Progressive Amyloid-Related Imaging Abnormalities After Discontinuation of Lecanemab in Early Alzheimer Disease in an APOE ε2/ε4 Carrier.

BACKGROUND: Amyloid-related imaging abnormalities (ARIA) are the most relevant adverse events associated with anti-amyloid therapies in Alzheimer's disease. METHODS: We report a case of progressive ARIA after discontinuation of lecanemab in a patient with early-onset Alzheimer's disease carrying the rare APOE ε2/ε4 genotype. RESULTS: After two infusions, mild ARIA-edema (ARIA-E) and ARIA-hemorrhage (ARIA-H) were detected on routine MRI, prompting treatment interruption. Despite partial radiologic regression, follow-up MRI 8 weeks after discontinuation demonstrated marked progression with mul

Circulating microRNAs reflect hemorrhagic MRI markers in cerebral amyloid angiopathy.

BACKGROUND: Cerebral amyloid angiopathy is a major cause of intracerebral hemorrhage in older individuals. Although cerebral microbleeds (CMBs) and cortical superficial siderosis (cSS) aid MRI diagnosis they offer limited pathophysiological insight. MicroRNAs (miRs) regulate microvascular function and may serve as noninvasive biomarkers for distinguishing CAA subtypes and assessing hemorrhagic risk. This study investigated circulating miR profiles in probable CAA and Dutch-type hereditary CAA and examined their relationship with hemorrhagic MRI markers. METHODS: Small RNA sequencing was perfo

Prevalence and impact of cerebral small vessel lesions in mild cognitive impairment and mild dementia due to Alzheimer's disease patients in memory clinic: Preparation for anti-amyloid therapy in Thailand.

BackgroundAlzheimer's disease (AD) raises global concern with its impact on daily living. Anti-amyloid therapy (AAT) are specific treatments used in mild cognitive impairment (MCI) and mild dementia stage of AD (mild AD). Severe cerebral small vessel disease (SVD) lesions such as microbleeds, superficial siderosis, lacunes, white matter hyperintensities are listed as exclusions criteria for AAT. In Thailand, there is no prevalence study of SVD lesions specifically in these patient groups.ObjectiveThe purpose of this study is to report the prevalence of SVD burdens which are excluded in AAT, in

Centrum Semiovale Perivascular Spaces Predict First Hemorrhagic Lesion in Asymptomatic Amyloid-Positive Individuals.

OBJECTIVE: Identification of cerebral amyloid angiopathy (CAA) before its hemorrhagic manifestations remains limited. We aimed to determine whether severe enlarged perivascular spaces in the centrum semiovale (EPVS-CSO) identify a prehemorrhagic stage of sporadic CAA by predicting the first appearance of CAA-related hemorrhagic lesions or incident cognitive decline in amyloid-positive, cognitively unimpaired, hemorrhage-free older adults. METHODS: We performed a secondary analysis of the Anti-Amyloid Treatment in Asymptomatic Alzheimer's Disease (A4) trial, a phase 3 randomized study in cogni

Alpha-1 Antitrypsin PiMZ Heterozygosity in the Setting of Alcohol-Associated Cirrhosis: A Case Report.

Alpha-1 antitrypsin deficiency (AATD) is classically associated with the PiZZ genotype, while PiMZ heterozygosity has historically been considered a benign carrier state. Emerging evidence, however, suggests PiMZ status functions as a clinically significant cofactor for liver fibrosis when combined with metabolic or environmental stressors such as alcohol use. We present the case of a 57-year-old woman with a diagnosis of cirrhosis of unclear etiology and a remote history of alcohol use, who was admitted with jaundice and profound hyponatremia. Workup for viral, autoimmune, and hereditary hemo

Cerebral Small Vessel Disease and Risk of Cardiovascular Events and All-Cause Mortality: The Framingham Heart Study.

BACKGROUND: The effect of neurovascular health on cardiovascular outcomes is not well-known. Cerebral small vessel disease (CSVD) signals subclinical vascular brain injury and its quantification may serve as a tool to stratify risk in healthy individuals. Studying the relations of CSVD to cardiovascular events and mortality could aid in targeted preventive efforts. METHODS: We included Framingham Heart Study participants free of diagnosed stroke and dementia. CSVD was assessed on brain MRI?considering covert infarcts, cerebral microbleeds, cortical superficial siderosis, high perivascular spa

Vascular β-amyloid potentially colocalises with phosphorylated tau in cerebral amyloid angiopathy.

BACKGROUND: Tau pathology is observed in cerebral amyloid angiopathy (CAA) and is related to cognitive impairment and neurodegeneration. However, the relationship between tau pathology and amyloid accumulation in the vasculature is unknown. We aimed to assess if regional associations exist between vascular amyloid and tau protein in sporadic CAA. METHOD: We assessed cerebral amyloid and hyperphosphorylated tau in patients with probable CAA or Alzheimer's disease (AD) using 11C-Pittsburgh compound B (PiB) and 18F-T807 positron emission tomography (PET). PET data for each region of interest wer

Accelerated MRI Sequences for Intracranial Hemorrhage Screening.

