Trends in primary care antibiotic prescribing during the implementation of national stewardship policies: Türkiye, 2011-2019.

BACKGROUND: Antimicrobial resistance is driven by inappropriate antibiotic use, and Türkiye has long shown one of the highest outpatient consumption rates in the WHO European Region. In response, the Ministry of Health implemented stewardship measures from 2014 onward, including the National Action Plan for Rational Drug Use, banning over-the-counter antibiotic sales, integrating e-prescribing with mandatory diagnostic coding, educational initiatives, and audit-feedback systems. This study evaluated temporal trends in primary care antibiotic prescribing in Türkiye between 2011 and 2019 and ass

Oral and oropharyngeal colonization by Enterobacteriaceae: A narrative review on its epidemiology and clinical impact.

The family encompasses a large group of Gram-negative bacteria of the order Enterobacterales, which are habitual residents of the intestinal and urinary tracts, behaving only as allochthons in the oral cavity. There is no consensus regarding the prevalence, determinants and consequences of high aero-digestive colonization by this family of bacteria. In light of this, a narrative review was conducted on the epidemiology and clinical impact of oral and oropharyngeal colonization by , differentiating by age groups. A bibliographic search was conducted in four digital databases: PubMed/MEDLINE, Sc

Microbiological aspects and clinical impact of lower lung field tuberculosis: An observational cohort study in Peru.

OBJECTIVES: Approximately 40% of tuberculosis (TB) cases remain undiagnosed globally. Lower lung field TB (LLF TB) presents atypically and is often misidentified as other lung diseases, leading to diagnostic delays in resource-limited settings. It may contribute to increased TB transmission and mortality. We aimed to identify microbiological determinants of LLF TB and evaluate treatment responses to optimize care. METHODS: We conducted an observational cohort study in Lima, Peru, enrolling adults with microbiologically confirmed pulmonary TB (PTB) diagnosed by GeneXpert MTB/RIF assay or sputu

Testing the efficacy and safety of BIO101, for the prevention of respiratory deterioration, in patients with COVID-19 pneumonia (COVA study): a structured summary of a study protocol for a randomised controlled trial.

OBJECTIVES: As of December, 1st, 2020, coronavirus disease 2019 (COVID-19) caused by SARS-CoV-2, resulted in more than 1 472 917 deaths worldwide and death toll is still increasing exponentially. Many COVID-19 infected people are asymptomatic or experience moderate symptoms and recover without medical intervention. However, older people and those with comorbid hypertension, diabetes, obesity, or heart disease are at higher risk of mortality. Because current therapeutic options for COVID-19 patients are limited specifically for this elderly population at risk, Biophytis is developing BIO101 (20

A Phase I/II Clinical Trial to evaluate the efficacy of baricitinib to prevent respiratory insufficiency progression in onco-hematological patients affected with COVID19: A structured summary of a study protocol for a randomised controlled trial.

OBJECTIVES: Baricitinib is supposed to have a double effect on SARS-CoV2 infection. Firstly, it reduces the inflammatory response through the inhibition of the Januse-Kinase signalling transducer and activator of transcription (JAK-STAT) pathway. Moreover, it reduces the receptor mediated viral endocytosis by AP2-associated protein kinase 1 (AAK1) inhibition. We propose the use of baricinitib to prevent the progression of the respiratory insufficiency in SARS-CoV2 pneumonia in onco-haematological patients. In this phase Ib/II study, the primary objective in the safety cohort is to describe the

Contamination by respiratory viruses on outer surface of medical masks used by hospital healthcare workers.

BACKGROUND: Medical masks are commonly used in health care settings to protect healthcare workers (HCWs) from respiratory and other infections. Airborne respiratory pathogens may settle on the surface of used masks layers, resulting in contamination. The main aim of this study was to study the presence of viruses on the surface of medical masks. METHODS: Two pilot studies in laboratory and clinical settings were carried out to determine the areas of masks likely to contain maximum viral particles. A laboratory study using a mannequin and fluorescent spray showed maximum particles concentrated

The Face of Distaste: A Preliminary Study.

Distaste is a primitive rejection impulse triggered by the ingestion of unpleasant tasting substances, many of which are toxic. Theoretical work has suggested that distaste may be the evolutionary precursor for both physical disgust, which serves to defend against disease and other threats to biological fitness, and moral disgust, which defends against threats to the social order. Consistent with this proposal, recent work has found that the facial expression of distaste may be similar to that of disgust. Specifically, raising of the upper lip has been reported in distaste, physical disgust, a

Laryngomalacia: is there an evidence base for management?

BACKGROUND: The advent of supraglottoplasty clearly has transformed the surgical management of severe laryngomalacia. The condition, however, generally runs a milder course, with spontaneous resolution the norm. OBJECTIVES: To identify gaps in the knowledge and identify topics for future study. METHOD: Systematic review of the literature. RESULTS: The literature suggests that there is a range of abnormalities leading to the typical collapsing upper airway, and that neurological disease, other airway abnormalities, syndromes and gastroesophageal reflux all contribute to disease severity and

Two girl patients with medulloblastoma. Case reports.

UNLABELLED: In childhood, the most common type of brain tumors is medulloblastoma, a highly malignant primary brain tumor that is found in the cerebellum or posterior fossa. The tumor mass increases and generates obstructive hydrocephalus. Risk factors (that might be involved in some cases) include the genetic syndrome such as type 1 neurofibromatosis, exposure to ionizing radiation and Epstein-Barr virus. Medulloblastoma is associated with recessively inherited Turcot disease and with conditions as ataxia-telangiectasia syndrome in several cases. The authors presented two cases of female pati

Minimally interactive segmentation of 4D dynamic upper airway MR images via fuzzy connectedness.

PURPOSE: There are several disease conditions that lead to upper airway restrictive disorders. In the study of these conditions, it is important to take into account the dynamic nature of the upper airway. Currently, dynamic magnetic resonance imaging is the modality of choice for studying these diseases. Unfortunately, the contrast resolution obtainable in the images poses many challenges for an effective segmentation of the upper airway structures. No viable methods have been developed to date to solve this problem. In this paper, the authors demonstrate a practical solution by employing an

Honey plus coffee versus systemic steroid in the treatment of persistent post-infectious cough: a randomised controlled trial.

BACKGROUND: Persistent post-infectious cough (PPC) is a cough that remains after a common cold or an upper respiratory tract infection for more than three weeks or perhaps for many months. Two of the suggested treatments for PPC are systemic steroid and honey plus coffee. AIMS: The aim of this study was to evaluate and compare scientifically the therapeutic effects of these two regimens. METHODS: A double-blind randomised controlled trial was conducted from 2008 to 2011 at the Baqiyatallah University Hospital, Tehran, Iran.
