Association of hypothyroidism and respiratory diseases: A cross-sectional study among Europeans using Mendelian randomization.

To explore the causal relationship between hypothyroidism and various respiratory diseases through a 2-sample Mendelian randomization (MR) approach. The genetic summary data of hypothyroidism and pulmonary diseases were obtained from 2 genome-wide association studies. A series of quality control steps were carried out to select instrumental variables. We conducted 2 independent 2-sample MR analyses, including MR-Egger regression, weighted median, random-effects inverse variance weighted method, and weighted model 4 regression models, to evaluate the causal associations between hypothyroidism a

The importance of biomarkers in the treatable-trait approach to chronic airway diseases.

Chronic airway diseases (CAD), including asthma, chronic obstructive pulmonary disease (COPD), bronchiectasis, and cystic fibrosis, are increasingly recognized as heterogeneous and overlapping syndromes that share treatable biological and clinical characteristics. The Treatable Traits (TT) approach is a precision medicine framework that transcends diagnostic labels. It identifies and targets modifiable pulmonary, extrapulmonary, and behavioral characteristics in each patient. Biomarkers are central to this paradigm, translating latent endotypes into measurable traits that inform diagnosis, tre

Cardiac autonomic function in bronchiectasis and age and gender-matched healthy participants: case-control study.

This study aims to evaluate and compare cardiac autonomic function, specifically through heart rate variability (HRV), between individuals with bronchiectasis and their age- and gender-matched healthy counterparts. This study employed a case-control design, involving 60 participants diagnosed with bronchiectasis (cases) and a control group of healthy individuals matched by age and gender. HRV data was collected over a five-minute interval, focusing on frequency domain parameters including total power, very low frequency (VLF), low frequency (LF), high frequency (HF), and the LF/HF ratio. The b

Genetically predicted associations between blood cell perturbation responses and bronchiectasis through immune mediation: A Mendelian Randomization study.

BACKGROUND: Bronchiectasis is a chronic airway disease characterized by persistent inflammation and structural damage, with substantial clinical and etiologic heterogeneity. Although previous studies have identified associations between blood cells and bronchiectasis, the causal relationships remain unclear. Moreover, the mechanisms underlying blood cell perturbation responses and their potential mediation by immune cells in disease progression are largely unexplored. METHODS: Two-sample Mendelian randomization (MR) analysis was used to explore genetically predicted associations among immune

Specialty-led inpatient chronic obstructive pulmonary disease management and mortality: a propensity-weighted cohort study.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is a major cause of hospitalization and mortality. Outcomes may vary by admitting specialty due to differences in expertise and adherence to evidence-based care. This study assessed whether specialty-led management influences inpatient outcomes for COPD exacerbations after adjusting for selection bias using propensity weighting. METHODS: This retrospective cohort study was conducted for adults aged ≥40 years admitted with COPD exacerbations between January 2017 and March 2025. Patients with asthma, bronchiectasis, or direct intensive ca

Brensocatib-Another Therapeutic "Window of Opportunity" for Patients with Bronchiectasis.

Introduction: Bronchiectasis is a chronic, heterogeneous airway disease characterised by irreversible bronchial dilatation, recurrent infections, and persistent inflammation, leading to progressive lung damage, frequent exacerbations, and impaired quality of life. Neutrophil-driven inflammation, largely mediated by excessive activity of neutrophil serine proteases such as neutrophil elastase, represents a central pathogenic mechanism and an important therapeutic target. Methods: Brensocatib, a first-in-class, selective, and reversible inhibitor of dipeptidyl peptidase-1 (DPP-1), prevents the a

Successful use of high-flow nasal cannula to treat hemoptysis and acute global respiratory failure in two cystic fibrosis bronchiectasis patients and review of the current literature.

Cystic fibrosis (CF) is a multisystemic disease, and despite the continuous improvement in understanding its pathophysiology and the present therapeutic possibilities, complications may occur. Indeed, literature shows that the onset of hemoptysis is a common complication reported in CF patients: in this context, the high-flow nasal cannula (HFNC) respiratory support has recently gained more importance because it helps increase the hygroscopic degree in the bronchial secretions by improving ciliary function and mucus hydration; thus, it reduces the patients' work of breathing, and through high

Pediatric Bronchiectasis Quality-of-Life Questionnaire-What Does the Literature Say?

