The role of mucus in chronic obstructive pulmonary disease.

Chronic bronchitis is characterized by mucociliary dysfunction resulting from structural and functional defects of cilia and the secretory apparatus. The combination of hypersecretion and ciliary impairment leads to disruption of mucociliary interaction and hence the accumulation of secretions in the lower airways. Cigarette smoke appears to play a critical role in the pathogenesis of chronic bronchitis-associated mucociliary dysfunction. While the excessive lower airway secretions may have only minor effects on the natural course of airflow obstruction, they could transiently compromise airwa

[Tuberculosis in a general hospital 1987].

Tuberculosis (TB) was diagnosed in 15 patients admitted during a 1-year period starting November 1986. 4 had been treated with corticosteroids prior to diagnosis. Most were admitted with exacerbation of chronic obstructive pulmonary disease, acute respiratory failure and prolonged fever. The most frequent X-ray findings were infiltrates in the lower lobes and the middle lobe. Sputum cultures were positive for TB in only 4 patients, while culture of gastric aspirates was diagnostic in 9. Our findings point to an increase in the detection of TB in the Negev. It must be ruled out in patients with

[Controlled multicentric study comparing cefixime and amoxicillin in the treatment of lower respiratory tract infections in adults].

This multicentre, randomized, double-blind study evaluated the effectiveness and safety of cefixime versus amoxicillin. Patients were admitted if they had lower respiratory tract infection with a bacterial pathogen susceptible to both study drugs. Diagnoses included acute respiratory tract infections with no underlying pulmonary pathology (cefixime 21, amoxicillin 27), acute exacerbation of chronic obstructive lung disease (cefixime 32, amoxicillin 42), superinfection of viral bronchitis or lung cancer, and pleuritis (cefixime 10, amoxicillin 6). Patients were treated for at least 10 days with

Long-term pulmonary sequelae after Legionella pneumophila infection in the hamster.

It has been reported that infections with Legionella pneumophila can lead to chronic inflammatory and fibrotic reactions in the human lung. To better characterize the nature of the residual abnormalities caused by this bacterium, we inoculated Syrian hamsters intratracheally with 10(8) serotype 1 L. pneumophila organisms and assessed histologic, functional, and biochemical changes at intervals up to 180 days. Acutely, L. pneumophila caused an intense alveolar macrophage (AM) and polymorphonuclear leukocyte (PMN) response within the lower air spaces, air-filled lungs were noncompliant, and ther

Enoxacin in the treatment of lower respiratory tract infections.

Twenty patients admitted to hospital with serious lower respiratory tract infections entered an open study of 400 mg enoxacin given orally twice a day for a minimum of seven days. Clinical signs and symptoms were completely cured or improved in six of eight patients with pneumonia, in nine of ten patients with bronchitis, and in two patients with bronchiectasis. Enoxacin was effective in eradicating the initial pathogens in 10 of 12 patients with positive cultures. Enoxacin was well tolerated. Only one treatment-related side-effect was observed during the study. Nine patients received concurre

Clinical outcome of respiratory failure in patients requiring prolonged (greater than 24 hours) mechanical ventilation.

Patients requiring prolonged (greater than 24 hours) mechanical ventilation have various conditions that result in respiratory failure. All patients requiring prolonged mechanical ventilation were subdivided into the following six groups: uncomplicated acute lung injury; respiratory failure complicated by multisystem failure; previous lung disease; trauma; other medical causes; and routine postoperative ventilation. During a one-year period, 327 patients required prolonged mechanical ventilation; acute lung injury and chronic obstructive pulmonary disease were the predominant conditions. Sepsi

Incidence, utilization, and costs associated with acute respiratory conditions, United States, 1980.

Acute respiratory conditions are common causes of health disturbance in the general population. They are generally self-limiting, although occasionally recurrent, and seldom result in large health care costs for each episode of illness. The National Medical Care Utilization and Expenditure Survey (NMCUES), conducted during 1980, provided an opportunity to assess the effect of acute respiratory conditions on utilization of medical services and on functional capability as well as the cost of related medical care. Acute respiratory conditions were reported by survey respondents and separated into

Serological, clinical and radiological findings in adults with bronchopulmonary infections caused by Chlamydia trachomatis.

We report here four cases of lower respiratory tract infection caused by Chlamydia trachomatis. Three of them had clinical and radiological findings of atypical pneumonia, while one presented with an acute exacerbation of chronic obstructive pulmonary disease (COPD). None of the patients had signs of ocular or genitourinary infections. All patients had specific IgM, IgA and IgG antibodies for C. trachomatis during the acute infection, and were seronegative for C. psittaci. No specific clinical or radiological patterns were found. Two of the patients with pneumonia were laboratory workers who w

Respiratory morbidity in smokers of low- and high-yield cigarettes.

