Hypersensitivity pneumonitis caused by Penicillium citrinum, not Enoki spores.

BACKGROUND: Flammulina velutipes is called the Enoki mushroom in Japanese and is cultivated indoors. Mushroom workers face occupational exposure to a tremendous number of fungi and organic antigens capable of causing hypersensitivity pneumonitis (HP). One worker employed at an Enoki farm developed HP due to Penicillium citrinum. This study investigated new cases of HP among the workers cultivating Enoki. METHODS: Serum Krebs von der Lungen-6 (KL-6), surfactant protein (SP)-A and SP-D were measured. Lymphocyte stimulation tests (LST) and double immunodiffusion tests (DIT) were performed to ide

[Recent advances in extrinsic allergic alveolitis].

Extrinsic allergic alveolitis (hypersensitivity pneumonitis), especially humidifier lung, has been more frequently diagnosed over the last decades, whereas farmer's lung has decreased over the same time period. Today two types of the chronic course of extrinsic allergic alveolitis can be distinguished. The recurrent chronic course with a good prognosis may be differentiated from the insidious course with a poor prognosis by means of different histological patterns (UIP, NSIP, BOOP pattern). The characteristic neutrophilic infiltration of the lung in the insidious course cannot be detected by b

Sepsis due to gestational psittacosis: A multidisciplinary approach within a perinatological center--review of reported cases.

Chlamydia psittaci is associated with significant morbidity and mortality during pregnancy, and its rarity can delay early diagnosis and treatment. A farmer's wife presented at 31 weeks with febrile illness and developed signs of septic shock, indicating immediate delivery. The child developed uneventfully. The mother survived after symptomatic mechanical ventilation, including extracorporeal lung assistance, for 11 days due to multi-organ failure. Only two weeks after admission antibody titres against Chlamydia were rising. The placenta demonstrated acute intervillositis and destruction of th

Hypersensitivity pneumonitis induced by spores of Penicillium citrinum in a worker cultivating Enoki mushroom.

A 47-year-old Japanese woman was admitted to our hospital with a 2-week history of dry cough and shortness of breath. She had been engaged in Enoki mushroom production for 22 years. Chest X-ray and chest computed tomography (CT) scan showed bilateral fine-nodular shadows and ground glass opacity. Bronchoalveolar lavage fluid demonstrated an increase of total cell counts with predominant lymphocytosis. Pathological specimens obtained by video-assisted thoracoscopic surgery revealed alveolitis and noncaseating granuloma with giant cells. Lymphocyte stimulation test showed positive responses with

Hypersensitivity pneumonitis.

Hypersensitivity pneumonitis (HP) is a pulmonary disease with symptoms of dyspnea and cough resulting from the inhalation of an antigen to which the subject has been previously sensitized. The incidence of HP is unknown. A population-based study estimated the annual incidence of interstitial lung diseases as 30:100,000 and HP accounted for less than 2% of these cases. The diagnosis of HP can often be made or rejected with confidence, especially in areas of high or low prevalence respectively, using simple diagnostic criteria. Chest X-rays may be normal in active HP; High Resolution Computed To

Farmer's lung is now in decline.

Farmer's lung incidence in Ireland was constant until 1996, even though hay making methods were revolutionised in late 1980's. We undertook this study to find out the incidence of farmer's lung in Ireland from 1982-2002 and its correlation with rainfall and the effect of changing farm practices. The primary cases of farmer's lung were identified from Hospital in Patients Enquiry (HIPE) unit of the national Economic & Social Research Institute (ESRI) Dublin. Rainfall data were obtained from Met Eireann whereas population, hay production and silage production were obtained from the Central Stati

Occupational respiratory health of New Zealand horse trainers.

OBJECTIVES: To confirm that working with horses is an occupational respiratory hazard and observed associations are not attributable to confounding. METHODS: A postal survey of the respiratory health of 659 horse trainers and a comparison group of 506 vegetable growers was conducted. Data were analysed using logistic regression. RESULTS: Relative to vegetable growers, horse trainers reported higher rates of chronic bronchitis [odds ratio (OR) = 6.8, 95% confidence interval (CI): 2.2-21.4] and organic dust toxic syndrome/farmers' lung (ODTS/FL) (OR = 3.5, 95% CI: 1.8-6.8). Pesticides and adult respiratory outcomes in the agricultural health study.

In the 1700s, Bernardino Ramazzini was among the first to describe respiratory disease among agricultural workers. Since then, farmers continue to have higher rates of respiratory illnesses, even as changes occur in occupational and environmental exposures on farms. While grain and animal exposures have been well studied for their role in agricultural lung diseases, pesticide exposures have not. Using the Agricultural Health Study, a prospective cohort study of approximately 89,000 licensed pesticide applicators and their spouses in Iowa and North Carolina, we are currently assessing the assoc

Pesticides and other agricultural factors associated with self-reported farmer's lung among farm residents in the Agricultural Health Study.

BACKGROUND: Farmer's lung, or hypersensitivity pneumonitis, is an important contributor to respiratory morbidity among farmers. METHODS: Using the 1993-7 enrolment data from the Agricultural Health Study, we conducted a cross-sectional study of occupational risk factors for farmer's lung among 50,000 farmers and farm spouses in Iowa and North Carolina using hierarchical logistic regression controlling for age, state, and smoking status. Participants provided information on agricultural exposures, demographic characteristics, and medical history via self-administered questionnaires. Approximat

Impact of agricultural practices on microbiology of hay, silage and flour on Finnish and French farms.

