A 71-year-old man presented for a routine physical examination with multiple comorbidities, including severe panvascular disease and valvulopathy, requiring anticoagulation therapy. He had a history of chronic hemolytic anemia and had been taking oral ferrous sulfate for two years. Upper gastrointestinal endoscopy (UGE) was performed, as part of the study of the persist anemia, revealing an extensive nodular area with multiple brownish deposits and spontaneous hemorrhage. Biopsy showed mild inflammation in the lamina propria and ferric pigment confirmed by positive Perl's iron stain, suggestin

Iron is the most common nutritional deficiency encountered in the United States, with over 15% of Americans using some form of daily oral iron supplementation. Although commonly associated with minor gastrointestinal side effects, severe gastrointestinal complications are rare. Direct cytotoxic mucosal damage and subsequent ulcer formation have rarely been reported. Among reported cases, most are found in the stomach with extragastric manifestations of siderosis being less commonly described. We report a rare case of esophageal siderosis related to oral iron supplementation.

Cerebral amyloid angiopathy (CAA) is one of the most prevalent small vessel diseases (SVDs). Its neuroradiological hallmarks are both hemorrhagic and non-hemorrhagic ones. Among the clinical manifestations, transient focal neurological episodes (TFNEs) are associated with an increased risk of bleeding in a short time period and with convexal subarachnoid hemorrhage (SAH). The natural history of CAA is incompletely characterized in the literature, because the focus has been mostly on hemorrhagic events, while both clinical and non-hemorrhagic presentations are possible and sometimes underestima

BACKGROUND AND OBJECTIVES: Amyloid-related imaging abnormalities (ARIA) are key safety considerations in anti-amyloid-β (Aβ) immunotherapy. ARIA can be categorized into 2 types: ARIA characterized by edema and effusion (ARIA-E) or microhemorrhages and superficial siderosis (ARIA-H). In this study, we assessed the incidence of ARIA in phase 3 randomized controlled trials (RCTs) of anti-Aβ immunotherapy and compared the incidence among different agents and APOE ε4 carrier status.
METHODS: PubMed and Embase databases were searched for phase 3 RCTs of anti-Aβ immunotherapy published on or before

Anti-amyloid monoclonal antibodies (MABs) have introduced a new era of Alzheimer's disease (AD) therapeutics with disease-targeted drugs. Three agents --- aducanumab, lecanemab, donanemab --- have been approved and others are in development. These agents are administered intravenously, once in the brain they activate microglia to engulf amyloid-beta protein fibrillar plaques. Each approved agent has a specific profile of administration, titration, amyloid target, and adverse events. In 18 month trials of participants with early AD (defined as mild cognitive impairment due to AD or mild AD deme

BACKGROUND: Amyloid plaque removal by monoclonal antibody therapies slows clinical progression in symptomatic Alzheimer's disease; however, the potential for delaying the onset of clinical symptoms in asymptomatic people is unknown. The Dominantly Inherited Alzheimer Network Trials Unit (DIAN-TU) is an ongoing platform trial assessing the safety and efficacy of multiple investigational products in participants with dominantly inherited Alzheimer's disease (DIAD). Based on findings of amyloid removal and downstream biological effects from the gantenerumab group of the platform trial, we continu

Donanemab (Kisunla®), an IgG1 monoclonal antibody targeting N-terminal pyroglutamate-modified forms of amyloid-β, is approved in the United States for treatment of early symptomatic Alzheimer's disease (AD). Appropriate Use Recommendations (AUR) were developed to guide the implementation of donanemab in real-world practice, prioritizing safety considerations and opportunity for effectiveness. The AUR were developed by the AD and Related Disorders Therapeutic Workgroup by consensus, integrating available data and expert opinion. Appropriate candidates for donanemab treatment include persons wit

PURPOSE OF REVIEW: Cerebral amyloid angiopathy (CAA) is a disease of the cerebral vasculature that can result in microhemorrhages, as well as intraparenchymal and subarachnoid hemorrhage, superficial siderosis (SS), and/or secondary infarct/inflammation. CAA may be encountered as an isolated pathology or with Alzheimer disease and has been demonstrated to be associated with an increased risk of seizures. However, the overall rates of seizures and specific pathologies related to CAA and their subsequent risk of seizures have not been elucidated.
RECENT FINDINGS: Prior studies of CAA and seizur

