Occupational health research in India.

India being a developing nation is faced with traditional public health problems like communicable diseases, malnutrition, poor environmental sanitation and inadequate medical care. However, globalization and rapid industrial growth in the last few years has resulted in emergence of occupational health related issues. Agriculture (cultivators i.e. land owners + agriculture labourers) is the main occupation in India giving employment to about 58% of the people. The major occupational diseases/morbidity of concern in India are silicosis, musculo-skeletal injuries, coal workers' pneumoconiosis, c

The measurement and health impact of endotoxin contamination in organic dusts from multiple sources: focus on the cotton industry.

Endotoxin is derived from Gram-negative bacterial membranes, and its inflammatory effects following inhalation are well characterized. The significance of this fact becomes apparent when the wide-ranging environments containing high levels of this microbial product are considered. Endotoxin is present in numerous industrial environments, especially where organic fibers are processed. Microbial contamination of these fibers mainly occurs at the agricultural stage. Materials such as flax and hemp are affected in this way, but the most important product in this context is cotton, from which chron

Diffuse lung disease caused by cotton fibre inhalation but distinct from byssinosis.

A 66 year old man had inhaled cotton fibre for 50 years at his workplace. He did not have any respiratory symptoms. Chest CT scans revealed diffuse centrilobular and peribronchovascular interstitial thickening. Lung biopsy specimens confirmed the presence of string-like foreign bodies as well as granulomas and fibrosis in the peribronchial region. Infrared spectrophotometry confirmed that the foreign bodies were composed of natural cellulose. This is the first study to show directly by examination of biopsy samples that cotton fibre inhalation can cause diffuse lung disease. The clinical featu

[One-week variation of cotton dust and endotoxin levels in a cotton mill. Relation with the daily variation of the expiratory flow rates].

UNLABELLED: The prevalence of respiratory occupational diseases in the textile industry depends on environmental factors such cotton dust (CTD) levels and gram negative bacteria endotoxin (EDX) contamination. OBJECTIVE: To assess the environmental conditions (CTD and EDX) during one week in a cotton mill with respect to FEV1 variation throughout the work shift. POPULATION AND METHODS: 47 cotton mill workers (24F, 23M); average age: 42 +/- 11 years; average exposure to cotton dust: 22.5 +/- 9.4 years; Asthma: 15 (32%), Byssinosis: 11 (23%) and Asymptomatics: 21 (45%); a past history of respir

Airborne endotoxin and its relationship to pulmonary function among workers in an Indian jute mill.

Air samples from various processing areas of an Indian jute mill were examined for endotoxin. The authors assessed work-related respiratory symptoms and pulmonary function changes in the dust-exposed workers from the different processing areas using a standard questionnaire and spirometry. Endotoxin was estimated in water extract of jute dust from 3 milling areas, and in outside air, by the Limulus amebocyte lysate gel clot technique. The batching, spinning, and weaving areas of the jute mill showed endotoxin levels of 0.22-4.42 microg/m3, 0.04-1.47 microg/m3, and 0.01-0.07 microg/m3, respecti

A longitudinal observation of early pulmonary responses to cotton dust.

AIMS: To examine early adverse pulmonary effects of exposure to cotton dust, and to identify potential risk factors, including atopy for pulmonary responses to cotton dust. METHODS: Spirometry, methacholine challenge testing, and questionnaire; performed among 101 non-smoking newly hired textile workers at baseline (prior to starting work), and at 3, 12, and 18 months after starting work. Concentrations of airborne cotton dust in various work areas were measured at each follow up survey using vertical elutriators. RESULTS: The incidence of non-specific respiratory symptoms was 8% at three mo

Bactericidal treatment of raw cotton as the method of byssinosis prevention.

In early studies, research to control byssinosis focused on methods to reduce the trash in the textile mill environment. Dust control has been effective in reducing the prevalence of byssinosis, but simple reduction in dust levels does not always assure its prevention. Also, bacteria and fungi present in cotton do not in themselves cause byssinosis, but the endotoxins-heat-stable lipopolysaccharide-protein complexes contained in the cell wall of Gram-negative bacteria-are responsible for the development of this respiratory disease of workers on cotton, flax, and some other fibers. Experimental

Respiratory effects of long-term exposure to cotton dust.

