Pulmonary diseases such as malignancies, empyema, bronchiectasis, digestive tract malignancies, inflammatory bowel diseases, cyanotic congenital heart diseases and infective endocarditis can cause clubbing. We present a 63-year-old female patient with infective endocarditis, who had clubbing that resolved very rapidly after cardiac surgery due to rupture of the mitral papillary muscle. She had persistent fever and in her echocardiographic examination rupture of the papillary muscle of the anterior mitral valve and significant aortic regurgitation was noted. She was scheduled for emergency oper

PURPOSE: A case of spontaneous pneumothorax with a subsequent bronchopleural fistula (BPF) treated with endoscopically administered fibrin glue is presented.
SUMMARY: A 76-year-old white man with a history of a benign lung mass and chronic obstructive pulmonary disease was admitted to the hospital with right-sided, anterior, pleuritic chest pain for the past three days and shortness of breath at rest, which worsened during exertion. Initial chest radiograph revealed a right 95% spontaneous tension pneumothorax. A chest tube was immediately placed in the right pleural space, resulting in reinf

BACKGROUND: IgA deficiency (IGAD) is the most common primary antibody deficiency. Although many affected individuals have no apparent symptom, selected patients suffer from recurrent mucosal infections, allergies, and autoimmune diseases. We aimed to investigate the clinical features in relation to immune function of Iranian patients with symptomatic IGAD.
METHODS: Thirty-seven patients (21 male and 16 female), aged 4-32 years, were evaluated in this study. Patients were followed for a total of 131 patient years with a mean follow-up of 3.5 years per patient.
RESULTS: The most prevalent pres

Aspergilloma refers to a fungal ball formed by saprophytic overgrowth of Aspergillus species and is seen secondary to cavitatory/cystic respiratory diseases. Paucity of clinical and pathological data of aspergilloma in India prompted us to analyze cases of aspergilloma over 15 years. The clinical features were recorded in all and correlated with detailed pathological examination. Aspergillomas were identified in 41 surgical excisions or at autopsy. There was male predominance; half the patients were in their fourth decade. Episodic hemoptysis was the commonest mode of presentation (85.4%). For

We report the case of a patient with chronic obstructive pulmonary disease and bronchiectasis, chronically using corticosteroids, who acquired pulmonary nocardiosis, which presented as multiple cavitated nodules. The principal symptoms were fever, dyspnea and productive cough with purulent sputum. Chest X-ray and computed tomography of the chest revealed nodules, some of which were cavitated, in both lungs. Sputum smear microscopy and culture revealed the presence of Nocardia spp. The patient was treated with imipenem and cilastatin, which produced an excellent clinical response.

Common variable immunodeficiency (CVID) is a heterogeneous group of disorders characterized by hypogammaglobulinemia and increased susceptibility to recurrent pyogenic infections. This study was performed to subclassify CVID on the basis of the bactericidal antibody responses of patients to polysaccharide meningococcal vaccine. Twenty-five patients with CVID (18 male and 7 female) and 25 healthy volunteers received meningococcal polysaccharide vaccine A + C. Serum bactericidal antibody (SBA) titers were measured at baseline and after 3 weeks. Response was correlated with clinical and immunolog

Common variable immunodeficiency (CVID) is a heterogeneous group of disorders, characterized by hypogammaglobulinemia and increased susceptibility to recurrent infections, autoimmunity and malignancies. We have previously shown that some pediatric patients with CVID can respond to meningococcal polysaccharide vaccine. Twelve pediatric cases with CVID were re-evaluated to determine whether bactericidal antibody responses or IgM memory B-cells correlate with the severity of disease resulting from the deficiency. We found that bronchiectasis and clinical manifestations of autoimmunity occur more

OBJECTIVE: Mycobacteriosis is frequently diagnosed among HIV-infected patients. In Mozambique, where few patients are under antiretroviral therapy and the prevalence of tuberculosis is high, there is need for better characterization of mycobacteria at the species level, as well as for the identification of patterns of resistance to antituberculous drugs.
METHODS: We studied a sample of 503 HIV-infected individuals suspected of having pulmonary tuberculosis. Of those 503, 320 tested positive for mycobacteria through sputum smear microscopy or culture of bronchoalveolar lavage fluid.

