Immune regulation by CD55 in chronic beryllium disease.

CD55 is an immune regulator that inhibits T-cell activation and also binds to CD97, a molecule involved in immune cell migration and signaling. While CD55 expression is reduced in chronic beryllium disease (CBD), its functional role in disease pathogenesis remains unclear. We hypothesized that CD55 downregulation in peripheral blood mononuclear cells (PBMCs) contributes to heightened beryllium (Be)-specific immune responses in CBD. To test this, we characterized CD55 expression and function in PBMCs from patients with CBD and individuals with beryllium sensitization (BeS), as well as in a huma

New aspects of chronic beryllium disease.

PURPOSE OF REVIEW: Chronic beryllium disease (CBD) is a granulomatous lung disorder resulting from a cell-mediated immune response to beryllium exposure. Despite exposure regulations, new cases continue to emerge across both traditional and nontraditional occupational settings. In this review, we underscore ongoing diagnostic and preventive challenges, and strategies for screening and treatment of beryllium exposed patients. RECENT FINDINGS: Recent updates to exposure standards and medical surveillance have broadened recognition of at-risk populations, including military personnel and constru

Mortality among workers at the Rocky Flats Plant, 1951-2017.

The Rocky Flats (RFs) Plant operated from 1951-1989 as part of the U.S. Department of Energy (DOE) nuclear complex. Its primary mission was weapons component fabrication, whereby workers were potentially exposed to radioactive and non-radioactive hazards. RF worker mortality was compared to the general population, and dose-response relationships between mortality and radiation organ doses were examined. RF workers first employed between 1951 and 1979 for ⩾30 d were identified (n= 9397). Vital status was determined using national and state death records up to 2017. Organ doses from external pho

Reduced CCL/Be-specific CD4+ T cells in CCL3-deficient or peptide-MHCII CAR-T cell-treated mice.

In chronic beryllium disease (CBD), elevated levels of the inflammatory chemokines CCL3 and CCL4 in the lungs coincide with expanded populations of CD4+ T cells specific to beryllium-modified (Be-modified) peptides derived from these chemokines. Here, we generated HLA-DP2 transgenic (Tg) CCL3-deficient mice (CCL3-/-) that also lack CCL4 to investigate their role in disease development. Be-exposed CCL3-/- mice maintained normal numbers of lung macrophages and dendritic cells (DCs) but exhibited significantly reduced total and HLA-DP2-CCL/Be tetramer-specific CD4+ T cells, IFN-γ-producing CD4+ T

Systemic chronic beryllium disease: beyond lung disease.

OBJECTIVE: Systemic involvement in chronic beryllium disease (CBD) has been reported only anecdotally. METHODS: We describe a case series of six patients with CBD demonstrating systemic involvement across diverse occupational settings. RESULTS: All patients had documented occupational beryllium exposure. CBD diagnosis was confirmed by positive histology and lymphocyte proliferation test in all cases. All patients presented with signs of extrapulmonary organ involvement, including hepatic, gastric, neurologic, splenic, ocular, cutaneous, as well as possible cardiac and renal involvement. CON

[Bronchoalveolar lavage: a current overview].

Bronchoalveolar lavage (BAL) is a minimally invasive procedure frequently used in the evaluation of pulmonary diseases. The primary indications for its use include the diagnosis of inflammation, infection, and interstitial lung diseases. Additionally, BAL plays a role in tumor diagnostics, detection of rejection following lung transplantation, and the diagnosis of occupational lung diseases (e.g., berylliosis, asbestosis).Although BAL alone mostly does not enable a definitive diagnosis, analysis of the BAL fluid provides important findings that are essential when interpreted in the clinical co

Parameterization of the CHARMM force field for divalent cations (Ca2+/Be2+) binding to anionic lipids with isothermal titration calorimetry.

Beryllium toxicity manifests as a rare lung disease, berylliosis, characterized by persistent granulomas. Studies indicate that, in the presence of Be2+, macrophage phagocytosis is markedly reduced, potentially hindering the removal of apoptotic cells. Although the exact mechanism by which Be2+ ions reduce phagocytosis is unclear, one hypothesis suggests that these small ions outcompete Ca2+ in critical ion-lipid and lipid-protein interactions mediating lipid recognition. To explore these interactions at the atomic level using molecular dynamics (MD), we aim to improve existing MD force fields

Structural basis for Mg²⁺-glycine synergy in the tandem glycine riboswitch revealed by cryo-EM.

