A questionnaire survey of 1005 consecutive attenders at four outpatient clinics yielded 117 (12%) budgerigar fanciers (exposed to budgerigars- known in North America as parakeets-for at least three months) and 296 (29%) former fanciers. Twnety had precipitins to budgerigar serum or droppings or both, and 10 of these together with 39 precipitin-negative patients reported undue breathlessness on exertion during exposure to buderigars. Precipitin responses to different avian serum antigens occur in bird fancier's lung (BFL) and coeliac disease. Failure to distinguish between them could encourage an erroneous diagnosis of BFL in patients with coeliac disease, and the recent suggestion that these two disorders are strongly associated may be questioned, partly for this reason. Thirty specimens of lung obtained by Abram's needle biopsy were examined using direct immunofluorescence. In 2 cases of Goodpasture's syndrome, linear deposits of IgG were demonstrated in alveolar walls. Diffuse deposits of IgG were found in the alveolar spaces of one patient with pigeon fancier's lung. Two subjects with cryptogenic fibrosing alveolitis had dense granular deposits of IgG in alveolar walls. The agglutination of avian red cells, particularly pigeon and budgerigar cells, by sera from patients who had been exposed to birds and had respiratory symptoms, as well as from healthy bird fanciers, has been used to detect the presence of avian specific antibody. Agglutination tests performed by standard techniques using bird erythrocytes can be read within 45 minutes. One hundred and sixty-five sera were tested, and the 44 which were precipitin positive gave a positive agglutination test with one exception. HLA antigen status was assessed in non-related individuals and related family members with bird breeder's hypersensitivity pneumonitis (HP). 35% of 20 non-related patients carried HLABW40 compared to 10% of 861 normal controls (p less than 0.001, Pc less than 0.05, respectively). A consistent association of an HLA haplotype with disease was demonstrated in three of four families. Selected HLA antisera contained antibodies which blocked the blastogenic response to bird antigen. This study suggests the existence of an HLA associated Ir gene in bird breeder's HP.

False positive raised complement fixation titres to egg-grown virus preparations were found in six pigeon fanciers suffering from extrinsic allergic alveolitis but not in six similar budgerigar fanciers. The raised titres in the pigeon fanciers fell fourfold over a short interval in the absence of avian exposure and were attributable to antibodies directed against antigens from hen's egg in which the test virus preparations were grown. The so-called interstitial pulmonary diseases are considered diseases of the pulmonary parenchyma and discussed according to different pathogenesis and etiology. The disturbance of function is emphasized. On the examples of allergic alveolitis, fibrosing alveolitis, drug damage to the parenchyma and shock-lung the clinical appearances of diseases of pulmonary parenchyma are discussed. Diagnostic methods are described and the causes of faulty interpretation are examined.

Approximately 6% of the dry organic dust from pigeon coops was extracted in the form of soluble, nondialyzable components. These pigeon dropping extracts contain a number of stable protein and glycoprotein antigens together with pigments and other components that cause nonspecific precipitation of normal and immune sera. Based on their migration in immunoelectrophoresis, the antigens have been designated PDEB, 1, 2, 3 or 4.
