Iatrogenic Cerebral Amyloid Angiopathy After Childhood Brain Surgery: Novel Findings of MRI and CT.

Background/Objectives: A subtype of cerebral amyloid angiopathy (CAA), iatrogenic CAA (iCAA), has been increasingly reported. iCAA occurs primarily in patients who underwent surgery during childhood and is caused by the prion-like propagation of amyloid beta. This subtype of CAA tends to develop at a younger age than age-related CAA, usually before the age of 55. After a latency period of 20-40 years following surgery, it manifests as lobar intracerebral hemorrhage (ICH), cognitive impairment, or transient focal neurological episodes. Between 2023 and 2024, we observed four cases of possible i

Unraveling the nexus: Sleep's role in ferroptosis and health.

Sleep is an important physiological process to maintain physiological health, which involves the regulation of many systems in the body, which is not only closely related to hormone balance, immune system and nervous system function in the body. Recent studies have emphasized the complex relationship between sleep and cell death mechanisms. Ferroptosis is a form of iron-dependent cell death, which is related to a variety of neurodegenerative diseases. Sleep plays an important role in regulating brain metabolism, immune function and overall intracellular homeostasis. New evidence suggests that

Clinical Characteristics, Outcomes, and Complications Associated With Delayed Diagnosis of Intraocular Foreign Body.

Purpose: To describe the clinical characteristics, outcomes, and complications in cases of intraocular foreign bodies (IOFBs) when the diagnosis is missed or delayed. Methods: This nonconsecutive case series was a retrospective multicenter study of adult patients with a delayed diagnosis of IOFB, defined as initially presenting elsewhere with a missed diagnosis or a delay of more than 24 hours to seek care. Results: Eighteen eyes were included. The mean (±SD) presenting logMAR visual acuity (VA) was 0.45 ± 0.70 (Snellen equivalent, 20/56). The most common presenting symptom was decreased visio

Clinical Features and Diagnosis of Spontaneous Intracranial Hypotension.

OBJECTIVE: This article reviews the current understanding of the varied clinical presentations of spontaneous intracranial hypotension and discusses strategies and limitations in diagnosing this complex syndrome, including approaches for medical practitioners outside of specialty centers. LATEST DEVELOPMENTS: Radiologic algorithms applied to brain MRI (eg, the Bern score) can help assess the probability of an underlying spinal CSF leak, but they do not replace good history-taking and clinical acumen. Brain MRI findings may evolve over time with or without leak-directed treatment. New techniqu

MRI protocols and sequences for amyloid-related imaging abnormalities monitoring in Alzheimer's disease patients treated with monoclonal antibodies.

PURPOSE OF REVIEW: This review provides an updated overview of amyloid-related imaging abnormalities (ARIA) associated with antiamyloid monoclonal antibodies (mAbs) in Alzheimer's disease (AD). Following regulatory approvals for both lecanemab and donanemab in the United States, and pending decisions in Europe, standardized understanding of ARIA definitions, risk factors, and optimal MRI surveillance is increasingly important to guide treatment and ensure safety. RECENT FINDINGS: ARIA, including vasogenic edema (ARIA-E) and microhemorrhages/siderosis (ARIA-H), are a frequent adverse event in

"Real-world" eligibility for anti-amyloid treatment in a tertiary memory clinic setting.

INTRODUCTION: The societal impact of anti-amyloid treatment (AAT) for Alzheimer's disease (AD) depends largely on patient eligibility. Estimates suggest that 1% to 18% of individuals with AD may qualify for AAT; however, data from everyday clinical practice remain limited. This study assessed AAT eligibility in patients at a tertiary memory clinic. METHODS: We included 1309 new patients (63 ± 8 years, 45% women, Mini-Mental State Examination [MMSE] 25 ± 5) who presented to the Alzheimer Center Amsterdam (2020-2022) for standardized diagnostic workup. Eligibility for AAT was based on lecanemab

Prediction Model to Optimize Long-Term Antithrombotic Therapy Using Covert Vascular Brain Injury and Clinical Features.

