Comparison of SWI with T2* in the Diagnosis of Cerebral Amyloid Angiopathy Using ARIA Scoring and Boston Criteria 2.0 at 7T MRI.

BACKGROUND AND PURPOSE: 7T-MRI enables detection of subtle hemorrhagic biomarkers associated with cerebral amyloid angiopathy (CAA). However, the relative performance of T2* compared with susceptibility-weighted imaging (SWI), as well as the applicability of Boston Criteria 2.0 at 7T, remains uncertain. This study evaluated diagnostic agreement between T2* and SWI, interobserver variability, and the utility of the Boston Criteria 2.0 at 7T-MRI and characterized the prevalence of emerging imaging biomarkers, such as the intragyral hemorrhage sign (IGHS). MATERIALS AND METHODS: In this retrospe

When sulcal enhancement is not leptomeningeal metastasis: diagnostic value of T1-T2FLAIR matching for differentiating benign contrast leakage.

The incidence and imaging characteristics of non-neoplastic sulcal enhancement on contrast-enhanced T2-weighted fluid-attenuated inversion recovery (CE-T2FLAIR) imaging in routine neuro-oncologic magnetic resonance imaging (MRI) were investigated. Contrast-enhanced T1-T2FLAIR sequence matching was used to differentiate benign contrast leakage from leptomeningeal metastasis (LMS). This retrospective, single-center study included 337 patients who underwent contrast-enhanced brain MRI for evaluation of suspected intracranial metastasis. Sulcal enhancement on CE-T2FLAIR was classified as absent, c

Conversion of 3 T liver, spleen, pancreas, and kidney R2* measurements to 1.5 T R2* equivalents: Validation of a theoretical framework.

Iron quantification of the liver and heart at 1.5 T have become the standard of care for patients with transfusional siderosis [1]. However some institutions must rely on higher field systems to perform iron quantitation. While studies performed at both field strengths have cross-calibrated measurements for liver and heart, a similar validation has not been performed for other abdominal organs. The purpose of this study was to validate a formula to convert 3 T abdominal R2* measurements to 1.5 T R2* equivalents; we hypothesized similar accuracy in spleen, kidney, and pancreas as previously sho

Spatiotemporal Associations Between Cortical Microinfarcts and Cortical Superficial Siderosis in Cerebral Amyloid Angiopathy.

BACKGROUND AND OBJECTIVES: Cerebral amyloid angiopathy (CAA) is a small vessel disease characterized by hemorrhagic and nonhemorrhagic brain injuries. While cortical cerebral microinfarcts (CMI) have been associated with cortical superficial siderosis (cSS), the mechanisms linking these lesions remain unknown. This study aims to determine the spatiotemporal relationship between cortical CMI and cSS in CAA, with the hypothesis that cSS can promote local ischemic injury. METHODS: Patients with probable CAA (Boston Criteria v2.0) without previous symptomatic intracerebral hemorrhage or CAA-relat

Cerebral amyloid angiopathy-related inflammation (CAA-ri): an updated systematic review and meta-analysis.

BACKGROUND: Cerebral amyloid angiopathy-related inflammation (CAA-ri) is a rare subtype of CAA, and it is correlated with pathological evidence of inflammation against amyloid-β in the walls of blood vessels and the surrounding tissue. Limited data exist on the prevalence of clinical, neuroimaging, and genetic characteristics in CAA-ri. METHOD: A systematic review and meta-analysis pooled data from published studies on CAA-ri. Random-effects models were used to calculate pooled prevalence rates, and heterogeneity was assessed using I2 and τ2 statistics. RESULTS: We identified 4 prospective a

Pharmacological management of transient focal neurological episodes in cerebral amyloid angiopathy: a systematic review.

Cerebral amyloid angiopathy (CAA) is associated with transient focal neurological episodes (TFNEs), which are brief, stereotyped symptoms. As these may occur regularly, pharmacological treatment of TFNEs is needed but remains poorly investigated. To systematically evaluate the available evidence on the use of pharmacological treatment to reduce the frequency, severity, or duration of TFNEs in patients with CAA. A systematic literature search of PubMed and Embase was conducted. Eligible studies included observational studies, interventional trials, and case reports or series that assessed pharm

Role of Low-Field Portable MRI in Small Vessel Disease Identification.

