Posterior confluent white matter hyperintensities and intracerebral hemorrhage recurrence risk in patients with cerebral amyloid angiopathy.

BACKGROUND: Cerebral amyloid angiopathy (CAA) is associated with a high risk of recurrence of intracerebral hemorrhage (ICH). This study aimed to identify risk factors of CAA-related ICH recurrence, especially focused on CAA-characteristic neuroimaging markers. METHODS: Consecutive survivors of spontaneous lobar ICH for probable CAA, possible CAA, and mixed cerebral small vessel disease (CSVD) were enrolled at Tianjin Huanhu Hospital between 2017 and 2024. Baseline clinical data and magnetic resonance imaging (MRI) findings were collected. Posterior confluent white matter hyperintensities (WM

Push pin induced artifact: A mimic of iron pill injury in gastrointestinal pathology.

BACKGROUND: Dissection push pins (PP) are commonly used during formalin fixation to secure gastrointestinal (GI) resection specimens, allowing proper specimen fixation. We have observed iron diffusion from the PPs into the tissues that results in an artifact that can be confused with iron deposition described with iron pill associated mucosal injury and gastric glandular siderosis. We sought to characterize this tissue artifact in GI resection specimens. METHODS: Sections from 20 GI specimens (colon 11, small intestine 5, stomach 3, esophagus 1) were obtained from areas where PPs were applied

Giant cauda equina neuroendocrine tumor with superficial siderosis: illustrative case.

BACKGROUND: Cauda equina neuroendocrine tumor (CENET) is a rare spinal neuroendocrine tumor, and its coexistence with superficial siderosis (SS) is even more rare. CENETs are usually small and most commonly located below L2, and their imaging often overlaps with that of myxopapillary ependymomas or schwannomas, thus complicating their preoperative differentiation. The coexistence of SS and intratumoral fluid-fluid level (FFL) in CENET has not been documented. OBSERVATIONS: The authors describe a giant thoracolumbar CENET (T12-L3) with intracranial SS in a patient with progressive paraparesis

Cardiovascular Complications in Hemoglobinopathies: Pathophysiology, Clinical Spectrum, and Management.

Sickle cell disease and thalassemia syndromes are increasingly recognized as chronic multisystem disorders, with cardiovascular (CV) sequelae representing a significant proportion of long-term morbidity and mortality. Survival has improved considerably due to better transfusion practice, the administration of iron chelation, and disease-modifying treatments; a broad and expanding phenotypic spectrum of cardiac and vascular manifestations is being recognized. These include cardiomyopathy, heart failure, pulmonary hypertension, arrhythmias, microvascular ischemia, and functional valvular disease

MRI evaluation of cerebral perivascular spaces predicts amyloid-related imaging abnormalities risk in preclinical Alzheimer's disease.

BACKGROUND AND PURPOSE: Amyloid-related imaging abnormalities (ARIA) are radiographic findings observed in the natural course of Alzheimer's disease and have been reported at higher rates in patients receiving anti-amyloid monoclonal antibody therapy. Identifying novel radiographic factors predicting ARIA risk may help prevent its occurrence, improve patient stratification, and provide insight on the underlying biological mechanisms. It remains unclear whether cerebral perivascular spaces (PVS) along with other quantitative radiographic markers of cerebral small vessel disease may help predict

Cerebrospinal Fluid Amyloid-β Biomarkers Predict Future Hemorrhage in Patients with Cerebral Amyloid Angiopathy.

OBJECTIVE: Accurately predicting future hemorrhagic events in patients with cerebral amyloid angiopathy (CAA) remains a major clinical challenge. It is unknown whether cerebrospinal fluid (CSF) biomarkers of amyloid-beta (Aβ) pathology are associated with increased hemorrhage risk in this population. METHODS: We analyzed consecutive patients meeting Boston criteria version 2.0 for probable CAA with CSF Aβ data obtained during diagnostic workup. The primary outcome was incident intracranial hemorrhage, including lobar intracerebral, convexity subarachnoid, and non-traumatic subdural hemorrhage

Contrasting patterns of leptomeningeal and parenchymal gadolinium extravasation in cerebral amyloid angiopathy: An MRI-based evaluation.

BACKGROUND: Previous studies suggest that blood-brain barrier (BBB) disruption may play a role in the pathophysiology of vessel rupture in cerebral amyloid angiopathy (CAA). Here, in a cross-sectional cohort study, we apply contrast-enhanced 3 Tesla magnetic resonance imaging (MRI) to test the hypothesis that the BBB is damaged in patients with CAA and to determine whether BBB leakage is associated with hemorrhagic brain injury in CAA. METHODS: Parenchymal BBB leakage rate (Ki) was assessed in the cortex and white matter with dynamic contrast-enhanced (DCE)-MRI and quantified with pharmacokin

Abducens nerve palsy as a false localising sign in spontaneous intracranial hypotension.

Spontaneous intracranial hypotension (SIH) with the formation of a spinal longitudinal epidural collection (SLEC) is an important treatable cause of headache. Left untreated, neurological sequelae can include cranial neuropathy, superficial siderosis and brain-sagging dementia. We describe a male in his late 40s presenting with orthostatic low-pressure headache and binocular horizontal diplopia due to an isolated left abducens nerve palsy, a recognised false localising sign. Imaging revealed intracranial hypotension associated with a SLEC, attributed to a cervical osteophyte associated dural t

Utility of multi-echo MRI for differentiating neonatal hemochromatosis from other causes of neonatal liver failure.

