Fulminant Leptomeningitis Without Hemorrhagic Lesions as an Atypical Initial Presentation of Cerebral Amyloid Angiopathy-Related Inflammation.

Cerebral amyloid angiopathy related inflammation (CAA-ri) is a rare neuroinflammatory condition that may present with a wide spectrum of clinical and radiographic findings. Classic imaging features include areas of asymmetric subcortical white matter hyperintensities due to vasogenic edema, together with the classic hemorrhagic lesions of cerebral amyloid angiopathy (CAA) including microhemorrhages, superficial siderosis, or convexity subarachnoid hemorrhage. We describe a case of CAA-ri appearing as fulminant leptomeningitis without other obvious radiographic features of CAA at presentation.

Dynamic imaging markers of incident microhemorrhages and superficial siderosis in the A4 study: A longitudinal person-interval analysis.

INTRODUCTION: Incident microhemorrhages and superficial siderosis are hemorrhagic magnetic resonance imaging (MRI) abnormalities relevant to amyloid-related imaging abnormalities with hemosiderin deposition (ARIA-H) risk stratification, but evidence exploring their short-term dynamic predictors remains limited. METHODS: We analyzed longitudinal MRI data from the A4 study, constructing a discrete person-interval dataset for modeling the short-term risk of incident hemorrhagic MRI abnormalities. Models incorporated baseline covariates, alongside dynamic variables of recent microhemorrhage accum

SWI versus T2*GRE for ARIA-H monitoring: a pragmatic perspective on adoption in clinical practice.

Although radiological severity thresholds for amyloid-related imaging abnormalities with hemosiderin deposition (ARIA-H) were developed using 2-dimensional acquisitions of T2*-weighted gradient-recalled echo (T2*GRE) MRI in pivotal anti-amyloid trials, adoption of well established 3D susceptibility-weighted imaging (SWI) in ARIA-H monitoring in clinical practice represents a reasonable long-term direction for clinical practice. SWI provides documented higher sensitivity and comparable or superior inter-rater reliability compared with conventional T2*GRE for detecting cerebral microbleeds and c

Evaluation of Disease Severity Using CSF Biomarkers in Patients With Probable Cerebral Amyloid Angiopathy.

BACKGROUND AND OBJECTIVES: Cerebral amyloid angiopathy (CAA) is currently diagnosed using MRI-based Boston criteria. However, imaging markers represent downstream consequences of vascular injury and may not fully reflect disease severity. CSF biomarkers, particularly β-amyloid 1-42 (Aβ42) and β-amyloid 1-40 (Aβ40), may provide a more direct measure of vascular amyloid burden. We investigated the association between CSF biomarkers and hemorrhagic and nonhemorrhagic MRI markers of disease severity in probable CAA. METHODS: We conducted a retrospective multicenter cohort study including consecut

EXPRESS: Covert cerebral small vessel disease and outcomes after spontaneous intracerebral hemorrhage.

The prognostic implications of covert cerebral small vessel disease (SVD) in spontaneous intracerebral hemorrhage (ICH) remain unclear. We investigated associations of SVD markers with hematoma severity and functional outcomes in patients with ICH. SVD markers (white matter hyperintensities [WMH], cerebral microbleeds [CMBs], lacunes, and cortical superficial siderosis [cSS]) were assessed. Outcomes included hematoma volume, hematoma expansion, and disability or death (mRS4-6 at 90 days and 1 year). Of 1,660 patients who underwent MRI (mean age:70±13years, 43% female), 397 had lobar (143 CAA-r

Imaging for anti-amyloid therapy eligibility: Comparing GRE and SWI in detecting microhemorrhages and superficial siderosis.

INTRODUCTION: Anti-amyloid therapies such as lecanemab require magnetic resonance imaging (MRI) screening to exclude patients with > 4 gradient-recalled echo (GRE)-detected microhemorrhages or any superficial siderosis (SS). Although GRE and susceptibility-weighted imaging (SWI) are both accepted, they differ in detection performance. METHODS: We retrospectively analyzed 224 eligibility MRIs with paired GRE and SWI from a single academic health system. Six neuroradiologists assessed a 20-scan reliability subset; the remaining 204 scans were assigned to one rater for hemorrhagic lesion detecti

A clinically interpretable nomogram for predicting discharge outcomes in cerebral amyloid angiopathy-related lobar intracerebral hemorrhage: development and internal validation.

BACKGROUND: Prognostic models for cerebral amyloid angiopathy (CAA)-related lobar intracerebral hemorrhage (ICH) remain limited. We aimed to develop and internally validate an interpretable nomogram for predicting unfavorable functional outcome at discharge. METHODS: We retrospectively included 124 patients aged ≥50 years with lobar ICH who were diagnosed with CAA according to the Boston criteria version 2.0. Discharge outcome was classified as favorable (modified Rankin Scale score 0-2) or unfavorable (3-6). The model was developed using Boruta feature screening, AIC-guided logistic regressi

Clinical and neuroimaging features, and associations with early post-stroke cognition in patients with intracerebral hemorrhage in Taiwan.