Exposure to microorganisms in farm environments may cause respiratory disorders, e.g. asthma, organic dust toxic syndrome and allergic alveolitis. By reducing microbiological deterioration of organic materials, some agricultural practices have a protective effect. Microbiological analyses were carried out on hay, silage and flour samples (n=107) from farms in Finland and France (n=23) that use different methods of haymaking. High concentrations of Absidia corymbifera were found in approximately 35 % of French hay samples and only 10 % of Finnish hay samples. Concentrations of Eurotium spp. wer

Hypersensitivity pneumonitis associated with mushroom worker's lung: an update on the clinical significance of the importation of exotic mushroom varieties.

Hypersensitivity pneumonitis remains an important industrial disease in mushroom workers. It has a significant morbidity, and early diagnosis and removal from exposure to the antigen are critically important in its management. Recently, several new allergens have been described, particularly those from mushroom species originating in the Far East, which are of clinical significance to workers occupationally exposed to such allergens in cultivation, picking, and packing of commercial mushroom crops. Importing of exotic mushrooms including Shiitake is common in EU countries, and some of the exot

[Hypersensitivity Pneumonitis (extrinsic allergic alveolitis)].

Farmer's lung was first described in 1932. We can define hypersensitivity pneumonitis as a pulmonary and systemic disease that is accompanied by dyspnoea and coughing; it is caused by an immunological type of inflammation of the alveolar walls and the terminal airways and it is secondary to the repeated inhalation of a variety of antigens by a susceptible host. It can be said that it is an underdiagnosed disease and only a high degree of clinical manifestations and a detailed history of exposure can lead to an early diagnosis and satisfactory treatment. A combination among clinical-radiologica

[Risk factors and their role in development of respiratory diseases among workers of contemporary battery farms].

The authors presented materials on respiratory diseases among workers of contemporary battery farms. Findings are that occupational dust of animal and plant origin is a major risk factor in respiratory diseases development. Clinical, physiologic and immunologic studies proved genesis of chronic nonspecific lung diseases.

Concentrations of dust, allergens and endotoxin in stables, living rooms and mattresses from cattle farmers in southern Bavaria.

Agricultural work is considered to be a major risk factor for occupational diseases. In particular, allergic reactions to cow dander cause numerous cases of airway disorders. We measured the concentration of allergens (e.g. Bos d2, Der p1) and endotoxin in the stables, living-rooms and mattresses of 46 farmers with a diagnosis of occupational asthma or allergic rhinitis caused by cow dander allergen. The concentration of cow dander allergen was highest in stables (median 20,400 microg/g) but also noticeable in dust samples from living-rooms (median 155 microg/g) and mattresses (median 195 micr

[An unusual cause of pulmonary cysts].

INTRODUCTION: Multiple pathologies may be associated with cystic appearances in the lungs. Lymphangioleiomyomatosis, Langerhans cell histiocytosis and lymphocytic interstitial pneumonia are the principal causes of thin walled pulmonary cysts. CASE REPORT: We report the case of a non-smoking woman of 71 years, a retired farmer, who was admitted on account of cough, increasing dyspnoea, fever and loss of weight that developed following exposure to hay. The thoracic CT scan revealed diffuse ground glass opacities and centrilobular nodules. Multiple thin walled cystic shadows were distributed hom

Future directions in the pharmacologic therapy of chronic obstructive pulmonary disease.

Current therapy for chronic obstructive pulmonary disease (COPD) fails to alter its relentless progression. This remains a significant challenge and unmet need. A recent advance is the demonstration that treatment with a fixed dose of an inhaled corticosteroid and a long-acting beta2-agonist in COPD improves lung function and quality of life, and reduces exacerbation more effectively than either drug alone. Other improvements include the introduction of tiotropium, a once-daily anticholinergic. In advanced clinical development are other once-daily bronchodilators and combinations of anticholin

[Farmer's lung disease caused by straw or hay? Case report].

A young woman was admitted to Akranes Regional Hospital because of dyspnea, fatigue and fever. She was found to have bilateral pneumonia but etiology was not found. She was treated with antibiotics with good resolution and was discharged after eight days from the hospital. Four weeks later she noticed rapidly progressive dyspnea and was found to be hypoxemic, and to have restrictive spirometry and diffuse interstitial changes on chest radiography. Computerized tomography of the lungs showed diffuse ground glass changes. Transbronchial biopsies from the lungs showed numerous small granulomas. S

Farmer's lung disease and microbiological composition of hay: a case-control study.

Previous studies performed in France have suggested that handling hay contaminated with high amounts of moulds, and especially Absidia corymbifera and Eurotium amstelodami, may favour farmer's lung disease. The circumstances favouring farmer's lung disease and the distinctive microbiological composition of hay samples that provoke attacks need to be specified. We present a case-control study which investigates the agricultural practices and the microbiological composition of hay handled in patients with farmer's lung disease as compared to those of a representative control population. Isolated melanoma in the lung where there is no known primary site: metastatic disease or primary lung tumour?

Patients with no history of melanoma occasionally present with apparently metastatic melanoma in the lungs, but have no evidence of a primary melanoma. The aims of this study were to investigate the role of surgical resection in the treatment of such patients, and to examine the evidence for a diagnosis of primary pulmonary melanoma in each case. Patients with an unknown primary melanoma who presented with pulmonary disease and subsequently underwent surgical resection were identified from the Sydney Melanoma Unit database. Fifteen patients fulfilled the study criteria. Multiple lesions were p