INTRODUCTION: Bronchiectasis in children is a neglected chronic suppurative lung disease that imposes a substantial burden on children and adolescents and their families, adversely affecting quality of life (QoL). Incorporating health-related quality of life (HRQoL) tools into routine care can improve outcomes by highlighting disease burden and guiding tailored interventions. This scoping review aimed to identify self-reported questionnaires assessing QoL in pediatric bronchiectasis (PB). METHODS: Following the PRISMA extension for scoping reviews, we searched Embase, CINAHL, MEDLINE, Cochran

Comparison of chest CT-derived muscle mass and body mass index in assessing disease severity and outcomes in non-cystic fibrosis bronchiectasis.

PURPOSE: While the association of body mass index (BMI) with bronchiectasis is recognized, the role of body muscle mass are not well understood, especially compared to BMI. METHODS: From January 2017 to June 2020, subjects with diagnoses of bronchiectasis, confirmed by chest computed tomography (CT), were enrolled. Muscle mass was quantified using the muscle area of a single slice of CT at the first lumbar vertebrae, corrected by height squared (L1MI). Subjects were categorized into normal and low L1MI groups based on sex-specific criteria. Low BMI was defined as BMI less than 18.5 kg/m2. RESU

Comparison of clinical and microbiological features between mucoid and non-mucoid Pseudomonas aeruginosa in non-cystic fibrosis bronchiectasis.

Mucoid Pseudomonas aeruginosa is frequently isolated from patients with non-cystic fibrosis bronchiectasis (NCFB), yet the clinical and microbiological consequences of this phenotype remain unclear. In this retrospective study, we analyzed 66 patients with NCFB from whom P. aeruginosa was isolated between 2021 and 2024 to compare mucoid and non-mucoid isolates. Among the patients, 32 harbored mucoid and 34 non-mucoid isolates. Compared with patients with non-mucoid isolates, those with mucoid isolates had poorer lung function (FEV1 % predicted: 52.3 ± 15.7 vs 63.8 ± 17.2; P = 0.006), more freq

Bronchiectasis in a child with a homozygous DCDC2 gene mutation: A case report.

The DCDC2 (Doublecortin Domain-Containing Protein 2) gene encodes a ciliary protein involved in microtubule polymerization and ciliary assembly. Although DCDC2 has been associated primarily with neuronal migration and ciliopathies affecting the liver and kidneys, its role in pulmonary disease has not been clearly defined. Ciliary dysfunction, particularly in motile cilia of the respiratory tract, can impair mucociliary clearance and lead to bronchiectasis, as seen in primary ciliary dyskinesia. Here, we report a rare case of bronchiectasis associated with a pathogenic DCDC2 (NM_016356.5):c.558

When Neutrophils Go Rogue: Dysregulated Inflammation in Airway Diseases and Implications for Therapy.

Failure to resolve the neutrophilic inflammation during the innate immune response results in neutrophil accumulation in the airways. These cells exhibit prolonged survival, increased necrosis, and impaired phagocytic capacity. Paradoxically, despite reduced pathogen-clearing ability, neutrophils release serine proteases that drive the destruction of airway tissue. Their persistence in the lung chronically promotes a cycle of tissue damage, remodeling, and inflammation, leading to progressive pulmonary dysfunction. Recent discoveries in neutrophil dysregulation open up the possibility of devel

Mucus as a Treatable Trait in Chronic Airway Diseases.

Chronic airway diseases, including asthma, chronic obstructive pulmonary disease, bronchiectasis, and cystic fibrosis, are increasingly recognized as heterogeneous disorders characterized by overlapping pathophysiological mechanisms. Among these, abnormalities in mucus production, composition, and clearance have been identified as clinically significant contributors to symptoms, airflow limitation, exacerbations, and disease progression. Within the "treatable traits" framework, mucus-related abnormalities represent a distinct, modifiable phenotype that supports personalized management strategi
