Understanding histotripsy ablation safety: a systematic review of pre-clinical and clinical adverse events.

BACKGROUND: Facet joint tropism is asymmetry in orientation of the bilateral facets. Some studies have shown that tropism may increase the risk of disc degeneration and herniations, as well as degenerative spondylolisthesis (DS). It remains controversial whether tropism is a pre-existing developmental phenomena or secondary to progressive remodeling of the joint structure due to degenerative changes. As such, the following study addressed the occurrence of tropism of the lower lumbar spine (i.e. L3-S1) in a degenerative spondylolisthesis patient model. METHODS: An international, multi-center

Review of NIOSH Cannabis-Related Health Hazard Evaluations and Research.

Since 2004, the National Institute for Occupational Safety and Health (NIOSH) has received 10 cannabis-related health hazard evaluation (HHE) investigation requests from law enforcement agencies (n = 5), state-approved cannabis grow operations (n = 4), and a coroner's office (n = 1). Earlier requests concerned potential illicit drug exposures (including cannabis) during law enforcement activities and criminal investigations. Most recently HHE requests have involved state-approved grow operations with potential occupational exposures during commercial cannabis production for medicinal and non-m

Dusting off the numbers of in situ particle analysis: a 35-year experience.

A variety of fibrotic lung diseases are caused by the inhalation of organic dusts. Many of these disorders have distinctive histopathology and can be readily diagnosed by routine histopathologic examination. However, in some instances, there is overlap in morphology between diseases caused by dust inhalation (mineral pneumoconiosis) and other lung diseases. In such cases, analytical scanning electron microscopy (SEM) can provide valuable information to assist the pathologist in making the correct diagnosis. We report herein our findings in 96 cases in which in situ particle analysis (ISPA) was

Lung fibrosis and exposure to wood dusts: Two case reports and review of the literature.

INTRODUCTION: Increasing evidence suggests that idiopathic pulmonary fibrosis (IPF) occurs more often in subjects previously exposed to wood dusts than in non-exposed subjects. Here we report 2 cases of the IPF among workers prolongedly exposed to high levels of hardwood dusts. CASE REPORT: The case No. 1: An 83 year-old male former smoker, retired joiner developed mild dyspnoea and chronic dry cough over the period preceding the examination. Pulmonary function tests (PFT) identified a mild restrictive pattern and diffusion capacity for carbon dioxide (CO₂) that was severely impaired (57% of

Pigeon hypersensitivity pneumonitis: immunohistochemical demonstration of the causative antigen in the lung.

A number of clinicopathological manifestations may define the presence of hypersensitivity pneumonitis. Histological study is used to establish the diagnosis and to differentiate the disease from other respiratory disorders. This case report suggests that immunohistological demonstration of the causative antigen in the lung may be a useful diagnostic approach in cases of pigeon hypersensitivity pneumonitis. A 52 year-old woman was studied. She had a prior history of pigeon exposure, and lived in an area with a high prevalence of tuberculosis. Her clinical presentation, respiratory function tes

Bronchoalveolar lavage in occupational lung diseases.

Occupational lung diseases (OLDs) are related to the exposure and inhalation of organic, inorganic, and synthetic particles, fumes, gases, or infectious agents. From the long list of OLDs this article focuses the discussion on bronchoalveolar lavage (BAL) in parenchymal immunoinflammatory conditions, such as hypersensitivity pneumonitis (HP) and pneumoconiosis. Several antigens may cause HP, including products of plant or animal origin, aerosolized microorganisms, and organic chemicals. BAL is used not only to assess the pathogenesis of these diseases but also to identify the typical pattern o

Working Group report 3: sensitivity to organic dusts--atopy and gene polymorphisms.

Working Group 3 (Sensitivity to Organic Dusts-Atopy and Gene Polymorphisms) was convened to review the current understanding of how effects of inhaled organic dust may be modified by genetic factors-both those that increase as well as those that may reduce susceptibility. Furthermore, the group was asked to suggest areas that require more investigation in this field. The discussion focused on individual sensitivity to inhaled agents as the most important determinant of inter-individual heterogeneiety in responses to exposures. Genetic modifiers are known for a number of pathologic conditions (

Dust-induced interstitial lung disease in the tropics.

Inhalation of dusts is an important cause of interstitial lung disease in the tropical countries such as India. While dusts of organic origin, such as the cotton dust causing byssinosis, generally cause bronchial or bronchiolar involvement and hypersensitivity pneumonitis, inorganic metallic dusts cause progressive pulmonary fibrosis. Silicosis, coal workers' pneumoconiosis, and asbestosis are the three most commonly recognized forms of pneumoconiotic pulmonary fibrosis. Pulmonary tuberculosis is an important complication seen in up to 50% of patients of silicosis in some reports from India. T

Stipatosis or hypersensitivity pneumonitis caused by esparto (Stipa tenacissima) fibers.

Hypersensitivity pneumonitis or extrinsic allergic alveolitis may be defined as an immunological pulmonary disease caused by a variety of antigens reaching the lungs through inhaled organic and inorganic dusts derived from different sources, although they are usually occupational. Farmer's lung and pigeon breeder's lung are probably the most well-known types of hypersensitivity pneumonitis worldwide. Esparto grass (Stipa tenacissima), which is a grammineous plant which is commonly found in the Mediterranean countries, has a wide variety of uses. Esparto fiber is used for the manufacturing of r

[Pathogenesis and treatment of hypersensitivity pneumonitis].

