountered 3 patients with differing presentations (tender nodule over anterior neck, recurrent abscess overlying the thyroid gland, and nonhealing fistula with inflammation of the anterior neck). These were caused by persistent embryological communication from the pyriform sinus to the thyroid gland to the left lobe. Excluding the first patient in whom an abnormal communication with oropharynx was suspected when actinomyces was detected

Production of white colonies on CHROMagar Candida medium by members of the Candida glabrata clade and other species with overlapping phenotypic traits.

We hypothesized that species of the Candida glabrata clade and species with phenotypic traits that overlap those of C. glabrata would produce white colonies on CHROMagar Candida medium. Of 154 isolates (seven species) tested, C. bracarensis, C. nivariensis, C. norvegensis, C. glabrata, and C. inconspicua produced white colonies; the Pichia fermentans group and C. krusei did not. Many of these species are difficult to identify phenotypically; white colonies may signal the need for the use of molecular approaches.

The masticator space: from anatomy to pathology.

The masticator space is a deep facial space with a complex anatomical structure. The purpose of the present study was to precisely define the masticator space to eliminate the use of obsolete and confusing terms to describe the area, and to illustrate the common mass syndromes. Primary tumors are uncommon, usually benign and of a vascular or neural origin. Adjacent lesions, mainly pharyngeal with secondary extension into the masticator space, are especially frequent. Metastases are rare, and infectious pathology is often odontogenic. The most frequent lesion of the masticator space is the odon

[Occult peritonsillar abscess: an unusual cause of unilateral tonsillar enlargement].

Peritonsillar abscess is a suppurative infection of the tissues adjacent to the palatine tonsil and is regarded in otolarygologic practice as a complication of pharyngotonsillitis. The most common presentation is gradually increasing pain, fever, trismus, drooling and a muffled voice. Asymmetric tonsils are common in clinical practice. Differential diagnosis includes infectious, granulomatous, congenital and neoplastic lesions. This is a case study of two patients evaluated for unilateral tonsillar enlargement who were found to have a previously unexpected peritonsillar abscess at tonsillectom

Calcific retropharyngeal tendinitis: a frequently missed diagnosis. Case report.

Neck pain is a common presentation in emergency departments that requires the exclusion of serious underlying pathological entities. Acute calcific retropharyngeal tendinitis is a rare and underrecognized cause of atraumatic neck pain and stiffness. Although this entity has been reported previously, it is often poorly recognized and is often confused with other more serious lesions, such as infectious, traumatic, and neoplastic conditions. Clinically, it can be very difficult to distinguish from these three disorders, and a definitive diagnosis can only be established using imaging studies. A