OBJECTIVE: To assess whether multi-echo T2* MRI can differentiate neonatal hemochromatosis (NH) from other causes of neonatal liver failure (NLF) by evaluating siderosis in the liver and extrahepatic organs. MATERIALS AND METHODS: This retrospective study evaluated 26 infants ≤ 20 weeks of age, who underwent MRI with Gradient-Recalled (GRE) multi-echo T2* sequence for liver failure. Two radiologists noted the degree of siderosis in the liver, pancreas, thyroid, spleen and renal cortex using a semiquantitative scale. Kendall's tau was used to assess the correlation between siderosis grades and

Drug-Induced Amyloid-Related Imaging Abnormalities: A Neurovascular Perspective on Risk Assessment.

BACKGROUND: Anti-amyloid therapies (AAT) are reshaping the therapeutic landscape of Alzheimer's disease (AD), yet their implementation remains constrained by the risk of amyloid-related imaging abnormalities (ARIA). Although the ARIA phenomenon is well recognized, most available evidence stems from clinical trial safety reports framed predominantly from a dementia-oriented perspective, with relatively limited integration of vascular neurology principles. METHODS: In this narrative review, we examine drug-induced ARIA through a neurovascular lens, highlighting how cerebrovascular comorbidity,

[Spontaneous intracranial hypotension - a spinal disease].

BACKGROUND: Spontaneous intracranial hypotension (SIH) remains an underdiagnosed condition despite increasing awareness due to recent scientific advances. Diagnosis can be delayed by the broad clinical presentation and imaging pitfalls. This results in a high degree of physical impairment for patients, including social and psychological sequelae as well as long-term damage in the case of delayed diagnosis and treatment. METHOD: The study is based on a selective literature search on PubMed including articles from 1990 to 2023 and the authors' clinical experience from working in a CSF center.

Hemorrhage Pattern and Amyloid Burden in Cerebral Amyloid Angiopathy.

BACKGROUND: Cerebral amyloid angiopathy (CAA), a cerebral small vessel disease characterized by vascular amyloid deposition, presents with heterogeneous imaging features, but the biological mechanisms underlying hemorrhagic markers remain unclear. We assessed hemorrhage patterns in CAA to determine their associations with amyloid burden and related imaging markers. METHODS: Sixty-two patients with probable CAA underwent Pittsburgh compound B-positron emission tomography and structural magnetic resonance imaging. Participants were classified by dominant hemorrhagic pattern: lobar cerebral micr

QT and corrected QT prolongation as early markers of cardiac iron overload in pediatric transfusion-dependent thalassemia: A systematic review and meta-analysis.

Transfusion-dependent thalassemia (TDT) is a major health problem across the thalassemia belt. Although regular transfusions extend survival, they cause cardiac iron overload (CIO). Cardiac magnetic resonance imaging T2-star (MRI T2*) is the gold standard for detecting myocardial siderosis, but it remains costly and has limited availability. QT and corrected QT (QTc) intervals have been proposed as simple, inexpensive alternatives, yet pediatric evidence is limited. This systematic review aims to assess the association between QT and QTc prolongation and CIO in pediatric TDT. We conducted a sy

Observed frequency of increased iron indices and associated factors in welders.

Increased iron indices among welders are underrecognized. This study investigated their frequency and associated factors in an observational cross-sectional study including 55 male welders. Increased iron indices were defined as serum ferritin >400 µg/L and/or TS ≥45%. Standard statistical and regression analyses were used. Increased iron indices were identified in 25.5% of welders. Compared to those with normal indices, affected welders had longer employment duration. Iron parameters were significantly higher in the increased group (serum iron 139.6 vs. 93.3 µg/dL, ferritin 498.9 vs. 142.3 µg

Temporal Evolution of ARIA After Lecanemab Interruption: A Longitudinal Volumetric MRI Study.

BACKGROUND AND PURPOSE: The temporal evolution of amyloid-related imaging abnormalities (ARIA) after lecanemab interruption is incompletely characterized. We quantified longitudinal ARIA-E (vasogenic edema and/or sulcal effusion) volumetric change and associated ARIA-H (cerebral microhemorrhages and/or superficial siderosis) on follow-up MRI, including assessment across APOE genotype groups. MATERIALS AND METHODS: In this retrospective study at a tertiary academic center, we identified patients who developed ARIA-E during lecanemab therapy and had longitudinal brain MRI exams for analysis. Characterisation of patients with intracerebral haemorrhage with mixed location haemorrhages: clinical and imaging features, CSF neurodegeneration markers and recurrence risk.

INTRODUCTION: The most frequent causes of ICH include arteriolosclerotic small vessel disease (aSVD-ICH) and cerebral amyloid angiopathy (CAA-ICH). The predominant underlying pathology and natural history of ICH patients with mixed deep and lobar location haemorrhages (MLH-ICH) remain insufficiently studied. PATIENTS AND METHODS: We performed a retrospective analysis of consecutive patients with ICH admitted between 2018 and 2024 and selected patients without macrostructural cause of haemorrhage, with available magnetic resonance imaging. We compared clinical characteristics, imaging markers,

Spinal CSF-Venous Fistulas as the Potential Cause of Chronic Subarachnoid Hemorrhage: Superficial Siderosis and Adhesive Arachnoiditis.

BACKGROUND AND PURPOSE: Spontaneous spinal CSF-venous fistulas were first described in 2014 and much remains unknown about this common cause of intracranial hypotension, including their basic pathophysiology. One point of contention is whether there is 1-way or 2-way flow through these fistulas. The purpose of the present study was to test the hypothesis of bidirectional flow in spinal CSF-venous fistulas and we asked the following 2 questions: 1) does xanthochromia, which indicates bleeding into the subarachnoid space, resolve after treatment of spinal CSF-venous fistulas and 2) is the progre