BACKGROUND: Defining the risk of developing major bleeding, especially intracranial hemorrhage (ICH), or ischemic stroke (IS) in patients receiving antithrombotic therapy is crucial. Existing risk prediction tools would inadequately assess the net clinical benefit of antithrombotic therapy. We aimed to develop novel risk scores incorporating covert vascular brain injury to personalize the risk assessment of major bleeding, ICH, and IS in patients receiving antithrombotic therapy. METHODS: The prospective, multicenter, observational study (BAT2 [Bleeding With Antithrombotic Therapy Study-2]) e

Monitoring of amyloid related imaging abnormalities: SWI vs T2*-GRE.

Amyloid-β-directed monoclonal antibody therapies may lead to amyloid-related imaging abnormalities (ARIA). Clinical trials that formed the basis for the ARIA radiographic severity grading scale adopted by the approved drugs' labels utilized T2* gradient recalled echo (T2*-GRE) images for ARIA-hemorrhagic (ARIA-H) assessment. Little is known about the application of susceptibility-weighted imaging (SWI) to ARIA-H assessment. We exploited comparative studies on the usage of SWI instead of 2D T2*-GRE and simulated the impact of SWI's higher sensitivity on the derived ARIA-H severity distribution

Outcomes of CT-Guided Targeted Epidural Patching for Lateral Dural Tears in Spontaneous Intracranial Hypotension: A Multicenter Retrospective Cohort Study.

BACKGROUND AND PURPOSE: Spontaneous intracranial hypotension (SIH) due to lateral dural tears with spinal longitudinal extradural fluid collections (SLECs) can cause disabling orthostatic headaches. While epidural patching is commonly used as first-line treatment, outcomes specific to lateral dural tears have not been well-characterized. We aimed to evaluate clinical and radiologic outcomes following CT-guided patching for lateral dural tears and assess whether anatomic or procedural factors-including the presence of a herniated arachnoid pouch, patch volume, material, or approach-influence tr

Clinical Management of Cerebral Amyloid Angiopathy.

Background: Cerebral amyloid angiopathy (CAA) represents a progressive cerebrovascular disorder, characterized by aberrant accumulation of beta-amyloid isoforms in cortical and leptomeningeal vessel walls of cerebrum and cerebellum. Methods: We sought to investigate the clinical manifestations, current different diagnostic tools, various therapeutic strategies and most uncommon subtypes of the disease. Results: The vast majority of CAA remains sporadic, with increasing prevalence with age and very frequent coexistence with Alzheimer's disease. Clinically, CAA can present with spontaneous lobar

Expansively splenic reflective foci: A case-based résumé.

Splenic siderotic foci are scar tissue composing freckles, usually less than 1 cm. They turn reflective after further deposition of calcium. These foci are also known as Gamna-Gandy bodies and are most encountered in severe and long-standing portal hypertensive - congestive splenomegaly. Herein, we present a case of an unusual sonographic depiction of reflective foci wholly embracing an average-sized spleen and imposing a clinical dispute.

Successful IVIG Treatment in Neonatal Hemochromatosis Without Extrahepatic Siderosis: A Case Report.

Background and Clinical Significance: Neonatal hemochromatosis is a rare iron overload disorder that causes severe liver injury in newborns, typically with extrahepatic siderosis. Diagnosis of neonatal hemochromatosis is usually confirmed through a biopsy and MRI, demonstrating deposition of iron and liver failure. However, in severe patients who are not able to undergo biopsy, the diagnostic and management method remains unknown. Case Presentation: We present an unusual case of neonatal hemochromatosis without extrahepatic iron deposition in a 9-day-old male who showed signs of liver failure

Spinal Myxopapillary Ependymoma Presenting With Isolated Intracranial Hypertension: A Case Report.

Myxopapillary ependymomas (MPEs) are rare, slow-growing spinal tumors that arise from the filum terminale and are classified as World Health Organization (WHO) Grade 2 tumors. They typically present with features of lower spinal cord or cauda equina compression, such as radiculopathy, limb weakness, or sphincter dysfunction. Presentation with features of increased intracranial pressure (IIP) is rare and has been reported in a few isolated cases. We present a case of a 21-year-old woman who presented with a two-week history of worsening headaches, neck stiffness, nausea, double vision, and tinn
