Statin as Regulatory T Cell Modulator in COPD With Early Disease.

Early detection and intervention are critical to preventing or slowing the progression of COPD to advanced stages. However, no disease-modifying agent is currently available. COPD smokers exhibit reduced regulatory T (Treg) cell levels in lung tissue, whereas normal smokers demonstrate higher levels. Treg cells may mitigate lung damage by suppressing cytolytic CD8+ T-cells. Forced expiratory volume in 1 second (FEV1) correlates with Treg cell levels in COPD. In lung tissue from surgically removed centrilobular emphysema lungs, granzyme (Gzm) B+ cells correlated with FEV1. Gzm B is considered a

Novel mutation (Mangera-E288V) in alpha-1 antitrypsin deficiency.

BACKGROUND: Alpha-1 antitrypsin deficiency is an autosomal, codominant disorder caused by mutations of the SERPINA1 gene. Several mutations have been described associated with the development of pulmonary and/or chronic liver diseases. We report a novel mutation Mangera, identified for the first time in an Italian patient originating from the city of Angera (Varese), Italy. CASE PRESENTATION: This case report describes a 64-year-old Italian male, a lifelong non-smoker, diagnosed with severe alpha-1 antitrypsin deficiency (AATD) as composed heterozygous with S allele and a novel mutation named

Linking Lungs and Heart: Centrilobular Emphysema and Coronary Artery Calcification Progression Over 18 Years of Follow-Up.

BACKGROUND: Low-dose CT (LDCT) imaging of the chest for lung cancer screening provides an opportunity to evaluate smoking-related comorbidities such as emphysema and coronary artery calcification (CAC). Although cross-sectional associations exist, the long-term relationship between emphysema subtypes and CAC progression remains undefined. RESEARCH QUESTION: Is any emphysema subtype associated with long-term CAC progression? STUDY DESIGN AND METHODS: A total of 256 participants with ≥ 15 years of follow-up were included from a prospective cohort of 9,047 asymptomatic, high-risk individuals en

Prognostic impact of emphysema extent on mortality and lung function decline in combined pulmonary fibrosis and emphysema: An observational cohort study.

BACKGROUND: Combined pulmonary fibrosis and emphysema (CPFE) is a well-recognized syndrome characterized by the coexistence of emphysema and fibrotic interstitial lung disease (ILD) and is associated with high mortality. However, the prognostic significance of emphysema extent relative to fibrosis pattern remains incompletely defined. METHODS: We conducted an observational cohort study of participants with fibrotic ILD and radiographic emphysema. Baseline high-resolution computed tomography (HRCT) scans were independently assessed by two thoracic radiologists using a standardized semi-quantit

Comparison of Lung Cancer Screening Programs in Liver Transplant Recipients and the General Population.

OBJECTIVES: The objectives of this study are to analyse whether liver transplant recipients (LTR) have a higher incidence of lung cancer (LC) compared to a population of high risk smokers enrolled in a LC screening program and to evaluate if LC screening results allow an early diagnosis in LTRs. METHODS: We conducted a retrospective study comparing LC screening outcomes using low-dose computed tomography (LDCT) in 124 LTRs and 485 matched non-immunosuppressed controls. Matching criteria included age, sex, smoking history, active smoking status, and presence of emphysema. Tumour characteristic

CT findings in biomass smoke-associated interstitial lung disease: A retrospective analysis.

OBJECTIVES: Household air pollution from biomass fuel combustion remains a major global health burden, affecting nearly 3 billion people worldwide. Although biomass smoke exposure is a recognized risk factor for chronic airway disease, its role in interstitial lung disease (ILD) is less well defined, and characteristic CT features have not been systematically described in patients without coexisting chronic obstructive pulmonary disease (COPD). MATERIAL AND METHODS: We conducted a retrospective review of 23 patients with biomass smoke exposure and radiographic evidence of ILD, excluding those

Paraseptal Emphysema and Interstitial Lung Abnormalities in AGES-Reykjavik Study.

BACKGROUND: Paraseptal emphysema (PSE) is often underrecognized because it has minimal impact on pulmonary function. Recent studies suggest its frequent coexistence with interstitial lung abnormalities (ILA) and potential clinical significance, yet population-based data on its prevalence and prognostic impact remain unclear. PURPOSE: To examine the associations between visual emphysema subtypes and ILA, and to evaluate the prognostic impact of ILA in individuals with emphysema subtypes. MATERIALS AND METHODS: We analyzed 5,059 participants from Age Gene/Environment Susceptibility Reykjavik S

Emphysema in an 11-month-old boy with alpha-1 antitrypsin deficiency.

INTRODUCTION: Alpha-1 antitrypsin deficiency (A1ATD) is characterized by reduced levels of protease inhibitor alpha-1 antitrypsin, most commonly manifesting with liver disease in childhood, and/or pulmonary disease starting in the 3rd decade of life. Childhood-onset lung disease in A1ATD has almost never been reported. CASE PRESENTATION: An 11-month-old boy with a history of neonatal cholestasis and A1ATD phenotype PiZZ presented with prolonged fever of unknown origin. A computed tomography chest was obtained, given continued diagnostic uncertainty and the history of A1ATD, showing paramedias

Delayed Diagnosis of Alpha-1 Antitrypsin Deficiency in an Elderly Patient.