Cerebral small vessel disease (SVD) is the most common age-related brain pathology. Magnetic resonance imaging (MRI) is the reference standard for identifying SVD biomarkers, including white matter hyperintensity, recent small subcortical infarcts, lacunes, perivascular spaces, cerebral microbleeds, and cortical superficial siderosis. However, SVD is usually detected only when patients undergo conventional high-field MRI, limiting early identification, longitudinal monitoring, and equitable access to brain health evaluation. Low-field portable MRI systems (<0.1T) offer point-of-care acquisitio

Managing Antithrombotic Treatment in Intracerebral Hemorrhage and Ischemic Stroke Due to Cerebral Amyloid Angiopathy With Supporting Pathology.

Cerebral amyloid angiopathy (CAA) is a small vessel disease marked by β-amyloid deposition within cortical and leptomeningeal vessels. It is associated with a range of hemorrhagic manifestations, including lobar intracerebral hemorrhage (ICH), cortical superficial siderosis, and convexity subarachnoid hemorrhage (cSAH). In addition to hemorrhagic complications, cerebral ischemic events may occur due to impaired vascular reactivity and reduced cerebral perfusion. The coexistence of ischemic and hemorrhagic strokes in patients with biopsy-confirmed CAA poses significant diagnostic and therapeuti

Diverging Clinical and Amyloid Positron Emission Tomography Features in Iatrogenic Cerebral Amyloid Angiopathy.

Amyloid positron emission tomography (PET) features of cadaveric dural graft-associated iatrogenic cerebral amyloid angiopathy (iCAA) remain unclear. A 53-year-old man presented with forgetfulness and personality changes 41 years postgrafting. Cognitive test results revealed memory impairments; magnetic resonance imaging revealed medial temporal lobe atrophy, multiple microbleeds, and superficial siderosis; and amyloid PET revealed focal occipital uptake. Although focal amyloid accumulation associated with cognitive impairment has not been reported in patients with iCAA, decreased cerebrospina

Superficial siderosis caused by a tiny dural defect localized by an isodense crossing point on CT myelography: illustrative case.

BACKGROUND: Superficial siderosis (SS) is a rare disease characterized by hemosiderin deposition on the brain or spinal surface, potentially leading to cerebellar ataxia, hearing loss, and dementia. For SS caused by spinal dural defects, surgical closure prevents clinical progression; however, precise localization of very small defects remains challenging with conventional imaging modalities. OBSERVATIONS: A 47-year-old male with progressive SS was diagnosed with CSF leakage from a ventral spinal dural defect, but the lesion could not be localized clearly by MRI. CT myelography focusing on th

Cerebral Microbleeds and Superficial Siderosis: Vascular Correlates and Cognition in a Memory Clinic.

OBJECTIVE: Cerebral microbleeds (CMBs) and cortical superficial siderosis (cSS) are increasingly recognized in patients with cognitive impairment and are considered markers of cerebral small vessel disease and cerebral amyloid angiopathy (CAA). However, their associated variable and cognitive impact remain under-characterized in memory clinic populations. METHODS: We retrospectively reviewed 2923 patients referred for cognitive complaints who underwent brain MRI at the University Health Network (Toronto, 2014-2022). Of these, 137 patients with CMBs and/or cSS were compared with 278 control pa

Activating Cannabinoid receptor 2 alleviates iron overload-induced liver damage via dual modulation of STAT3/hepcidin and Nrf2/FPN1 pathways.

Iron overload-related liver injury is a major clinical problem in hereditary hemochromatosis, transfusional siderosis, and chronic liver diseases, yet current treatments such as iron chelation therapy and phlebotomy are limited by safety and efficacy issues. Cannabinoid receptor type 2 (CB2R) has been implicated in the regulation of iron metabolism, but its role and underlying mechanism in iron overload remain unclear. Here, we investigated whether CB2R can prevent liver damage caused by iron overload and explored its potential mechanism. Male C57BL/6J mice received 100 mg/kg iron-dextran intr

Reversible Subacute Liver Failure Following Revisional Malabsorptive Bariatric Surgery: A Case Report.

Bariatric surgery is an effective treatment for obesity and metabolic dysfunction-associated steatotic liver disease (MASLD). Although most patients experience improvement in hepatic steatosis and fibrosis after surgery, severe hepatic decompensation has rarely been reported following malabsorptive procedures. Early recognition of potentially reversible causes of liver failure is essential to prevent progression to irreversible hepatic injury or liver transplantation. We report the case of a 54-year-old woman with obesity-related MASLD who underwent revisional single-anastomosis duodeno-ileal
