End of life content in geriatric textbooks: what is the current situation?

BACKGROUND: Physicians caring for elderly people encounter death and dying more frequently than their colleagues in most other disciplines. Therefore we sought to examine the end-of-life content in popular geriatric textbooks and determine their usefulness in helping geriatricians manage patients at the end of their lives. METHODS: Five popular geriatric textbooks were chosen. Chapters on Alzheimer's disease, stroke, chronic heart failure, chronic obstructive pulmonary disease and lung cancer were examined because of their high mortality rates among the elderly patients. Text relevant to end-

[Projections of future resource use and the costs of asthma and COPD in the Netherlands].

OBJECTIVE: To determine the health-care utilisation and associated costs for asthma and chronic obstructive pulmonary disease (COPD) in the Netherlands for the year 2000 and to estimate future costs by means of projections. DESIGN: Descriptive and explorative. METHOD: The costs were based on data regarding resource use in 2000 obtained from representative national registries and surveys. The costs were specified by gender and age category and for COPD also by severity. Three projections for the costs of asthma and COPD were made by means of prevalence projections: first assuming constant res

Freemartinism in river buffalo: clinical and cytogenetic observations.

OBJECTIVES: The correlation between quartz dust concentrations in the Swiss enterprises and the incidence of silicosis and other related diseases acknowledged as occupational diseases (OD) was investigated. MATERIAL AND METHODS: Quartz dust concentrations were obtained from Suva's databases of occupational health surveillance measurements between 2005 and 2014. Information on quartz dust-related diseases was from medical dossiers of workers with OD acknowledged by Suva between 2005 and 2014. RESULTS: The median quartz dust concentration of the 2579 measurements between 2005 and 2014 was 0.09

[Dying in the hospital from an end-stage non-oncologic disease: a decision making analysis].

BACKGROUND AND OBJECTIVE: The aim of the study was to evaluate palliative decision-making in non- oncological patients who died in an acute care hospital. PATIENTS AND METHOD: 293 patients > 64 years old were analyzed. These patients suffered from dementia (46%), end-stage congestive heart failure (31%) and end-stage chronic obstructive pulmonary disease (23%). We evaluated written information about: do not resuscitate (DNR) orders, graduation of therapeutic decisions, information provided to relatives about prognosis, total withdrawal of other drug therapy and provision of terminal care. Age, aging, and the sense of control among older adults: a longitudinal reconsideration.

OBJECTIVES: The purpose of this study was to first estimate the crude cross-sectional and longitudinal associations between age and the sense of control, and then to partition any joint variance attributable to a theoretically specified set of potential confounders. METHODS: Sense of control was measured at baseline and at each of six bimonthly follow-up interviews among 1,662 patients at two medical centers. Potential confounders were measured at baseline. Analyses include descriptive assessments of level and normative stability, repeated measures analysis of covariance, and hierarchical mul

Comorbidities and associated treatment charges in patients with anxiety disorders.

OBJECTIVE: To describe comorbidities and treatment charges in patients receiving initial anxiolytic therapy for anxiety disorders. DESIGN: Retrospective data analysis. PATIENTS: Six thousand six hundred forty-seven patients with anxiety disorders. MEASUREMENTS AND MAIN RESULTS: Analysis was conducted using medical and pharmacy data for patients in 26 United States health plans from the PharMetrics Integrated Outcomes database. Data were collected for 12 months before and after the first anxiolytic prescription date (index date) during the study period (January 1, 1998-December 31, 2000). Th

Noninvasive positive pressure ventilation for respiratory failure caused by exacerbations of chronic obstructive pulmonary disease: a standard of care?

This editorial comments on a meta-analysis of the use of noninvasive positive pressure ventilation to treat patients with acute respiratory failure caused by chronic obstructive pulmonary disease (COPD) exacerbations published in the British Medical Journal earlier this year. Based on its analysis of seven randomized controlled trials that met pre-specified criteria, the meta-analysis demonstrated highly significant benefits of noninvasive positive pressure ventilation in COPD patients, including a reduction in a combined end-point consisting of death and endotracheal intubation. The editorial

Modulation of oesophago-UOS contractile reflex: effect of proximal and distal esophageal distention and swallowing.

BACKGROUND: Respiratory depression is a serious perioperative complication associated with morbidity and mortality. Recently, technology has become available to wirelessly monitor patients on regular surgical wards with continuous pulse oximetry and wireless clinician notification with alarms. When a patient's SpO2 falls below a set threshold, the clinician is notified via a pager and may intervene earlier to prevent further clinical deterioration. To date, the technology has not been evaluated with a randomized controlled trial (RCT). METHODS: We designed a parallel-group unblinded pilot RCT

Role of commensal bacteria in chronic experimental colitis: lessons from the HLA-B27 transgenic rat.

BACKGROUND: The combination of an inhaled corticosteroid (ICS) and a long-acting bronchodilator is recommended in the treatment of patients with chronic obstructive pulmonary disease (COPD) who have frequent exacerbations. Budesonide/formoterol dry powder inhaler (DPI) has demonstrated efficacy and tolerability in patients with COPD. OBJECTIVE: To evaluate the efficacy and tolerability of budesonide/formoterol administered via one hydrofluoroalkane pressurized metered-dose inhaler (pMDI) in patients with COPD. METHODS: This was a 6-month, randomized, double-blind, double-dummy, placebo-contr

Effect of macrolides as part of initial empiric therapy on medical outcomes for hospitalized patients with community-acquired pneumonia.

OBJECTIVE: The goal of this study was to compare the outcomes of hospitalized patients receiving a nonpseudomonal third-generation cephalosporin with or without a macrolide for the treatment of community-acquired pneumonia (CAP). BACKGROUND: The initial treatment of CAP is usually based on empirically selected antibiotic therapy. The need for coverage of atypical pathogens in hospitalized patients is widely debated, and regional variations may exist. METHODS: All patients admitted to a community hospital or to a university hospital for 1 year who were aged > or =18 years and had a principal

Patient compliance with peak flow monitoring in chronic obstructive pulmonary disease.

BACKGROUND: The factors affecting patient compliance with peak flow monitoring in advanced chronic obstructive pulmonary disease (COPD) were examined using a prospective, blinded study. METHODS: Twenty-eight male veterans were instructed in the use of an electronic, hand-held peak flow meter and the modified Medical Research Council dyspnea scale. They then entered a 6-month monitoring phase in which they recorded a dyspnea score once daily and peak expiratory flow rates twice daily, before and after bronchodilator use. The meter displays were disabled so that the patients were blinded to the

Institutional and patient criteria for heart-lung transplantation.

Heart-lung transplantation is a complex, expensive, and resource-intensive procedure. It is performed more often where there is coexistence of end-stage pulmonary disease with advanced cardiac disease that is not secondary to pulmonary hypertension (e.g., some forms of congenital heart disease, some situations with Eisenmenger's syndrome, and possibly the coexistence of end-stage heart and end-stage lung disease). Available data indicate that some disease states are associated with a higher probability of successful outcome after HLT than are others. Objectively validated and reliable patient