Traditionally, the concept of cotton dust-related disease focused on acute airway responses. A possible connection of long-term exposure to cotton dust with chronic obstructive airway disease was not addressed until the past two decades. This review summarizes the latest findings relevant to this topic that were observed from longitudinal cohort studies and discusses the relation between acute airway responses and chronic losses of lung function.

Occupational health nursing in Thailand. Insight into international occupational health.

1. As a country that is both agricultural by nature, and moving toward new industrialization, Thailand is increasingly facing work related health problems. Major occupational illnesses are pesticide poisoning, silicosis, byssinosis, lead poisoning, and occupational injuries. 2. Although the demand for occupational health services is high, occupational health services in Thailand are still not comprehensive. 3. Because of the lack of role perception and inadequate preparation, Thai occupational health nurses have limited functions in providing occupational health services. The services are prim

Additive effect of smoking and cotton dust exposure on respiratory symptoms and pulmonary function of cotton textile workers.

One hundred and sixty-nine and 175 cotton textile workers (CTWs) were enrolled in the first (1991) and second (1996) surveys to investigate the prevalence of byssinosis. The synergistic effect of smoking on cotton dust exposure was also evaluated. Although the difference in prevalence of abnormal pulmonary function between the first (38.5%) and second study (38.9%) was not statistically significant, smokers had significantly higher frequency than nonsmokers in both surveys. A significant trend existed between the cotton dust levels and the frequency of abnormal lung function. The significant t

[The role of the "Clinica del Lavoro Luigi Devoto" in international research].

During its long history, the Clinica del Lavoro Luigi Devoto has produced numerous internationally recognized scientific contributions in the specific field of occupational medicine and health, and in other broader areas as, for example, immunology, toxicology, cytogenetics. Pivotal in this activity was the figure of Enrico C. Vigliani, director of the Clinica for over 35 years. We mention here, in particular, the results of the study of "early" markers of lead intoxication; the research on the carcinogenicity of benzene and aromatic amines; the study of the pathogenesis of silicosis and byssi

Byssinosis among male textile workers in Pondicherry: a case-control study.

BACKGROUND: In India, 20 million workers are involved in the manufacturing of textiles. However, there are few epidemiological studies from India that have assessed the magnitude or the risk factors associated with byssinosis. In Pondicherry, textile mills have been in existence for over a century. This case-control study aimed to find the factors associated with the development of byssinosis in textile workers. METHODS: The sample consisted of 761 men above the age of 30 years who had worked for at least 10 years in a textile factory. All the respondents were interviewed by a pretested quest

Byssinosis among jute mill workers.

Although byssinosis in jute mill workers remains controversial, studies in a few jute mills in West-Bengal, India, revealed typical byssinotic syndrome associated with acute changes in FEV1 on the first working day after rest. The present study on 148 jute mill workers is reported to confirm the occurrence of byssinosis in jute mill workers. Work related respiratory symptoms; acute and chronic pulmonary function changes among exposed workers were studied on the basis of standard questionnaire and spirometric method along with dust level, particle mass size distributions and gram-negative bacte

Mondays without dread: the Trade Union response to byssinosis in the Lancashire cotton industry in the twentieth century.

Trade unions have often been criticized for their failure to address occupational health issues. This article explores their response to byssinosis-a chronic respiratory disease caused by exposure to cotton dust that was rife in the Lancashire cotton industry in the early nineteenth and twentieth centuries. Using the archives of the cardroom and spinning unions, it is demonstrated that trade union efforts to combat byssinosis began before the First World War and were sustained for over 70 years. During that period, byssinosis became a recognized medical condition and a compensatable disease, d

Respiratory symptoms and cotton dust exposure; results of a 15 year follow up observation.

AIMS: To determine chronic effects of long term exposure to cotton dust and endotoxin on incidence of respiratory symptoms and the effect of cessation of exposure. METHODS: Respiratory health in 429 Chinese cotton textile workers (study group) and 449 silk textile workers (control group) was followed prospectively from 1981 to 1996. Byssinosis, chest tightness, and non-specific respiratory symptoms were assessed by means of identical standardised questionnaires at four time points. Exposures to cotton dust and endotoxin were estimated using area samples collected at each survey. Incidence and