To study the association between smoking cigarettes with a low yield of tar and nicotine (tar less than 15.0 mg per cigarette and nicotine less than 1.0 mg) and respiratory disease, we reviewed the medical records of 4,610 current, regular cigarette smokers and 2,035 persons who had never used any form of tobacco and who were enrolled in a smoking study. In the year after recruitment to the study, the percentage of subjects with pneumonia or influenza was lower in female but not in male smokers of low-yield cigarettes. The percentage of subjects with any disease of the respiratory tract was lo

Nutrition and respiratory disease.

Correlations between nutritional status and respiratory function, effects of nutritional substrates on respiration, and optimal nutritional support during respiratory failure are reviewed. Somatic protein depletion is common in patients with either acute respiratory failure or chronic obstructive pulmonary disease. The etiology of the malnutrition includes decreased nutrient intake, increased work of breathing, and increased metabolic rate caused by infections. Excessive administration of glucose or protein can have detrimental effects on respiratory status. Weaning patients from ventilators m

Relationship of neutrophil cytoplasmic protein (L1) to acute and chronic lung disease.

L1 protein is a neutrophil cytoplasmic protein and its measurement in body fluids probably reflects neutrophil turnover. The serum range in normal subjects is wide (60-5700 microgram/l) with a median value of 547 microgram/l but lower (2P less than 0.01) than in patients with chronic obstructive bronchitis (median, 750; range, 99-500 microgram/l). Patients with "emphysema" do not have increased serum L1 protein concentrations. On the other hand patients with a variety of active lung diseases have raised concentrations compared to normal subjects suggesting that L1 protein is a marker of inflam

Long-term prospective study in children after respiratory syncytial virus infection.

We have prospectively evaluated for the past 8 years 29 children who were hospitalized during infancy with acute lower respiratory tract illness caused by respiratory syncytial virus (RSV). No differences in the prevalence of a family history of atopy or breast-feeding in these infants compared with controls were noted. However, a history of parental smoking was significantly associated with hospital admission for RSV lower respiratory tract disease. Evidence of atopy, as defined by serum IgE levels and radioallergosorbent testing, have developed in only three (10%) of 29 children. Six childre

Serum and sputum alpha 2 macroglobulin in patients with chronic obstructive airways disease.

Serum alpha 2 macroglobulin concentrations were measured in patients with chronic obstructive airways disease and an age-matched group of control subjects. The mean serum level of alpha 2 macroglobulin was significantly lower in bronchitic subjects with acute chest infections than the mean value of the controls. No significant differences were observed between serum alpha 2 macroglobulin values in controls, subjects with "emphysema", and bronchitic patients who did not have chest infections. Sputum alpha 2 macroglobulin concentrations were compared in sputum samples from bronchitic patients wi

Persistent wheeze. Its relation to respiratory illness, cigarette smoking, and level of pulmonary function in a population sample of children.

In a study of early-life risk factors for the development of adult obstructive airway disease, respiratory symptoms, disease and smoking histories, and spirometry were obtained for 650 children 5 to 9 yr of age and their families in East Boston, Massachusetts. Persistent wheezing was the most frequently reported chronic symptom, occurring in 9.2% (60/650) of the population. Children with persistent wheezing were more likely to report cough and phlegm (p < 0.001), a history of asthma (p < 0.001), hay fever (p < 0.02), or past hospitalization with a respiratory illness (p < 0.001) than their asy

Bronchiolitis obliterans, bronchiectasis, and other sequelae of adenovirus type 21 infection in young children.

A remarkably high incidence of bronchiectasis and other pulmonary sequelae was observed in young children affected during an epidemic of severe lower respiratory tract infections apparently caused by adenovirus type 21. The histopathological findings are described in four cases in which one or both lungs were obtained for examination at intervals ranging from two months to three years after the acute infections. Widespread bronchiolar obliteration (bronchiolitis obliterans) was a striking finding in all four. The severity of bronchial inflammation and of bronchiectasis was proportional to the

Lung disease with chronic obstruction in opium smokers in Singapore. Clinical, electrocardiographic, radiological, functional and pathological features.

Fifty-four opium smokers with chronic obstructive lung disease were studied for two-and-a-half years. Forty-eight patients had a cough for at least two years before the onset of inappropriate exertional dyspnoea. Fine, bubbling adventitious sounds suggesting small airway disease were heard on auscultation over the middle and lower lobes in 38 patients. The prevalence of inflammatory lung disease and chronic respiratory failure in this series is suggested as the main cause for the frequent finding of right ventricular hypertrophy and congestive heart failure. Physiological studies revealed mode
