Cerebral amyloid angiopathy-related inflammation superimposed on alpha-synucleinopathies: a case report and literature review.

INTRODUCTION: Cerebral amyloid angiopathy-related inflammation (CAAri) is a rare CAA variant characterized by an autoinflammatory response against beta-amyloid and subacute neurological presentation. While biologically plausible, given the frequent co-presence of beta-amyloidosis in synucleinopathies, no documented cases of CAAri superimposed on a dementia with Lewy bodies (DLB) have been previously described. METHODS: We presented a case of prodromal DLB, which subsequently manifested a clinical and radiological syndrome consistent with a CAAri. A literature review on case reports of CAAri o

Clinical observation and electroretinogram analysis of ocular siderosis following ocular trauma: a retrospective case series.

BACKGROUND: To observe demographics and clinical characteristics of patients with ocular siderosis and to further analyze corresponding changes in electroretinogram (ERG) findings. METHODS: The demographics and clinical characteristics from patients with ocular siderosis were collected and summarized, including sex, age, disease duration, visual acuity, and intraocular pressure. ERG parameters, including a-wave and b-wave latencies in photopic ERG and both amplitude and latency of b-wave in scotopic ERG, were collected. Paired comparisons were conducted to verify the difference between the af

Compression Post-Extravasation of Parenteral Iron May Reduce Skin Staining.

We describe four cases of iron extravasation injuries presenting to a dermatology clinic, where compression applied post-infusion appeared to reduce or prevent cutaneous siderosis. In each case, distinct patterns of hyperpigmentation were observed, with areas of compression from bandages or dressings showing reduced staining. Iron extravasation, a complication of intravenous iron therapy, can cause permanent cosmetic damage and distress. Current treatment options for reversing staining are limited, costly and require multiple sessions. This case series suggests that compression may be a simple

In Vivo Imaging of Blood-Brain Barrier Leakage Using a Contrast Agent in Patients With Cerebral Amyloid Angiopathy: An Exploratory Study.

BACKGROUND AND OBJECTIVES: Blood-brain barrier (BBB) leakage may be an early step in the pathophysiology of cerebral amyloid angiopathy (CAA), possibly preceding hemorrhages. This exploratory study measured BBB leakage in vivo at the level of the leptomeningeal and small parenchymal vessels in patients with probable CAA. We hypothesized that BBB leakage from leptomeningeal and cortical small vessels would be higher in patients with CAA compared with controls and that leakage would be associated with hemorrhagic manifestations of CAA, that is, cortical superficial siderosis (cSS) and lobar cere

To select or not to select: evaluating the effectiveness of SeLECT score in predicting the risk of post-stroke seizures: a systematic review.

BACKGROUND: SeLECT score identifies patients at high risk of post-stroke epilepsy(PSE). This systematic review summarizes the effectiveness of SeLECT and its modified versions in predicting PSE risk. METHODS: A systematic search was done from inception to 21st July 2025. Studies using SeLECT score to predict the risk of PSE were included. Two authors independently screened and collected data. Due to the heterogeneity of the studies, a meta-analysis could not be done. RESULTS: Of 3079 articles, 15 were included. Over 10,000 S patients from eleven countries were studied. The original SeLECT sc

Higher Burden of Cerebral Small Vessel Disease Is Associated With Risk of Incident Stroke in Community-Dwelling Individuals.

BACKGROUND: Mild manifestations of individual cerebral small vessel disease (CSVD) markers are common and may not denote increased risk, but high CSVD burden identifies individuals at increased risk of stroke and dementia. Scores incorporating multiple individual CSVD markers may better identify a person's risk. We related a multimarker CSVD score to risk of incident stroke and compared it with the Framingham Stroke Risk Profile (FSRP) in community-dwelling individuals. METHODS: Framingham Heart Study participants aged ≥55 years, free of stroke and dementia, and with brain magnetic resonance

Neuroimaging Markers Associated With Recurrent Stroke in Intracerebral Hemorrhage and Atrial Fibrillation: Secondary Analysis of PRESTIGE-AF.

BACKGROUND AND OBJECTIVES: Anticoagulation in patients with intracerebral hemorrhage (ICH) and atrial fibrillation reduces the risk of ischemic stroke (IS), but also potentially causes an excess risk of recurrent ICH. In this context, we assessed the role of neuroimaging in identifying patients with particular susceptibility to IS or ICH, potentially allowing for individualized risk stratification. METHODS: In PRESTIGE-AF, a prospective, randomized clinical trial conducted across 75 hospitals in 6 European countries, participants with spontaneous ICH and atrial fibrillation were randomized to

Iatrogenic cerebral amyloid angiopathy with inflammatory pathogenesis: A case report and literature review.