OBJECTIVES: Anti-amyloid-beta immunotherapy requires frequent MRI screening for amyloid-related imaging abnormalities-hemorrhage subtype (ARIA-H), consisting of cerebral microbleeds (CMB) and/or superficial siderosis (SS), using gradient-recalled echo (GRE) or susceptibility-weighted imaging (SWI). Screening MRI sequences for ARIA-H may benefit from acceleration to maximize patient enrollment by increased throughput and reduced motion degradation. This study assessed the diagnostic performance of standard GRE and SWI to echo-planar imaging (EPI) accelerated substitutions for detecting CMB and

Artificial Intelligence-Assisted Detection of Amyloid-Related Imaging Abnormalities: Promise and Pitfalls.

The advent of anti-amyloid therapies (AATs) for Alzheimer disease (AD) has elevated the importance of MRI surveillance for amyloid-related imaging abnormalities (ARIA) such as microhemorrhages and siderosis (ARIA-H) and edema (ARIA-E). We report a literature review and early quality-assurance experience with an FDA-cleared decision support artificial intelligence (AI) tool intended for detection of ARIA in MRI clinical workflows. The AI system improved the sensitivity for detection of subtle ARIA-E and ARIA-H lesions but at the cost of a reduction in specificity. We propose a tiered workflow c

Superficial siderosis in Marfan syndrome.

A 49-year-old woman presented with a dorsal column-predominant sensory myelopathy secondary to superficial siderosis. She had Marfan syndrome and was found to have dural ectasia. Superficial siderosis results from subpial haemosiderin deposition due to chronic subarachnoid space bleeding, often from a dural pathology, such as a tear. Dural ectasia is common in Marfan syndrome, but superficial siderosis is not. Gait ataxia, often with hearing impairment, is a common clinical presentation of superficial siderosis. Patients may develop myelopathy, here being a pure sensory myelopathy with predomi

The Storm Behind the Silence: Detecting Cerebral Amyloid Angiopathy-Related Inflammation in a Patient with Dementia.

Cerebral amyloid angiopathy-related inflammation (CAA-RI) is a rare, treatable inflammatory variant of cerebral amyloid angiopathy that often presents with acute or subacute cognitive or behavioral decline. We describe a 76-year-old man with frontotemporal dementia who was admitted for an episode of unresponsiveness following several weeks of worsening agitation. MRI demonstrated bifrontal vasogenic edema, cortical superficial siderosis, and microhemorrhages, consistent with CAA-RI. The patient was treated with high-dose intravenous corticosteroids, followed by a prolonged taper, with clinical

Meningovascular Inflammation in Cerebral Amyloid Angiopathy-Related Cortical Superficial Siderosis.

The role of inflammation in cortical superficial siderosis (cSS), a marker of cerebral amyloid angiopathy (CAA) linked to high hemorrhage risk, is unclear. We examined 15 patients with cSS using 3 T post-contrast vessel wall MRI (VWI) and CSF analysis. Although only 27% met current CAA-ri criteria, 93% showed vessel wall enhancement or sulcal hyperintensities near cSS, frequently extending beyond. Seven patients with follow-up VWI demonstrated corticosteroid-responsive regression of inflammation. CSF albumin quotients, indicating blood-brain barrier dysfunction, correlated with MRI inflammatio

A regional framework for the detection and management of ARIA with anti-amyloid therapies in early Alzheimer's disease in Asia.

Alzheimer's disease (AD) is a growing public health concern in Asia, with an increasing prevalence driven by demographic shifts and rising life expectancy. The introduction of anti-amyloid monoclonal antibodies such as lecanemab and donanemab marks a pivotal transition from symptomatic management of AD to disease-modifying approaches, but their clinical use requires careful monitoring for amyloid-related imaging abnormalities (ARIA), a key safety consideration that presents either as vasogenic edema or as microhemorrhages and superficial siderosis. ARIA has been observed in varying frequencies

Imaging and fluid biomarkers for prognostic stratification in cerebral amyloid angiopathy.

Cerebral amyloid angiopathy (CAA) is a common small vessel disease characterized by Aβ deposition in cortical and leptomeningeal arteries, leading to lobar intracerebral hemorrhage and vascular cognitive impairment. Despite advances in diagnosis, prognosis remains highly heterogeneous, encompassing risks of recurrent hemorrhage and progressive cognitive decline. This review summarizes recent developments in imaging and fluid biomarkers for prognostic stratification in CAA. Imaging markers, including advanced MRI and molecular PET techniques, have evolved from traditional hemorrhagic indicators

Cerebral microbleed patterns and the risk of incident dementia in elderly adults: The ARIC study.

OBJECTIVE: The association between cerebral microbleeds (CMBs), an indicator of microvascular damage on MRI, and incident dementia (ID) remains inconclusive. We aim to investigate the interplay of CMB presence, number, and location with ID in a large community-based elderly cohort. METHODS: The study included 1532 dementia-free participants (aged ≥ 65 years) derived from baseline examinations (2012-2013) of the Atherosclerosis Risk in Communities Neurocognitive Study (ARIC-NCS). The ID cases were diagnosed at visits 6 (2016-2017), 7 (2018-2019), and 8 (2019-2020). Cox proportional-hazards mod