BACKGROUND AND OBJECTIVES: Knowledge on the short-term progression of cerebral amyloid angiopathy (CAA) is important for clinical practice and the design of clinical treatment trials. We investigated the 1-year progression of CAA-related MRI markers in sporadic (sCAA) and Dutch-type hereditary (D-CAA).
METHODS: Participants were included from 2 prospective cohort studies. 3T-MRI was performed at baseline and after 1 year. We assessed macrobleeds, cerebral microbleeds (CMBs), cortical superficial siderosis (cSS), convexity subarachnoid hemorrhages (cSAHs), white matter hyperintensities (WMH),

We report a case of a man in his 40s who presented with a swollen optic disc and a metallic intraocular foreign body (IOFB) impacted on the retinal surface of the same eye. The IOFB was removed by vitrectomy and the swelling of the optic nerve slowly resolved. Literature search revealed one previous report of a retained IOFB associated with optic disc swelling as part of an ocular siderosis case series. The possibility that the optic disc swelling in our patient was as a result of a local toxic reaction to the metallic foreign body is discussed.

INTRODUCTION: Parkinson's disease (PD) is a common neurodegenerative disorder, and its diagnosis remains a clinical one. Hence, the emergence of new symptoms warrants consideration of a new diagnostic work-up even at later disease stages. Important differential diagnoses include atypical or symptomatic parkinsonism, like multiple system atrophy, normal pressure hydrocephalus, or rarely, superficial siderosis (SS).
CASE PRESENTATION: A 66-year-old woman with longstanding PD presented to our clinic with a 4-year history of progressive ataxia and gait impairment. The emergence of a new symptom (

BACKGROUND: The significance between cerebral small vessel disease (CSVD) markers on MRI and clinical outcome in intracerebral hemorrhage (ICH) patients is unclear. This study aimed to explore the connection between CSVD markers and short-term outcome in ICH patients.
METHODS: We retrospectively included 318 ICH patients. Patients were categorized into unfavorable and favorable prognosis groups based on their 90-day modified Rankin Scale (mRS) score (mRS > 3 indicates unfavorable). We conducted an MRI-based assessment of CSVD markers and investigated the predictive power of individual markers

Superficial siderosis (SS) of the central nervous system is a rare chronic neurological disorder characterized by hemosiderin deposition over the subpial surface of the brain, cranial nerves, and spinal cord. This deposition can result from acute subarachnoid hemorrhage or from chronic or repeated hemorrhage, most often due to cerebral amyloid angiopathy and less commonly to cerebrospinal fluid (CSF) leaks, usually from ventral dural tears. Chronic microhemorrhages associated with spinal CSF leaks without ventral epidural CSF collections or meningoceles are exceedingly rare. Herewith, we descr

IMPORTANCE: As amyloid-targeted therapies have become commercially available, the monitoring of cerebral amyloid angiopathy (CAA), which is an important risk factor for amyloid-related imaging abnormalities, has received increasing attention. However, comprehensive evidence on the association between Alzheimer disease (AD) plasma biomarkers and various CAA imaging markers is still lacking.
OBJECTIVE: To examine the association of CAA imaging markers with downstream AD plasma biomarkers in relation to amyloid-β (Aβ) uptake on positron emission tomography (PET) and whether their interplay is as

IMPORTANCE: Two monoclonal antibodies targeting amyloid plaques, lecanemab and donanemab, have received traditional US Food and Drug Administration (FDA) approval for the treatment of early symptomatic Alzheimer disease (AD). The most significant adverse events associated with these therapies are infusion-related reactions and amyloid-related imaging abnormalities (ARIA) with edema/effusion (ARIA-E) and/or hemorrhage/hemosiderin deposition (ARIA-H). The feasibility and safety of providing these treatments in clinical practice is unclear.

The prevalence of cerebral amyloid angiopathy (CAA) has been shown to increase with age, with rates reported to be around 50-60% in individuals over 80 years old who have cognitive impairment. The disease often presents as spontaneous lobar intracerebral hemorrhage (ICH), which carries a high risk of recurrence, along with transient focal neurologic episodes (TFNE) and progressive cognitive decline, potentially leading to Alzheimer's disease (AD). In addition to ICH, neuroradiologic findings of CAA include cortical and subcortical microbleeds (MB), cortical subarachnoid hemorrhage (cSAH) and c
