A randomized controlled trial of adjunctive speleotherapy in asthma, COPD and long COVID.

Speleotherapy (underground climate therapy) is a non-pharmacological intervention for chronic respiratory diseases. This randomized controlled trial investigated whether a 3-week speleotherapy course (6 sessions, 2 h/week) improves respiratory outcomes in patients on standard background therapy with asthma, COPD, or Long COVID, and whether it affects blood CO2 levels. The control group did not receive speleotherapy but continued their standard therapy. A total of 208 patients (asthma: n = 107; COPD: n = 59; Long COVID: n = 42) were enrolled across nine centers in Germany, Austria, and Italy. A

Global, regional and national burden of chronic obstructive pulmonary disease and lower respiratory infection attributable to household air pollution and secondhand smoke from 1990 to 2021: an analysis of the Global Burden of Disease Study (GBD 2021).

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is a heterogeneous disease that can be influenced by a multitude of factors. As the most significant cause of acute exacerbation of COPD, lower respiratory tract infections (LRIs) still result in a heavy global burden in 2021. The 1990-2021 period encapsulates major global shifts, including the energy transition from solid fuels to clean energy in emerging economies and the implementation of the World Health Organization (WHO) Framework Convention on Tobacco Control (FCTC), making it crucial for public health analysis. This study aims to

Consensus based expert recommendations for management and prevention of RSV infections in older adults in emerging market countries: results from an expert panel structured survey.

BACKGROUND: Respiratory syncytial virus (RSV) can cause serious disease in older adults (OAs), particularly those with comorbidities. It is a leading cause of acute respiratory illness and lower respiratory tract infections in adults ≥60 years. Its impact on morbidity, mortality, and healthcare systems is becoming evident. Questions about the burden of disease (BoD) are raised due to underdiagnosis and the similarity with other respiratory illnesses. First opportunities for prevention came in 2023 and 2024, with vaccines approved for ≥60 years, expanding eligibility to additional age groups.

Upper and lower airway crosstalk in acute exacerbations of COPD: a clinical and biological overview.

INTRODUCTION: Acute exacerbations of chronic obstructive pulmonary disease (AE-COPD) are acute worsening events characterized by increased dyspnea, cough, and sputum production. Although traditionally viewed as lower airway events, growing evidence suggests that AE-COPD may reflect broader pan-airway dysfunction involving both upper and lower respiratory compartments. AREAS COVERED: This overview examines upper - lower airway crosstalk in AE-COPD across three domains: pan-airway inflammation, epithelial alarmin/cytokine networks, and the continuous airway microbiome. We discuss the coexistenc

High-fidelity simulation as a translational and educational platform for crisis ventilation: quantifying and mitigating tidal volume maldistribution during shared mechanical ventilation.

BACKGROUND: Shared mechanical ventilation has been proposed during ventilator shortages, but unequal tidal volume delivery remains a major limitation when lung compliance differs. Professional societies have cautioned against this practice, yet the intervention has never been validated clinically, and ethical constraints preclude direct patient testing. Translational simulation-simulation applied beyond education to probe clinical systems, test crisis-driven innovations, and generate actionable safety data-offers a methodological pathway for evaluating interventions that cannot be studied in v

Association between Clonorchis sinensis infection and the risk of pulmonary fungal infection in patients with AECOPD: a retrospective propensity score matching study.

BACKGROUND: Pulmonary fungal infection (PFI) is a severe complication in patients with acute exacerbations of chronic obstructive pulmonary disease (AECOPD). Although helminth infections are increasingly recognized as potent modulators of host immunity, their influence on susceptibility to opportunistic fungal infections remains poorly understood. We investigated whether Clonorchis sinensis (C. sinensis) infection is associated with risk of PFI in AECOPD patients and explored potential immunological mechanisms underlying this association. METHODS: We conducted a retrospective cohort study inc

Antibiotic pressure does not uncover intra-host heterogeneity of Pseudomonas aeruginosa in patients with chronic lung disease.

Pseudomonas aeruginosa colonization and infection in patients with chronic obstructive pulmonary disease (COPD) or non-cystic fibrosis bronchiectasis (NCFBE) are associated with increased airway inflammation and mortality. Pseudomonas-active antibiotic therapy is a frequent therapeutic intervention in acute bacteria-driven exacerbation or when eradication of P. aeruginosa colonization is intended. However, the impact of intra-patient heterogeneity of P. aeruginosa on eradication outcomes remains unclear. We enrolled 30 patients receiving inpatient care for COPD and/or NCFBE exacerbation with kn