Part I of this article discussed the potential functional importance of genetic mutations and alleles of the human cytochrome P450 2D6 (CYP2D6) gene. The impact of CYP2D6 polymorphisms on the clearance of and response to a series of cardiovascular drugs was addressed. Since CYP2D6 plays a major role in the metabolism of a large number of other drugs, Part II of the article highlights the impact of CYP2D6 polymorphisms on the response to other groups of clinically used drugs. Although clinical studies have observed a gene-dose effect for some tricyclic antidepressants, it is difficult to establ

Interleukin-1 signaling and CD4+ T cells control B cell recruitment to the lungs in chronic beryllium disease.

Chronic beryllium disease (CBD) is a debilitating pulmonary disorder that occurs due to persistent exposure to beryllium (Be) particles in the workplace. Be-exposure causes activation of the innate immune system, resulting in the secretion of interleukins and chemokines that drive the accumulation of B and T cells in the lungs. However, the mechanisms by which innate molecules influence the recruitment of B cells and B cell-mediated protection in CBD are poorly understood. In this study, we employed multiple approaches to examine the role of innate immune signaling and CD4+ T cells in B cell r

Beryllium Lymphocyte Proliferation Test: Differential Diagnosis of Sarcoidosis and Chronic Beryllium Disease.

BACKGROUND: Chronic beryllium disease (CBD) is considered a phenocopy of sarcoidosis, generally caused by occupational exposure to beryllium. Its diagnosis relies on the demonstration of beryllium sensitization by the beryllium lymphocyte proliferation test (BeLPT). RESEARCH QUESTION: How well does the BeLPT discriminate between CBD and sarcoidosis in patients with suspected beryllium exposure and what are the clinical characteristics of these 2 groups? STUDY DESIGN AND METHODS: BeLPT results and clinical characteristics of patients with suspected beryllium exposure were retrospectively anal

Genome-wide association study and HLA genotyping for beryllium disease susceptibility in a European descent population.

BACKGROUND: Workplace exposure to beryllium can result in beryllium sensitization (BeS), a cell-mediated immune response that can progress to chronic beryllium disease (CBD), a granulomatous lung disease. DPB1-E69 is highly associated with CBD and BeS, although DRB1-E71 may also be a risk factor in the absence of DPB1-E69. OBJECTIVE: This study 1) identified novel genetic variants associated with CBD/BeS using a genome-wide association study (GWAS) approach and 2) clarified the role of DRB1-E71 in conjunction with DPB1-E69. METHODS: We performed GWAS and HLA analysis on 1626 subjects with Be

Additional diagnostic potential of transbronchial lung cryobiopsy in bronchoscopic assessment of sarcoidosis and chronic beryllium disease: a retrospective analysis of 321 patients.

INTRODUCTION: Non-necrotising granulomas are the histological hallmark of sarcoidosis and chronic beryllium disease (CBD). Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA), transbronchial lung forceps biopsy (TBLB) and endobronchial biopsy (EBB) are often used as standard methods for obtaining suitable tissue. Transbronchial lung cryobiopsy (TBLC) is a novel biopsy technique well studied in different fibrosing lung diseases; however, data on its use in sarcoidosis are scarce. This study aimed to assess whether TBLC provides additional diagnostic value compared with

Multisystemic Beryllium Disease: An Exceptional Case Revealed by a Urinary Tract Granulomatosis.

Chronic beryllium disease (CBD), or berylliosis, is an interstitial lung disease caused by the chronic inhalation of finely particulate beryllium, frequently mistaken for sarcoidosis. It is rarely associated with skin nodular lesions, asymptomatic granulomatous hepatitis or calcium nephrolithiasis. To date, it has never been reported as a diffused multi-organ granulomatous disease. A 60-year-old Pakistani man, a former excavation worker with ancient history of suspected sarcoidosis, underwent a left nephroureterectomy for suspected papillary kidney carcinoma. The histopathological analysis sho

Sarcoidosis in Beryllium Exposed Workers: A Case-Case Study.

BACKGROUND: Despite the utility of the beryllium lymphocyte proliferation test (BeLPT), distinguishing sarcoidosis, a disease of unknown etiology, from chronic beryllium disease (CBD), has long posed a diagnostic challenge. It is unclear if beryllium-exposed sarcoidosis cases (Be-exp-Sarc) are clinically distinct from CBD, or are misdiagnosed cases of CBD. METHODS: We performed a case-case study of 40 beryllium-exposed individuals diagnosed with Be-exp-Sarc compared to 40 frequency-matched CBD cases. We compared demographics, exposure, clinical, physical, and radiographic characteristics and