Background and Clinical Significance: Alpha-1 antitrypsin deficiency (AATD) is an autosomal codominant disorder caused by pathogenic variants in the SERPINA1 gene, resulting in reduced circulating alpha-1 antitrypsin (AAT) or production of dysfunctional protein. AAT is the principal inhibitor of neutrophil elastase, and its deficiency leads to unchecked proteolytic activity, progressive destruction of lung parenchyma, and increased susceptibility to infections. Severe deficiency, particularly in individuals homozygous for the Z allele (PI*ZZ), predisposes to early-onset panacinar emphysema, ch

Synthetic data augmentation for CT-based emphysema subtype classification: A comparative evaluation of generative and classical approaches.

Data scarcity is a persistent challenge in medical image analysis. Synthetic data generation using deep generative models has been proposed as a potential approach to address this limitation, yet its performance in small-data settings remains poorly characterized. This study compared three class-conditional generative approaches, a conditional variational autoencoder (cVAE), a conditional shallow-decoder VAE variant (cSD-VAE), and a conditional Wasserstein GAN with gradient penalty (cWGAN-GP), against classical geometric augmentation for emphysema subtype classification on 168 CT patches (thre

Nebulized Potassium-Conjugated Linoleic Acid (K-CLA) Therapy in a Patient With Chronic Obstructive Pulmonary Disease and Centrilobular Emphysema: A Case Report.

Chronic obstructive pulmonary disease (COPD) is a progressive respiratory disorder characterized by persistent airflow limitation that commonly causes dyspnea and reduced exercise capacity. Emphysema, a major component of COPD, involves destruction of alveolar tissue and loss of elastic recoil, resulting in impaired gas exchange and air trapping. We report a 60-year-old female with COPD and centrilobular emphysema who underwent nebulized potassium-conjugated linoleic acid (K-CLA) therapy for persistent dyspnea and exercise intolerance despite standard inhaled therapy. Following two months of t

A Single-Cell Transcriptional Landscape of Emphysema: Heterogeneous Cellular Dynamics in Centrilobular, Panlobular, and Paraseptal Subtypes.

Emphysema is a chronic lung disease characterized by irreversible alveolar destruction, with distinct imaging subtypes-centrilobular emphysema (CLE), panlobular emphysema (PLE), and paraseptal emphysema (PSE). The cellular and molecular mechanisms driving subtype-specific pathogenesis remain poorly understood. Here, we performed single-cell RNA sequencing on emphysematous lung tissues from nine patients (three per subtype) to map the transcriptional landscape across CLE, PLE, and PSE. Profiling 84,704 high-quality cells, we identified eight major cell types, and subsequent subclustering uncove

Dexmedetomidine-esketamine combination for anesthesia in a patient with severe Swyer-James-Macleod syndrome: a case report.

BACKGROUND: Swyer-James-Macleod Syndrome (SJMS) is a rare respiratory disease usually caused by obliterative bronchiolitis since childhood and characterized by expiratory air trapping, hypoxia-induced parenchymal destruction, pulmonary hypoperfusion, and impaired gas exchange. Anesthetic management for bronchoscopy in patients with SJMS is challenging due to compromised cardiopulmonary function and shared airway paradigm during the procedure. Herein, we report the use of dexmedetomidine-esketamine combination during anesthesia induction and maintenance for bronchoscopy in a patient with severe

Swyer-James-MacLeod Syndrome: A Rare Cause of Unilateral Hyperlucent Lung in a Two-Year-Old Male Child.

Swyer-James-MacLeod syndrome (SJMS) is an uncommon post-infectious bronchiolitis obliterans-related pulmonary disorder characterized by a unilateral hyperlucent lung with associated hypovascularity on chest radiography and CT, and markedly reduced perfusion on ventilation-perfusion scintigraphy. Although typically identified in older children or adolescents due to its insidious progression, diagnosis in early childhood remains rare and diagnostically challenging. This report describes a two-year-old male child with recurrent lower respiratory tract infections who was ultimately diagnosed with

cGAS‑STING pathway in digestive tract cancer: From molecular mechanisms to therapeutic applications (Review).

BACKGROUND: Idiopathic lymphoplasmacytic rhinitis (LPR) is a common inflammatory disorder of the nasal cavity in dogs due to unknown etiology. It is characterised by non-specific clinical signs, including nasal discharge, epistaxis and breathing problems. Diagnosis is usually based on the histopathologic identification of infiltrating plasmocytes and lymphocytes in the nasal mucosa and the exclusion of other underlying diseases. Treatment strategies include glucocorticoids, non-steroidal anti-inflammatory drugs, antibiotics and antifungal medications. The aim of this study was to evaluate the

Intrapulmonary percussive ventilation for the management of destroyed lung in Swyer-James-MacLeod syndrome.

Swyer-James-MacLeod syndrome (SJMS) is a rare disorder resulting from post-infectious bronchiolitis obliterans following lower respiratory tract infection in childhood and may be complicated by bronchiectasis. We report the case of a man in his 60s with a history of childhood pneumonia who was diagnosed with bronchiectasis associated with SJMS. Against a background of chronic Pseudomonas aeruginosa airway infection, progressive structural destruction of the left lung eventually resulted in a destroyed lung.