BACKGROUND: Iatrogenic cerebral amyloid angiopathy (iCAA) is a rare form of amyloid β (Aβ)-mediated small vessel disease associated with prior neurosurgical exposure, such as cadaveric dura mater grafting or other Aβ-contaminated materials. The condition generally manifests decades later, presenting as intracerebral hemorrhage (ICH), seizures, or cognitive decline. While the presence of inflammatory forms of sporadic cerebral amyloid angiopathy (CAA) is well-documented, an inflammatory variant of iCAA has not been previously documented. The aims of this report are to present the first case of

White Matter Free Water and PSMD as Neuroimaging Biomarkers of Cerebral Amyloid Angiopathy Severity.

BACKGROUND AND OBJECTIVES: Cerebral amyloid angiopathy (CAA) is a cerebral small vessel disease (cSVD) characterized by β-amyloid deposition in cortical and leptomeningeal vessels. Free water (FW) and peak width of skeletonized mean diffusivity (PSMD) are diffusion-weighted MRI biomarkers of cSVD. Although PSMD has been associated with CAA, it is unknown whether FW reflects CAA-related tissue injury. We investigated whether FW is elevated in CAA compared with non-CAA participants with cognitive complaints and assessed FW's neuroimaging and cognitive associations in CAA compared with PSMD. MET

Profiles of blood-brain barrier and neurodegeneration markers in cerebrospinal fluid of patients with cerebral amyloid angiopathy.

INTRODUCTION: There are few studies analyzing cerebrospinal fluid (CSF) in patients with cerebral amyloid angiopathy (CAA). Our goal was to compare blood-brain barrier and neurodegeneration markers in CSF in CAA patients with and without hemorrhagic markers. METHODS: In a retrospective study of patients with CAA (Boston criteria version 2.0) identified from the Aachen Memory Database and from in-hospital admission records, we compared CSF neurodegeneration markers and albumin ratio (a blood-brain barrier permeability marker) in patients with and without hemorrhagic markers. RESULTS: Among 37

Cognitive Age Delta as a Marker of Healthy and Pathological Cognitive Aging: The Role of Lifestyle, Cognitive Reserve, and Vascular Risk.

Background: Chronological age is an imprecise proxy for cognitive aging. The Cognitive Age Delta (CAD)-the difference between predicted cognitive age and chronological age-offers a scalable, individualized marker of functional brain aging. We examined determinants of CAD in cognitively unimpaired (CU) adults stratified by Alzheimer's disease (AD) and vascular biomarkers. Methods: We analyzed 177 CU participants from the Gipuzkoa Alzheimer Project (Basque Country, Northern Spain) classified as amyloid-negative/vascular-negative (CUA-V-, n = 140), amyloid-positive (CUA+, n = 23), or vascular-pos

Pathological findings from surgical specimens and long-term outcome in cerebral amyloid angiopathy patients with intracerebral hemorrhage.

Sporadic cerebral amyloid angiopathy (CAA) is a small-vessel disease. We sought to assess pathological findings and outcomes in CAA patients with non-traumatic intracerebral lobe hemorrhage (ICH). Sixty-three CAA-ICH patients underwent Hematoxylin-eosin, amyloid-β, smooth muscle actin, and CD34 staining. Arterioles were graded using a CAA-severity scale. Prospective ≥ 12-month follow-up identified prognostic factors via Spearman correlation analysis and binary logistic regression. Cox proportional hazard regression models assessed mortality or recurrent ICH risk associations. Among 93 Chinese

Biopsy-Confirmed Cerebral Amyloid Angiopathy-Related Inflammation With Absence of Cerebral Microbleeds on T2* and Detectable Only by Susceptibility-Weighted Imaging.

Cerebral amyloid angiopathy-related inflammation (CAA-ri) is a rare but treatable inflammatory condition in which there is amyloid-beta deposition in cortical and leptomeningeal vessels. Patients with CAA-ri often present with a high number of cerebral microbleeds (CMBs). Susceptibility-weighted imaging (SWI) is more sensitive than T2* for detecting these CMBs. However, SWI is not currently mandatory for the diagnosis of CAA-ri. Here, we present the case of an 82-year-old man with worsening confusion and bilateral visual loss. MRI revealed no CMBs on T2*, but a small number were detected using