BACKGROUND: East Asians have a higher incidence rate of intracerebral hemorrhage (ICH). As clinical and neuroimaging characteristics, genetic and vascular risk factors, and prognosis might vary between populations, we aimed to make comparisons between different ICH locations and etiologies, and to investigate the factors associated with early post-stroke cognition in Chinese patients in Taiwan. METHODS: We prospectively recruited acute spontaneous Chinese ICH patients from two large Hospitals in Taiwan. For included patients, we compared major vascular risk factors, APOE genotypes, clinical a

Stroke recurrence after primary intracerebral haemorrhage: a population-based study of clinical and MRI factors associated with recurrence.

BACKGROUND: Survivors of primary intracerebral haemorrhage (ICH) face competing risks of recurrent ICH and ischaemic stroke. We quantified ischaemic stroke recurrence (ISR) and haemorrhagic stroke recurrence (HSR) and examined associated imaging factors. METHODS: We analysed 30-day survivors of primary ICH in the Brest Stroke Registry (2008-2020). Cumulative incidence was estimated with death as a competing event. Cause-specific Cox models were fitted in the cohort and MRI subgroup. RESULTS: Among 806 survivors, 118 experienced recurrence: 437 died without recurrence. The overall incidence r

Associations Between Cerebral Microbleed Patterns and Incident Dementia: The ARIC-Neurocognitive Study.

BACKGROUND: Cerebral microbleeds (CMBs) are associated with dementia, but the impact of specific microbleed patterns (with distinct pathophysiologies) is unclear. Lobar CMBs commonly result from cerebral amyloid angiopathy (sometimes with cortical superficial siderosis [cSS]), while subcortical microbleeds indicate a hypertensive cause. This study investigates the association of CMB presence, patterns, and frequency with dementia risk. METHODS: Participants from the ARIC-NCS (Atherosclerosis Risk in Communities-Neurocognitive Study), a community-based longitudinal cohort, with a 3T research m

Plasma fractionation in Latin America: a path towards sovereignty and sustainability in the supply of plasma-derived medicines.

PURPOSE: Increased reactive oxygen species (ROS) released by NADPH oxidase and inflammation are associated with arterial hypertension and eye diseases associated with high blood pressure, including glaucoma, retinopathies (e.g., age-related macular degeneration), and choroidopathies affecting ocular function; however, the mechanisms underlying these adverse outcomes remain undefined. The present study was designed to highlight the importance of oxidative stress in severe hypertension-related eye damage. METHODS: Male Wistar rats (n = 7, unless otherwise specified for specific experiments) wer

Prevalence of vestibulo-ocular reflex dysfunction in people with neurological disorders: a systematic review and meta-analysis.

BACKGROUND: Neurological disorders, a leading cause of global disability, often cause debilitating dizziness and imbalance. While subjective symptoms are well-documented, the actual prevalence of vestibulo-ocular reflex (VOR) dysfunction in patients with central nervous system (CNS) damage remains unclear due to inconsistent primary studies. This research aims to determine the prevalence of VOR gain dysfunction, as measured by the video Head Impulse Test (vHIT), across neurological disorders. METHODS: Our systematic review searched MEDLINE, CENTRAL, CINAHL, Scopus, ClinicalTrials.gov, and the

Quantitative evaluation of iron chelator effects on central motor and sensory tracts in superficial siderosis.

BACKGROUND: Superficial siderosis (SS) is a rare neurodegenerative disease characterized by hemosiderin deposition in the central nervous system, leading to progressive neuronal degeneration. Although myelopathy is a main clinical manifestation in SS, objective biomarkers for monitoring disease progression and treatment remain unclarified. We assessed electrophysiology of central motor and sensory conduction in patients with SS before and after iron chelator administration. METHODS: In this controlled trial, we evaluated 12 patients with SS (mean follow-up: 33.6 months [range: 18.9-36.1]), wh

Gene therapy in transfusion-dependent non-β0/β0 genotype β-thalassemia: first real-world experience of beti-cel.

Gene addition and editing strategies for transfusion-dependent β-thalassemia have gained momentum as potentially curative treatment options, with studies showcasing their efficacy and safety. We report, to our knowledge, the first real-world application of betibeglogene autotemcel (beti-cel; Zynteglo) during its period of active license in Europe from January 2020 to March 2022 for patients aged ≥12 years without a β0/β0 genotype and without a HLA-matched sibling donor, before beti-cel marketing authorization was withdrawn by its holder because of nonsafety reasons. Among 15 screened patients,

Amyloid-related imaging abnormalities: manifestations, metrics and mechanisms.

Three monoclonal antibodies directed against specific forms of the amyloid-β (Aβ) peptide have been granted accelerated or traditional approval by the FDA as treatments for Alzheimer disease, representing the first step towards bringing disease-modifying treatments for this disease into clinical practice. Here, we review the detection, underlying pathophysiological mechanisms and clinical implications of amyloid-related imaging abnormalities (ARIA), the most impactful adverse effect of anti-Aβ immunotherapy. ARIA appears as regions of oedema or effusions (ARIA-E) in brain parenchyma or sulci o