Hypersensitivity pneumonitis (HP) is a granulomatous interstitial lung disease resulting from an immunologic reaction to the repeated inhalation of organic dusts and active chemicals. There are 50 or more groups of HP, but the prevalence varies from country to country, and even within a country, depending on a variety of occupational or environmental inhalants. In Western coutries farmer's lung, bird fancier's disease, humidifier lung, and air-conditioner disease are common, but in Japan summer-type HP is the most prevalent group. Summer-type HP is a house-related illnes induced by Trichosporo

[Immunopathogenesis of extrinsic allergic alveolitis].

Inhalation of a variety of organic dusts may cause the onset of hypersensitivity pneumonitis (HP) finally leading to irreversible pulmonary fibrosis in some individuals. So far, the pathogenesis of HP remains partially unclear. Besides patient-related factors this is probably attributable to the complex composition of the causative dusts: in addition to specific antigens that may induce type III and type IV reactions they contain a variety of additional components like particles and toxins with the ability to promote several antigen-independent reactions. During an acute episode of HP a marked

Epidemiology of health and safety risks in agriculture and related industries. Practical applications for rural physicians.

Epidemiologic studies document that work in the agricultural sector is associated with many occupational health hazards. Exposure to organic dusts and airborne microorganisms and their toxins may lead to respiratory disorders. The burden of exposure-related chronic bronchitis, asthma, hypersensitivity pneumonitis, organic-dust toxic syndrome, and chronic airflow limitation can be diminished by appropriate preventive measures. The contribution of exposures to agricultural chemicals to cancers and neurodegenerative disorders is being investigated. Some studies document that farmers and those in

Organic dust exposures from compost handling: case presentation and respiratory exposure assessment.

Inhalation of dust from contaminated organic materials may result in acute respiratory tract illness. Possible mechanisms include toxic and cellular reactions to microbial and other organic products or immunologic responses after prior sensitization to an antigen. A case is presented of a 52 year old male who developed fever, myalgia, and marked dyspnea 12 hr after shoveling composted wood chips and leaves. Inspiratory crackles, hypoxemia, and bilateral patchy pulmonary infiltrates were seen. Precipitating antibody tests for the usual antigens were inconclusive. He improved over 3 days. In ord

Unifying concepts underlying the effects of organic dust exposures.

Defense mechanisms protect the lung very well from inhaled organic dusts. With sufficient exposure to certain dusts, however, susceptible individuals develop hypersensitivity pneumonitis (HP), the organic dust toxic syndrome (ODTS), or asthma. Mucous membrane irritation (MMI) bothers some individuals inhaling grain dust. Allergic asthma is caused by IgE-mediated immunologic responses to allergenic dust contaminants. ODTS can be explained by a nonimmunologic release of interleukin 1 (IL-1) and perhaps other endogenous pyrogens from alveolar macrophages by endotoxin or other ingredients of dusts

Health effects of organic dust exposure in dairy farmers.

Exposure to farm dust may cause asthma, rhinitis, allergic alveolitis, organic dust toxic syndrome, and chronic bronchitis. Febrile reactions to inhaled dusts are common and are usually of a toxic rather than an allergic type. Some evidence indicates that dairy farmers may have an increased prevalence of chronic bronchitis, allergic asthma, rhinitis, and chronic airflow limitation, but this has not been conclusively shown. Storage mites and cow dander are important allergens in dairy farming. Inflammation of airways may in part be caused by immediate or delayed hypersensitivity. Other mechanis

Agricultural exposures to organic dusts.

Occupational respiratory hazards in agriculture are varied and widespread, and farm workers have been found to have high rates of respiratory disability compared with other industrial sectors. The common denominator in most agricultural exposures is organic dust. The author reviews agricultural respiratory syndromes and their diseases, including acute febrile syndromes, asthma, hypersensitivity pneumonitis and chronic bronchitis and airway obstruction.

In vitro release of arachidonic acid and in vivo responses to respirable fractions of cotton dust.

It was considered that the fall in lung function seen after exposure to cotton dust may be attributable in part to the activity of arachidonic acid metabolites, such as leucotrienes as well as to the more established release of histamine by cotton dust. However, we found that cotton and barley dusts elicited poor release of arachidonic acid from an established macrophage like cell line compared with that observed with other organic dusts. In the experimental animal, pulmonary cellular responses to both cotton and barley dust were similar to those evoked by moldy hay and pigeon dropping dusts,

[Effect of pregnancy and the puerperium on the development of extrinsic allergic alveolitis].

Extrinsic Allergic Alveolitis (EAA) is a disease characterized by an exaggerated immune response to inhaled organic dusts that can result in alveolar, and occasionally bronchiolar, damage. This damage can disappear completely with the elimination of the antigen and/or treatment with anti-inflammatory agents. Nonetheless, an important group of these patients do not improve; the inflammatory reaction persists. With time, this results in the deposit of abnormal connective, tissue with respect to its quantity and quality, in the pulmonary interstitium destroying the normal parenchymal architecture