BACKGROUND: The outcome of patients with bronchiectasis during and after their stay in the intensive care unit (ICU) has seldom been reported in the literature. Managing these patients in the ICU can be challenging because of the complex nature of their disease. This study aims to identify the in-hospital and long-term outcome of patients with bronchiectasis and respiratory failure (RF) in ICU.
METHODS: A retrospective study was carried out by studying all bronchiectatic patients admitted to the medical ICU for RF over a 10-year period (1995-2004).
RESULTS: The mean (+/- standard deviation)

BACKGROUND: Cough is a prominent symptom across a range of common chronic respiratory diseases and impacts considerably on patient health status.
METHODS: We undertook a cross-sectional comparison of scores from two cough-specific health-related quality of life (HRQoL) questionnaires, the Leicester Cough Questionnaire (LCQ), and the Cough Quality of Life Questionnaire (CQLQ), together with a generic HRQoL measure, the EuroQol. Questionnaires were administered to and spirometry performed on 147 outpatients with chronic cough (n = 83), COPD (n = 18), asthma (n = 20), and bronchiectasis (n = 26)

Chronic obstructive pulmonary disease (COPD) exacerbations have a major impact on patients with COPD, yet they are complex events that are associated with a number of triggers and affected by the underlying disease process. A number of conditions can mimic the symptoms of an exacerbation and require evaluation. Airway and systemic inflammatory changes at exacerbation are modulated by infective factors (viruses and bacteria) and lead to the pathophysiologic effects seen at exacerbations with increase in airflow obstruction. Although bacteria or viruses can be isolated at exacerbation, often the

Although amyloid infiltration of the thyroid gland is an uncommon but well-known phenomenon, the appearance of a goiter secondary to amyloid deposits is rare. The goiter enlarges rapidly and progressively, often becoming compressive like thyroid cancer. The diagnosis is rarely suggested clinically even in the presence of known amyloidosis. We describe the case of a 45-year-old patient who presented an amyloid goiter as the first manifestation of systemic amyloidosis, probably secondary to bronchiectasis.

BACKGROUND: Bronchiectasis remains a significant cause of morbidity among specific populations world wide, including many indigenous groups. Data on prevalence in Australian adults are lacking. Indigenous children in Central Australia have the highest rates of bronchiectasis in the world. Outcomes for these individuals after they become adults are not currently available.
METHODS: We performed a retrospective case review of the presentation and likely aetiology of adult patients presenting to the Alice Springs Hospital with a primary diagnosis of bronchiectasis.
RESULTS: Sixty-one patients a

This case report describes a geriatric patient suffering from infected bronchiectasis and severe denutrition. This pathology is common and its treatment clearly defined, but an adapted and specific therapeutic approach, consisting in a global assessment and geriatric multidisciplinary interventions should be performed because of the frailty of the patient. This approach has result in a favorable issue for the patient.

BACKGROUND: Chronic hypersensitivity pneumonitis (HP) eventually ensues to extensive lung fibrosis when exposure to the causative antigen continues. Differential diagnosis from idiopathic interstitial pneumonias is sometimes difficult especially in the advanced stage.
AIM OF THE WORK: To describe the clinical course of chronic summer-type HP, which is the most prevalent type of HP in Japan, in terms of early diagnosis and similarity to idiopathic pulmonary fibrosis (IPF).
METHODS: 14 patients with chronic summer-type HP diagnosed between 2000 and 2005 were reviewed retrospectively.

PURPOSE: Motion artifacts, which can mimic thickened bronchial wall and the cystic appearance of bronchiectasis, constitute a potential pitfall in the diagnosis of interstitial or bronchial disease. Therefore, purpose of our study was to evaluate whether 64-detector row CT (64-MDCT) enables a reduction in respiratory or cardiac motion artifacts in the lung area on thin-section CT without ECG gating, and to examine the correlation between cardiac motion artifact and heart rate.
MATERIALS AND METHODS: Thirty-two patients with suspected diffuse lung disease, who underwent both 8- and 64-MDCT (ga
