Advances in the Diagnosis and Management of Allergic Bronchopulmonary Aspergillosis/Mycosis.

Allergic bronchopulmonary aspergillosis/mycosis (ABPA/ABPM) is an immunologically mediated pulmonary disorder triggered by the colonization of filamentous fungi in the airways of patients with asthma or cystic fibrosis, leading to type I and III hypersensitivity responses, eosinophilic mucus plug formation, and central bronchiectasis. Over the past decade, advances in basic and clinical research have substantially reshaped the understanding and management of this disease. Although Aspergillus species, particularly Aspergillus fumigatus, remain the predominant causative fungi, epidemiological s

Frailty and risk of common chronic diseases among cancer survivors: a prospective cohort study.

PURPOSE: We examined the impact of baseline frailty on the incidence of diverse chronic diseases among cancer survivors. METHODS: We analyzed 21,577 cancer survivors (aged 39-70) from the UK Biobank, excluding those with baseline diseases of interest. Baseline frailty was assessed using the frailty phenotype (range, 0-5) and the frailty index (range, 0-1). Incident cases across 39 chronic diseases were identified via hospital and death records. Cox proportional hazards models were used to estimate hazard ratios (HRs) and 95% CIs. RESULTS: Frailty was significantly associated with an increase

Evaluating the Efficacy of Brensocatib in Non-cystic Fibrosis Bronchiectasis.

Bronchiectasis is a widely recognised chronic respiratory illness classified in severity by frequency of exacerbations within the past year, percent predicted forced expiratory volume in 1 second (%FEV1), previous hospital admissions and radiological severity, among other factors. The physiological process is understood to be driven by neutrophilic inflammation and subsequent bronchial wall destruction and dilatation. Brensocatib has emerged as a novel adjunct in the management of chronic bronchiectasis as a dipeptidyl peptidase-1 inhibitor, directly targeting the inflammatory process. The edi

A novel homozygous frameshift mutation in the CCDC151 causing primary ciliary dyskinesia with azoospermia in a Chinese family.

BACKGROUND: CCDC151 is essential for outer dynein arms (ODA) docking and assembly, and loss of its function impairs mucociliary clearance, leading to primary ciliary dyskinesia (PCD). Pathogenic variants in CCDC151 have been reported rarely, and azoospermia associated with CCDC151-related PCD remains poorly characterized. METHODS: Whole exome sequencing (WES) and sanger sequencing were used to identify disease-related genes in a Chinese family with PCD. Domain analysis was applied to predict the potential impact of the variant on CCDC151. Respiratory multiciliated cells were examined by high

The dipeptidyl peptidase 1 inhibition landscape in bronchiectasis and its future direction.

Bronchiectasis (BE) is a heterogeneous lung disease characterised by chronic neutrophilic inflammation. A "vicious vortex" develops through the interactions between neutrophilic inflammation, infection and impaired mucociliary clearance, leading to progressive structural lung damage and worsening lung function. As part of this neutrophilic inflammation, neutrophil serine proteases (NSPs) overwhelm their natural inhibitors, with high NSP levels being associated with pulmonary exacerbations in people with BE. NSPs, activated by dipeptidyl peptidase 1 (DPP1) during neutrophil maturation in the bo

Nonparametric Prioritization of Breath VOC Signatures for Differential Diagnosis of Asthma, Bronchiectasis, and COPD.

Accurate differentiation among asthma, bronchiectasis, and chronic obstructive pulmonary disease (COPD) remains a major clinical challenge due to overlapping symptoms and limitations of conventional diagnostic approaches. This study proposes a nonparametric cumulative distribution function (CDF)-based framework combined with permutation testing to analyze volatile organic compounds (VOCs) in exhaled breath as non-invasive biomarkers for respiratory disease characterization. Breathomics data obtained via gas chromatography-mass spectrometry (GC-MS) from 121 patients were analyzed following log₂

Allergic bronchopulmonary mycosis caused by Schizophyllum commune in a patient with severe food allergy and asthma.

Schizophyllum commune is a basidiomycete. Allergic bronchopulmonary mycosis (ABPM) is mainly caused by Aspergillus species; however, that caused by S. commune is occasionally found, especially in Japan. The patient was a 46-year-old female with a history of severe food allergies, asthma, and anaphylaxis. Atelectasis and bronchiectasis were observed in the left lower lobes of the lungs. The patients showed high levels of IgG and IgE against Aspergillus; however, anti-Asp f 1 IgE, Aspergillus antigen in serum, and Aspergillus species in respiratory specimen cultures were not detected. S. commune

'Life has become a lot easier'-the acceptability of the treatable traits clinic experience to people with bronchiectasis.

INTRODUCTION: The treatable traits approach involves the identification of traits that are measurable and clinically relevant to the patient, which informs an individualized management plan. While improved quality of life and health outcomes have been demonstrated, the acceptability of this approach to people with bronchiectasis has not been explored. The aim of our study was to explore the acceptability of the treatable traits approach to people with bronchiectasis in a regional setting. PATIENTS AND METHODS: An exploratory qualitative study. Interviews were conducted with adults who first a

Bronchiectasis prevalence, incidence, and exacerbation rates among insured adults in the U.S.: a descriptive analysis.

RATIONALE: Contemporary population-representative data on bronchiectasis epidemiology in the United States remain limited. Existing exacerbation estimates are derived primarily from referral registries that overrepresent severe disease under subspecialty care, while incidence and prevalence calculations are largely based on outdated single-payer data, limiting generalizability. OBJECTIVES: To estimate non-cystic fibrosis bronchiectasis prevalence and incidence among insured U.S. adults during 2016-2024, characterize exacerbation rates and their composition, and identify baseline factors assoc

Patient-Reported Outcome scale for Bronchiectasis (BE-PRO): development and validation in China.

BACKGROUND: Patient-reported outcomes (PROs) have become essential endpoints in clinical trials for bronchiectasis. However, no validated PRO instrument currently exists for the Chinese population. This study aimed to develop and validate a disease-specific Patient-Reported Outcome scale for Bronchiectasis (BE-PRO) tailored to Chinese patients. METHODS: A mixed-methods approach was used to develop the BE-PRO. The conceptual framework and initial item pool were developed primarily from semi-structured patient interviews and supplemented by literature and existing-instrument reviews and medical

Exacerbation trajectories in biologic-eligible severe asthma patients.

BACKGROUND: Asthma biologics are highly effective in reducing exacerbations. Improvement in exacerbations after biologic initiation is often attributed to treatment effect. RESEARCH QUESTION: What proportion of biologic-eligible severe asthma patients improve without biologic therapy, and what characterises those who remain frequent exacerbators? STUDY DESIGN AND METHODS: Biologic-naïve adults with severe asthma were identified in nationwide UK electronic medical records. Year 0 was defined as the first year patients met severe asthma criteria with ≥3 systemic corticosteroid-treated exacerba

Aspergillus tubingensis pneumonia with pulmonary oxalosis in a virologically suppressed person with HIV: A case report.

Opportunistic aspergillosis has occasionally been reported in individuals with advanced HIV infection but remains rare overall and is particularly uncommon under well-controlled conditions. Here, we present a case of Aspergillus tubingensis pneumonia in a virologically suppressed person with HIV (PWH) in his late sixties. He was receiving bictegravir-based antiretroviral therapy, with a CD4 cell count of 368/μL and undetectable HIV RNA two months before symptom onset. His symptoms began with a cough, followed 20 days later by a fever and dyspnea, which required hospitalization. Computed tomogr

When a Lung Collapses: A Case of Polymicrobial Empyema Presenting as Massive Hydropneumothorax.

Aspiration-related pneumonia can progress to empyema and massive hydropneumothorax, potentially fatal complications not frequently documented in the literature. Patients with structural lung disease and poor dentition can be particularly vulnerable to severe polymicrobial infections arising from seemingly minor aspiration events. We report the case of a man in his early 70s with chronic bronchiectasis, heart failure with reduced ejection fraction, coronary artery disease status post coronary artery bypass grafting, type 2 diabetes mellitus, chronic kidney disease, and poor dentition who presen

Bronchiectasis as the Sentinel Manifestation of ANCA-Associated Vasculitis: A Case Report and Literature Review.

Bronchiectasis in ANCA-associated vasculitis (AAV) is usually attributed to structural damage from established disease. We report a 74-year-old woman with 7 years of culture-negative bronchiectasis, incompletely responsive to corticosteroids, in whom AAV was diagnosed after she developed palpable purpura, haemoptysis, hematuria and acute kidney injury. MPO-ANCA was markedly elevated (250.2 U/mL; reference < 1.0 U/mL) with PR3-ANCA negative. Renal biopsy showed pauci-immune crescentic glomerulonephritis, Berden focal class (one of 14 glomeruli with a cellular crescent). Induction with intraveno

Longitudinal Impact of Pseudomonas aeruginosa Versus Haemophilus influenzae Colonization on Lung Function Decline in Children With Primary Ciliary Dyskinesia.

BACKGROUND: The association of Pseudomonas aeruginosa (PsA) and Haemophilus influenzae (HI) colonization with lung function in children with primary ciliary dyskinesia (PCD) remains insufficiently defined. We aimed to compare spirometric parameters between PsA- and HI-colonized children and evaluate longitudinal changes before, at, and after chronic colonization. METHODS: This retrospective cohort included patients aged ≥ 5 years with definite or highly probable PCD and chronic PsA or HI colonization. Chronic colonization was defined using repeated sputum cultures over 12 months. Pulmonary fu
